Job Description
We are seeking a Diagnostic Medical Sonographer to join our night shift team. In this role, you will perform high-quality ultrasound imaging procedures, ensure patient safety, and support physicians in accurate diagnosis. The ideal candidate is self-motivated, detail-oriented, and demonstrates professionalism and compassion in patient care.
Responsibilities include:
Perform diagnostic ultrasound exams based on physician orders
Deliver high-quality imaging with proper positioning and documentation

Qualifications

Position Overview
A 280+ bed acute care hospital is seeking an experienced
RN Case Manager (Evenings)
to join the Case Management team. In this role, you will coordinate interdisciplinary care, conduct utilization review, determine appropriate level of care, and support safe, timely discharges across
ED, Med/Surg, Tele, Neuro, ICU, and PCU
populations.
You will report directly to the
Director of Case Management
.
Key Responsibilities
Complete comprehensive clinical and psychosocial assessments and develop criteria-based plans of care with clear discharge goals.
Lead interdisciplinary huddles and collaborate closely with physicians, nursing, therapy, pharmacy, and social work.
Conduct medical necessity reviews at admission and throughout hospitalization; escalate level-of-care issues as needed.
Manage observation status appropriateness and convert to inpatient status when indicated.
Perform concurrent/utilization reviews; document and communicate with third-party payers.
Identify barriers to throughput; resolve issues to reduce LOS and prevent avoidable days and readmissions.
Coordinate post-acute care services and connect patients/families with community resources.
Document case management interventions and track/trend variances; contribute to performance improvement initiatives.
Support compliance with regulatory standards and assist with staff education and protocol implementation.

3 years recent acute RN Case Management experience

In this role, you will coordinate interdisciplinary care, conduct utilization review, ensure appropriate patient status and level of care, and support safe, timely discharges across a variety of units including ED, Med/Surg, Telemetry, Neuro, ICU, and PCU.
This role reports directly to the Director of Case Management.
Key Responsibilities
Complete comprehensive clinical and psychosocial assessments; develop criteria-driven plans of care with clear discharge goals.
Lead interdisciplinary huddles and collaborate with physicians, RNs, therapy, pharmacy, and social work.
Conduct medical necessity reviews at admission and throughout hospitalization; escalate issues regarding patient status or level of care.
Manage observation status appropriateness and convert when indicated.
Perform concurrent/utilization reviews and communicate with third-party payers.
Identify barriers to care and throughput; remove obstacles to reduce LOS and prevent avoidable days or readmissions.
Coordinate post-acute care needs, including placement, home services, and community referrals.
Document case management interventions; track and trend variances and support performance improvement efforts.
Maintain compliance with regulatory, quality, and ethical standards; assist with staff education and protocol implementation.
QualificationsRequired
Active Florida RN license or Compact RN (must obtain FL license within 90 days if Compact).
3 years recent acute care RN Case Management experience
OR
3 years acute care RN experience (Med/Surg, Tele, Neuro, ICU, PCU, or ED) with strong UR/throughput skills.
Associate Degree or Diploma in Nursing.

Join a collaborative healthcare team as a Case Manager RN, coordinating care for acute care patients across the continuum. This role offers 12-hour evening shifts, rotating weekends, and opportunities to optimize patient outcomes while supporting resource management and quality care.
Responsibilities
Perform comprehensive assessments of patients medical and psychosocial needs
Develop, implement, and communicate individualized case management plans
Lead interdisciplinary team meetings to coordinate care and discharge planning
Evaluate admissions and observation patients for medical necessity and appropriateness
Conduct utilization management reviews and communicate with third-party payors
Act as liaison between patients, families, physicians, hospital staff, and external agencies
Document case management interventions and professional recommendations
Facilitate patient throughput and identify barriers to quality outcomes
Provide referrals to community resources and chronic disease management programs
Ensure compliance with regulatory requirements, policies, and quality initiatives
Identify high-risk patients and advocate for appropriate care and resources
Track trends in care barriers and implement action plans to improve processes
Qualifications
Required:
Minimum 3 years recent case management experience in acute care, Med/Surg, Telemetry, Neuro, ICU, PCU, ED, Home Health, or Insurance settings (must include 13 years acute care within last 5 years)
Associate Degree or Diploma in Nursing
Current Florida RN license or approved compact license (must obtain FL license within 90 days if compact)
Basic Cardiac Life Support (BCLS) certification
Preferred:
Bachelors Degree in Nursing (BSN)
Certification in Case Management, Nursing, or Utilization Review
Strong leadership and communication skills
Compensation & Benefits
Hourly pay: $31.94-$43.92 based on experience
Shift Differentials: Evening $2.50/hr, Weekend $2.00/hr (stackable for $4.50/hr on evening weekends)
$10,000 Sign-On Bonus paid in 5 installments over a 2-year commitment
