Are you an experienced and visionary medical professional looking to make a difference in your community? If so, we are seeking a Chief Medical Officer to join our team.
As the Chief Medical Officer, you will be responsible for overseeing and leading the clinical operations of our health centers, as well as providing direct care to our patients. You will work closely with our healthcare team which also includes behavioral health and pharmacy to provide medical oversight, develop and implement clinical policies and procedures, and ensure compliance with all regulatory and accreditation requirements.
In addition to your clinical responsibilities, you will also be responsible for developing and maintaining relationships with local healthcare providers and community partners. You will provide clinical supervision and mentorship to staff and collaborate with the executive team to develop and implement strategies to improve patient outcomes and access to care.

As a Clinical Safety Director, you will plan, coordinate, implement, evaluate and direct the infection control, immunization, radiation safety and other clinical safety programs at PHC. Oversight includes the preventative, operational, and administrative components of the programs to ensure high quality patient care and safety. Responsible for organizational compliance with health and safety regulations and standards, reduction or prevention of events, hazards, and risks in the areas of blood and body fluid exposure, infectious disease, sharps injury, instrument processing and sterilization, immunization safety, radiation safety, and environment of care. Work is completed in collaboration with impacted departments. The role ensures safety to PHC patients, visitors, and staff by serving as the organization's Infection Control Officer, Radiation Safety Officer, Patient Safety Officer, and immunization program manager. What You Will Do

Analyzes and reports surveillance findings and trends to leadership, utilizing the risk management reporting system. Promotes a culture of safety. Updates and effectively implements safety plans and policies.
Collaborates with interdisciplinary teams related to the scope of responsibility to maximize safety across the organization. Employs Lean process improvement principles and various tools to improve system efficiency.
Educates staff on principles and techniques of infection prevention. Serves as the Infection Control Officer.
Educates and guides staff on life safety and emergency management procedures, hazardous waste management, and radiation safety. Serves as the Radiation Safety Officer.
Provides oversight of PHC's immunization program. Serves as the chief point of contact for PHC's Vaccines for Children program, ensuring compliance with all program regulations. Collaborates with stakeholders to create processes and systems that ensure safe stewardship of vaccine inventory. Oversees the vaccine champion program within the clinics.
Performs quality assurance, rounding, and staff tracers ensuring compliance with infection control and safety expectations.
Ensures compliance with all regulatory agency requirements related to infection control and safety management along with other applicable requirements as required by local, state, and federal agencies and accrediting organizations.
Chairs the Infection Control and the Safety Committees, responsible for committee deliverables. Participates in PHC risk management activities.

Qualifications You Need to Bring
Required:
Current RN License recognized by the State of Iowa Board of Nursing.
A minimum of 5 years healthcare related experience with a minimum of 1 year infection prevention experience.
Knowledge of infection control, prevention, and safety regulations along with safety hazards and precautionary measures for a safe work environment in medical and dental clinics.
Proficient with computers, electronic health records, and Microsoft office applications.

The Case Manager I is responsible for assisting clients being served in one of PHC's social service programs (those who are living with HIV/AIDS) through needs assessment, case management, and advocacy with community partners. Collaborates with Director of Revenue Cycle to set priorities for excellent productivity. Monitors billing team work completion, identifies priorities, and assigns tasks to team members. Delegates specific tasks forwarded by COS/HBS staff for immediate resolution.
Works effectively to maintain billing rejects to a rate of less than 5% through audits and training, group and individual training.
Completes daily input of charges and ensures compliance with established goals for billing, including the timely submission of all clinic charges and daily editing, ensuring appropriate coding, pay codes, etc. Ensures compliance with all State and Federal patient financial service requirements, including, but not limited to patient-rights, confidentiality, and third-party billing.
Responds appropriately to inquiries on accounts by patients or third-party payors. Reviews and disseminates incoming correspondence, including mailed denials, within 48 hours of receipt.

