As a Registered Nurse Case Manager (RNCM), you will:
Conduct In Person patient interviews and comprehensive physical assessments.
Oversee the implementation and ongoing assessment of the patient's plan of care through the management of home health aides, LPNs, RNs, and other caregivers.
Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
Provide education to patients and families on proper home health care procedures. Ie. Wound care, IV administration, medication management.
Work to decrease readmissions by promoting preventative care and ensuring continuity of care.
To qualify as a Registered Nurse Case Manager (RNCM) with us, you will need:
Licensure: Current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position
Current CPR/AED/BLS/First Aid certification
Reliable transportation to/from care sites and/or work locations.
One (1) year of professional experience practicing as a Registered Nurse (RN) in home health or similar setting; previous case management/utilization review experience preferred.
OASIS experience preferred.
Practical trach and/or ventilator experience preferred, not required.

BASIC REQUIREMENTS for GS-7 and GS-9:
Degree: Physics; or related degree that included at least 24 semester hours in physics.
You may qualify if you meet one of the following:
1. GS-7: You must have completed or will complete a 4-year course of study leading to a bachelor's from an accredited institution AND must have documented Superior Academic Achievement (SAA) at the undergraduate level in the following:
a) Grade Point Average 2.95 or higher out of a possible 4.0 as recorded on your official transcript or as computed based on 4 years of education or as computed based on courses completed during the final 2 years of curriculum; OR 3.45 or higher out of a possible 4.0 based on the average of the required courses completed in your major field or the required courses in your major field completed during the final 2 years of your curriculum.
2. GS-9: You must have completed 2 years of progressively higher-level graduate education leading to a master's degree or equivalent graduate degree:

Essential Job Functions

Monitor sanitation practices and help ensure processes are followed to reduce food safety risks.

THE GROWTH YOU HELP PROVIDE AS A WAREHOUSE AND FLEET COORDINATOR
This is a full-time, on-site position with required overtime as needed. You will follow established daily, weekly, and monthly routines and attend scheduled meetings with various departments.
You will oversee all aspects of warehouse management, including receiving shipments, maintaining accurate inventory, and preparing materials for upcoming jobs. Your day will include communicating with Production, Sales, and Front Office teams, managing vendor relationships, and ensuring all vehicles and equipment are assigned and ready. You will be responsible for safety compliance checks, scheduling vehicle maintenance, and tracking inventory usage. Regular inventory audits and reports will be part of your routine, as well as overseeing warehouse organization and cleanliness. You will coordinate task lists, monitor job staging, and ensure all operational procedures are followed for efficient project support.

Qualifications:
High school diploma or equivalent
1+ year(s) of mechanical experience and warehousing or chimney-related experience
Ability to obtain CSIA, NCSG, or NFI certification within 6 months
