POSITION OVERVIEW The Desktop Support Specialist is a trained computer expert who provides technical support to users of computer software and hardware running the operating systems. Extensive experience with desktop hardware, software applications, operating systems and network connectivity is required. Will work closely with the Senior IT Service Support Manager when analyzing and making recommendations for hardware and software standardization. KEY ESSENTIAL FUNCTIONS Build, maintain, and implement desktop imagines for all different types of hardware/software. Drive to offsite locations in Western New York (Buffalo, Dunkirk, and Rochester). Document all pertinent end user identification information, including name, department, contact information, and nature of problem or issue. Prioritize, schedule, and administer all instances where enhancements and defect resolution are required. Perform hands-on fixes at the desktop level, including installing and upgrading software, installing hardware, implementing file backups, and configuring systems and applications. Record, track, and document the problem-solving process, including all successful and unsuccessful decisions made, and actions taken through to final resolution. Test fixes and perform post-resolution follow-ups to ensure problems have been adequately resolved. Maintain and enhance performance of all new and existing software and applications across the organization. Identify and learn appropriate software applications used and supported by the organization. Participate in the design, development, and delivery of software applications training programs and individual classes. Post software updates, drivers, knowledge bases, and frequently asked questions resources on company intranet to assist in problem resolution. Apply diagnostic utilities to aid in troubleshooting. Manage and/or provide guidance to junior members of the team. Evaluate documented resolutions and analyze trends for ways to prevent repeated future problems. REQUIREMENTS High school diploma plus relevant experience, required. Bachelor's degree in information technology, preferred. Minimum of 2 years' experience working in Information Technology with similar job duties. In-depth, hands-on knowledge of and experience with enterprise and desktop applications, and Operating Systems including Windows, MS Office Suite, etc. Proven experience with troubleshooting principles, methodologies, and issue resolution techniques. Able to develop and interpret technical and user-friendly language. Ability to effectively prioritize and execute tasks in a high-pressure environment. POSITION OVERVIEW The DOH ESSHI Housing Residential Aide will deliver support services to frail elderly/seniors who are formerly homeless or at risk of homelessness in community. The permanent supportive housing program's goals are to support tenants in maintain housing and thriving in the community as the age in place with an emphasis on health, safety, and overall welfare needs. The Residential Aide is expected to build rapport and maintain positive professional relationships with consumers and provide after hour and weekend support for residents, as well as assist the Case Manager. The Residential Aide is trained in incident reporting, crisis intervention and how to interact productively with residents. The Residential Aide also provides support and training for daily living skills and activities, provides supportive escorts for medical appointment/procedures and provides enhanced support for tenants identified as high risk. KEY ESSENTIAL FUNCTIONS Ensure the safety of the facility and the tenants. Ensure tenants are in basic adherence with Rules and Regulations, provide mild interventions, keep staff up to date to provide higher level interventions Provide all tenant information and daily report logs requested in a timely and accurate manner. Ensure facilities are maintained and repairs are followed up with to create a positive environment of care. This includes ensuring the staff areas and common areas are cleaned and organized and trash is emptied. Work orders to be forwarded to appropriate persons. Monitor the facilities, staff, and tenants to ensure that all after hours operations are within the Acacia expectations, regulations and contractual obligations. Provide on-site crisis intervention. Maintains appropriate documentation of all tenant's contacts in log. Handle special assignments, duties and functions as assigned. House Runs - regular presence in the lobby and on the residential floors. Maintains notes on approved tenant meetings held in conference room after hours. Train and encourage residents, as needed, in daily self-care practices. Assist tenants with laundry, shopping, appointments, if needed and directed by supervisor Train and monitor residents in the implementation and integration of independent skills such as bed making, laundry, etc. Converse with residents to reinforce positive behaviors and to promote social interaction. Accompany residents on shopping trips and counsel residents in purchase of items. Work with residents on achieving formal and informal goals such as use of public transportation. Observe, document, and review residents' daily progress, needs, medical issues, and behaviors. Transport or assist in transporting residents to and from medical appointments. Ensure laundry room is closed/opened at appropriate hours. Maintains a Monitor Log, which includes tenant/guest activity and facility observations. POSITION OVERVIEW Under supervision of the Clinical Supervisor the CASAC Counselor will provide counseling services in integrated outpatient program. The CASAC Counselor provides comprehensive assessment services to clients who may have substance use and mental health barriers to employment by applying clinical judgment to determine appropriate treatment needs and level of care. This includes performing SUD diagnostic screening, assessment, evaluation, orientation, treatment planning, case management, counseling, intervention, crisis intervention, patient education, reporting and record keeping in both individual and group counseling settings. The CASAC Counselor is also responsible for referrals and coordination of care in consultation with other professionals. KEY ESSENTIAL FUNCTIONS Intervention and employing a range of treatment modalities to include person centered evidence-based treatment approaches to include but not limited to motivational interviewing, stages of change, trauma informed care, anger management, medication assisted treatment, dialectical behavioral therapy, cognitive behavioral therapy and contingency management. Ability to perform SUD and mental health screenings and intakes, orientation, SUD assessment, SUD diagnostic Impression, level of care as determined by LOCADTR, treatment planning, individual/group counseling, family interventions, psychoeducation, case management, crisis intervention, client education, referral, reporting and recordkeeping, care coordination and administrative supervision with regular supervision. Participates in supervision regularly. Develops and conducts group and individual counseling on a regular scheduled basis to caseload clients within the integrated outpatient program. Obtain random observed urine samples according to established protocol. Orients