As a Patient Services Specialist II, you will be responsible for ensuring a positive and seamless experience for patients and visitors within PHC medical and dental clinics. You will conduct financial screenings, assist with insurance enrollment (including Medicaid and Marketplace plans) and financial assistance programs, address billing questions, perform back-billing, monitor the payer mix, and support compliance. This role involves patient outreach, in-person support, and maintaining accurate documentation for financial assistance and billing. You will collaborate with billing and clinic staff to ensure timely and effective patient support. You will also provide support and coverage for the Patient Access Representatives.
In this position, you will work at PHC Marshalltown Clinic and be scheduled 40 hours per week, Monday - Friday 11 AM - 8 PM. You'll also work every other Saturday 9 AM - 1 PM. This position is eligible for our $5.00 per hour shift differential if you work a 3-hour shift either after 5 PM on weekdays or anytime on Saturdays. Our team members are occasionally needed to cover other shifts, which may include a Saturday morning, or to work at other PHC clinics in Ames, Des Moines, and Marshalltown. Mileage reimbursement and/or an out of town travel bonus may apply.
What You Will Do
Ensures a consistent revenue cycle process in medical and dental clinics by reviewing patient schedules to identify patients with upcoming appointments who need assessment for financial assistance, other available benefits, or payment plans.
Completes phone outreach to patients with past balances, expired insurance plans, needed documents for income verification or uninsured to screen and determine appropriate next steps for patients.
Meets with patients as needed to help them apply for Medicaid (including Hawk-I), Marketplace coverage, or other insurance programs. Conducts outreach and enrollment activities including annual Marketplace enrollment events. May provide assistance at various locations, including PHC clinics, community agencies.
Helps patients apply for the sliding fee program.
Maintains patient financial assistance files including electronic notes in registration and/or financial note files.
Completes necessary back-billing for applicable patients when insurance is identified or approved after the visit.
Answers patients' questions related to billing statements and collaborates with the billing department to support patients in resolving issues related to medical and/or dental bills.
Verifies and follows-up on patient credits and/or refunds that are identified through patient question or account verification processes.
Completes monthly laboratory and radiology billing.
Communicates with patients about anticipated cost of visit, if applicable. Creates and distributes good faith estimate letters in accordance with the No Surprises Act and upon patient request.
Monitors and reports on payer mix for their assigned clinic and works with Clinic Director to address unfavourable payer mix changes.
Provides ongoing support and coverage to the Patient Access Representative role.

Job Description
As the Residency Team Lead (RN / Regisered Nurse) you will provide leadership for the Patient Care Team (PCT) support staff and nursing care for patients. What You Will Do

As a Faculty Physician, you will be responsible for educating and supervising the Program's residents on a day to day basis within a 24/7 inpatient service. These physicians are responsible for the development of rotation objectives and coordination of didactics, including lectures, subspecialty board reviews, etc. Accepts residents who rotate in their inpatient or outpatient service to increase the resident's understanding of the practice and service line. Faculty Physicians work collaboratively with the Medical Education Department in establishing rotation objectives. Is responsible for providing patient care services to the Primary Health Care Inc (PHC) patient population in assigned clinic sites. Works collaboratively with a multidisciplinary team to provide, delegate, evaluate, and coordinate comprehensive professional care for patients of all cultures and backgrounds. Meets ever changing healthcare challenges while engaging patients in holistic care. Utilizes established workflows to ensure patient and staff satisfaction and organizational financial success. What You Will Do
Delivers direct patient care to ambulatory patients. Serves in rotation for after-hours call system.
Delivers high-quality, evidence-based medical care to adult patients in accordance with PHC standards, including history taking, physical exams, diagnostic testing, procedures, and prescribing.
Conducts patient evaluations via in-person visits and telemedicine, ensuring equitable, effective care across platforms.
Completes all required electronic medical record documentation-including progress notes, referrals, prescriptions, and coding-within PHC-designated timeframes.
Works closely with multidisciplinary team members to coordinate patient care, support workflow efficiency, and contribute to quality improvement initiatives.
Adheres to all local, state, and federal regulations; accreditation standards; and internal policies related to clinical care, risk management, and peer review.
Provides clinical oversight, consultation, and support to Physician Assistants and other advanced practice clinicians as appropriate.
Serves as a faculty educator for residents and medical students, facilitating clinical instruction and didactic teaching in alignment with ACGME competencies. Acts as a professional role model and fosters a supportive learning environment.

What You Will Do

As a Nurse Care Manager at The Project, you will play critical role in delivering comprehensive, patient-centered care to individuals living with HIV. In this role, you will focus on coordinating medical and psychosocial services to improve health outcomes and enhance quality of life for clients. You will serve as a primary point of contact, providing education, support, and advocacy to empower patients in managing their condition effectively. By collaborating closely with multidisciplinary teams, you will ensure seamless integration of care plans tailored to each patient's unique needs. Ultimately, this position aims to reduce barriers to care, promote adherence to treatment, and support long-term wellness within the community.
This position works at PHC's University Campus in Des Moines, typically Monday - Friday 8 AM - 5 PM.
What You Will Do
Supports and empowers clients throughout the HIV health care continuum to include holistic counseling, problem-solving “barriers to care” issues, and coaching clients as needed regarding treatment and medication adherence, healthy life choices, and self-help behaviors.
Provides care coordination to address management of co-morbidities to satisfy HIV health maintenance protocols.
Completes RW Part C new patient coordinated intakes accurately and timely, according to established coordinated care workflows.
Performs nursing assessment/medical triage of acute medical issues and medication side effects and refers or monitors appropriately.
Thoroughly and accurately documents all patient encounters in the electronic medical record and assists with data collection, data management and data integrity according to grant guidelines.
Provides HIV and STI education to patients appropriate to their identified learning considerations and needs.
Assists with scheduling and tracking of outpatient referrals for program patients.
Assists clients in accessing specialty medical, mental health, substance abuse and other support services through community agencies.
Qualifications You Need to Bring
Registered Nurse license recognized by the State of Iowa.
Associate Degree in Nursing.

