* Performing case management activities based on clients' continuing care plans regarding initial housing, job searches, and establishment of medical services, social services, education, support services, and services from other community providers.
* Coordinating continuing care recovery referrals by developing positive relationships with community providers and including scheduling of initial appointments.
* Working closely with the Clinical staff in the Inpatient Residential Services program to ensure the clients transition smoothly from the residential portion of treatment to the continuing care program.
* Collecting data and program success rates as directed.
Qualifications:
* LADC, CADC or similar substance abuse treatment certification preferred.
* Bachelor's degree in education-counseling, psychology, rehabilitative counseling, social work, substance abuse treatment, mental health or closely related field preferred.
* A minimum of 2 years of supervised substance abuse counseling experience required; 3 years or more preferred.
* Re-integration planning and case management experience required.
* Must have a clean driving record.
* Must have the ability and willingness to spend up to 50% of their time on local travel using their personal vehicle in the completion of essential duties and responsibilities of the position.

- Supervises staff and oversees the day-to-day operations of a Continuing Education (CE) program.

- Plans and schedules CE classes for each term, creating a CE course catalogue to advertise available courses.
- Oversees the scheduling of CE courses per term and their entry on CS9 (PeopleSoft Campus Solutions) for students to register; assigns instructors to teach courses; and reserves appropriate classroom space.
- Monitors registration activities and cancels CE courses that do not meet minimum enrollment level goals. Ensures students are informed of cancellations.
- Prepares and processes Instructors' contracts for each semester.
- Responds to requests for assistance from Instructors' regarding teaching materials, reserving audio/visual equipment, submission of attendance and related documents, and related issues.
- Trains staff and administrators on processes and procedures relating to the operations of the Continuing Education Department.
- Oversees the preparation and processing of purchase requisitions for the ordering of supplies, equipment, and materials for continuing educations classes.
- Participates in preparing and processing legal agreements relating to partnerships and contract training in coordination with the CCC's legal department.
- Works with Business Office in monitoring budget expenditures and approval of purchase orders.
- Accountable for assigned projects including department and student survey administration, data analysis, and report preparation.
- Compiles and maintains CE program and student data including course and student learning outcome assessments.
- Provides operational support for special CE initiatives and mass enrollment offerings, including registering students and assisting with other system needs.
- Works on various special projects designed to support training and special initiatives when not assigned to a college.
- May work with Marketing/Communications to effectively promote CE programs, participating in outreach activities to promote CE programs.
- Must be available to work some evenings and weekends.
- Must be available to travel to all seven colleges and partner locations. May be stationed at a college for an extended period of time.