clients about program goals, objectives, rules and regulations, client's obligations, right's and program operations Knowledge of the obligation to engage in prevention as well as treatment techniques. Maintains and updates referral services and resources that may be useful to our client population; makes referral when necessary and appropriate. Ability to demonstrate proficiency in English including the ability to speak, write, comprehend aurally, and read at a level necessary to perform as a CASAC Counselor. Participates in outreach functions to reengage client lost to contact and maintain census capacity. Ability to complete all client progress update reports in a professional and timely manner to ensure coordination of care with client linkages to include the criminal justice system and entities. Position Overview The Certified Recovery Peer Advocate requires someone who is culturally aware and sensitive to the needs of the targeted population, possess excellent computer skills, be able to function as a part of an inter-disciplinary team and have understanding of therapy and group work rooted in evidenced based approaches for intervention and management strategies. Certified Recovery Peer Advocate services are peer-delivered services with a rehabilitation and recovery focus. They are designed to promote skills for coping with and managing behavioral health symptoms while facilitating the utilization of natural resources and the enhancement of recovery-oriented principles (e.g. hope and self-efficacy, and community living skills). The Certified Recovery Peer Advocate uses trauma-informed, non-clinical assistance to achieve long-term recovery from a behavioral health disorder. Under supervision of the Program Director, the requirements listed below represent the knowledge, skill, and/or ability required. The provider ensures strengths based, person centered quality of care provided in a culturally and linguistically meaningful therapeutic environment. POSITION OVERVIEW The maintenance worker is responsible for the general maintenance of the facilities which include but are not limited to repairs on heating and air conditioning equipment, hardware repairs, repairing light fixtures, plumbing repairs, moving equipment/furniture, painting and plastering walls, driving company vehicles and general carpentry needs of the facilities. A basic familiarity with the facilities mechanical plant is required. KEY ESSENTIAL FUNCTIONS Maintain the physical plant and facility in a safe and secure manner. Assure that the building has adequate heat or air conditioning, light, water and sewage disposal at all times; be on duty during interruptions of any of these to coordinate repairs and emergency contingency plans. Inspect, diagnose and repair physical plant problems as assigned. Maintains inventory of all departmental tools; issues and records the distribution and return of all tools used for daily. Adhere to Fire Safety / Emergency Regulations, Resident Rights and interests, general and special policies and procedures of the Facility. Perform services as designed at the Facility Manager; maintains grounds and properties owned and/or operated by the organization in compliance with all applicable codes, rules and regulations. Ensure that all garbage and trash, including hazardous materials, are disposed of properly. Perform in a wide range of trades including but not limited to maintenance, test, repair, installation and replacement of parts and equipment involving general skills in carpentry, plumbing, masonry, electricity, HVAC, refrigeration, sanitation, grounds, painting, wall covering, cleaning transportation and the preventative maintenance program. Maintain a clean, neat appearance wearing prescribed uniform at all times. Cleans all dust, dirt and debris accumulated as a result of maintenance performed, equipment malfunction and resulting repair, construction or renovation. Accurately completes job logs, records and other work order documentation as required. REQUIREMENTS High School diploma preferred. 3-5 yrs Maintenance Worker experience required. Current Boiler Operator Certificate. Ability to communicate effectively orally and in writing. Highly organized, motivated self-starter. Excellent time management skills. POSITION OVERVIEW Under supervision of the Clinical Supervisor the CASAC-T Counselor will provide counseling services in integrated outpatient program. The CASAC-T Counselor provides comprehensive assessment services to clients who may have substance use and mental health barriers to employment by applying clinical judgment to determine appropriate treatment needs and level of care. This includes performing SUD diagnostic screening, assessment, evaluation, orientation, treatment planning, case management, counseling, intervention, crisis intervention, patient education, reporting and record keeping in both individual and group counseling settings. The CASAC-T Counselor is also responsible for referrals and coordination of care in consultation with other professionals. KEY ESSENTIAL FUNCTIONS Intervention and employing a range of treatment modalities to include person centered evidence-based treatment approaches to include but not limited to motivational interviewing, stages of change, trauma informed care, anger management, medication assisted treatment, dialectical behavioral therapy, cognitive behavioral therapy and contingency management. Ability to perform SUD and mental health screenings and intakes, orientation, SUD assessment, SUD diagnostic Impression, level of care as determined by LOCADTR, treatment planning, individual/group counseling, family interventions, psychoeducation, case management, crisis intervention, client education, referral, reporting and recordkeeping, care coordination and administrative supervision with regular supervision. Participates in supervision regularly. Develops and conducts group and individual counseling on a regular scheduled basis to caseload clients within the integrated outpatient program. Obtain random observed urine samples according to established protocol. Orients clients about program goals, objectives, rules and regulations, client's obligations, right's and program operations Knowledge of the obligation to engage in prevention as well as treatment techniques. Maintains and updates referral services and resources that may be useful to our client population; makes referral when necessary and appropriate. Ability to demonstrate proficiency in English including the ability to speak, write, comprehend aurally, and read at a level necessary to perform as a CASAC Counselor. Participates in outreach functions to reengage client lost to contact and maintain census capacity. Ability to complete all client progress update reports in a professional and timely manner to ensure coordination of care with client linkages to include the criminal justice system and entities. Participates in program meetings and required trainings. Ability to complete any reasonable tasks or requests made by a supervisor. REQUIREMENTS High School Diploma or GED. Associate Degree preferred. Valid unrestricted OASAS certification as a CASAC-T. Minimum of three (3) years working in a Substance Use and Human Service setting. Must obtain Mandated reporter (2 hours) training/certificate prior to hire date.
