Job Description

· Facilitate communication between the patient and the interdisciplinary team.

Qualifications

· Licensed Practical Nurse with Valid CT LPN license

· One year of LPN experience; Homecare experience preferred.

· Valid driver's license and reliable transportation required.

· Computer proficiency required.
