In this role, you will coordinate interdisciplinary care, conduct utilization review, determine appropriate level of care, and support safe, timely discharges across ED, Med/Surg, Tele, Neuro, ICU, and PCU populations. You will report directly to the Director of Case Management . Key Responsibilities Complete comprehensive clinical and psychosocial assessments and develop criteria-based plans of care with clear discharge goals. Lead interdisciplinary huddles and collaborate closely with physicians, nursing, therapy, pharmacy, and social work. Conduct medical necessity reviews at admission and throughout hospitalization; escalate level-of-care issues as needed. Responsibilities Perform comprehensive assessments of patients medical and psychosocial needs Develop, implement, and communicate individualized case management plans Lead interdisciplinary team meetings to coordinate care and discharge planning Evaluate admissions and observation patients for medical necessity and appropriateness Conduct utilization management reviews and communicate with third-party payors Act as liaison between patients, families, physicians, hospital staff, and external agencies Document case management interventions and professional recommendations Facilitate patient throughput and identify barriers to quality outcomes Provide referrals to community resources and chronic disease management programs Ensure compliance with regulatory requirements, policies, and quality initiatives Identify high-risk patients and advocate for appropriate care and resources Track trends in care barriers and implement action plans to improve processes Qualifications Required: Minimum 3 years recent case management experience in acute care, Med/Surg, Telemetry, Neuro, ICU, PCU, ED, Home Health, or Insurance settings (must include 13 years acute care within last 5 years) Associate Degree or Diploma in Nursing Current Florida RN license or approved compact license (must obtain FL license within 90 days if compact) Basic Cardiac Life Support (BCLS) certification Preferred: Bachelors Degree in Nursing (BSN) Certification in Case Management, Nursing, or Utilization Review Strong leadership and communication skills Compensation & Benefits Hourly pay: $31.94-$43.92 based on experience Shift Differentials: Evening $2.50/hr, Weekend $2.00/hr (stackable for $4.50/hr on evening weekends) $10,000 Sign-On Bonus paid in 5 installments over a 2-year commitment Relocation assistance: Case-by-case Comprehensive medical, dental, and vision coverage 401(k) plan with company match and Employee Stock Purchase Program Paid time off, holidays, and leave programs Tuition reimbursement, certification support, and professional development resources Employee wellness and counseling services Ideal For This role is ideal for experienced RNs passionate about care coordination, discharge planning, and patient advocacy.