Ability to consistently meet care manager measures and acceptable performance standards.
Must be obtained within 90 days of employment:
Basic Life Support for Healthcare Providers.
Mandatory Reporter Training for Child Abuse and Dependent Adult Abuse recognized by the State of Iowa.
AIDS Certified Registered Nurse certification required within 4 years or as funding allows.

Candidates with extensive related experience may be considered for a higher pay rate up to the pay range maximum of $77,400. Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.
Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC's Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.
As a Clinical Safety Director, you will plan, coordinate, implement, evaluate and direct the infection control, immunization, radiation safety and other clinical safety programs at PHC. Oversight includes the preventative, operational, and administrative components of the programs to ensure high quality patient care and safety. Responsible for organizational compliance with health and safety regulations and standards, reduction or prevention of events, hazards, and risks in the areas of blood and body fluid exposure, infectious disease, sharps injury, instrument processing and sterilization, immunization safety, radiation safety, and environment of care. Work is completed in collaboration with impacted departments. The role ensures safety to PHC patients, visitors, and staff by serving as the organization's Infection Control Officer, Radiation Safety Officer, Patient Safety Officer, and immunization program manager. The position demonstrates PHC's iCare values in daily work.

What You Will Do

Educates staff on principles and techniques of infection prevention. Serves as the Infection Control Officer.
Educates and guides staff on life safety and emergency management procedures, hazardous waste management, and radiation safety. Serves as the Radiation Safety Officer.
Provides oversight of PHC's immunization program. Serves as the chief point of contact for PHC's Vaccines for Children program, ensuring compliance with all program regulations. Collaborates with stakeholders to create processes and systems that ensure safe stewardship of vaccine inventory. Oversees the vaccine champion program within the clinics.
Performs quality assurance, rounding, and staff tracers ensuring compliance with infection control and safety expectations.
Ensures compliance with all regulatory agency requirements related to infection control and safety management along with other applicable requirements as required by local, state, and federal agencies and accrediting organizations.
Chairs the Infection Control and the Safety Committees, responsible for committee deliverables. Participates in PHC risk management activities.

Qualifications You Need to Bring
Required:
Current RN License recognized by the State of Iowa Board of Nursing.
A minimum of 5 years healthcare related experience with a minimum of 1 year infection prevention experience.
Knowledge of infection control, prevention, and safety regulations along with safety hazards and precautionary measures for a safe work environment in medical and dental clinics.
Proficient with computers, electronic health records, and Microsoft office applications.
Training development and facilitation skills incorporating adult learning principals.

As the Residency Team Lead (RN / Regisered Nurse) you will provide leadership for the Patient Care Team (PCT) support staff and nursing care for patients. You will ensure that the PCT follows assigned workflows efficiently and within the specialized residency education model. In this position, you will work collaboratively with a multidisciplinary team to welcome patients to a safe and inclusive environment and to engage patients in holistic care. This position is a unique role that offers the opportunity to use a wide variety of skills, including educating medical residents as they learn and grow throughout the residency program.
This position works at PHC at Mercy Family Medicine Residency Clinic in Des Moines and is typically scheduled to work 40 hours per week: Monday - Friday 8 AM - 5 PM. Our team members are occasionally needed to cover other shifts or to work at other PHC clinics in Ames, Des Moines, and Marshalltown. What You Will Do
Leads the Patient Care Team (PCT), ensuring efficient patient flow and completion of daily tasks. Assigns care team members in accordance with patient needs, along with staff capabilities and qualifications, to meet the needs of the clinic and residency education.
Coordinates clinical support staffing schedule, including reviewing time off requests and recommending approval/denial for time off requests to the Clinic Director. Assigns care team members in accordance with patient needs, staff capabilities, and qualifications. Monitors the effectiveness of the care given by MAs, LPNs, students, and apprentices.
Assist in finding fill-in staff and coordinating floating staff between locations as requested.
Helps plan, facilitate, and delegate daily staff huddles when the Clinic Director is not present. May support clinical support staff meetings with content and presentation of material.
Performs quality assurance reviews for life safety controls/tasks and duties assigned to clinical support staff.
Triages patients in-person or on the phone according to established protocol.
Serves as a resource to clinical support staff regarding clinical functions such as waived testing, vaccinations, standing orders, EMR Superuser, regulatory standards, and procedures. Assists clinical support staff as needed, including but not limited to, facilitating patient rooming and intake, obtaining vital signs, performing point-of-care, lab, and diagnostic testing per orders, and administering medications, immunizations, and treatments per orders to ensure efficient care team workflow.
Participates in training, orientation, competency assessment, and education of employees. Serves as a resource person to other staff and promotes effective collaboration. Provides feedback for MAs and LPNs as requested as part of the competency or evaluation process.
Serves as a patient advocate, resource, and point-of-contact for patients through the continuum of care.
Performs pre-cleaning and transport of contaminated instruments per infection control policies and procedures.
Performs other duties as assigned.
Qualifications You Need
Completion of an accredited nursing program accepted by the Iowa Board of Nursing for Registered Nurse licensure.
A minimum of 2 years of RN experience.
