Job Description

· Facilitate communication between the patient and the interdisciplinary team.

· Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence.

· Utilize state-of-the-art technology to document skilled services provided.

Qualifications

· Licensed Practical Nurse with Valid CT LPN license

· One year of LPN experience; Homecare experience preferred.

· Valid driver's license and reliable transportation required.

· Computer proficiency required.

The Certified Nurse Attendant facilitates the provision of timely and effective patient care under the supervision of licensed members of the healthcare team. Required Experience

Required:



Current California CNA certification
Evidence of continuing education where required.
Current Basic Life Support Care
LA City Fire Card within six (6) months of employment




Preferred:



Prior experience in acute facility
Basic windows and mouse skills

FISCAL RESPONSIBILITY:
• Manages supply authorization and promotes judicious use and cost effective practices for patient supply utilization; provides product recommendations based on quality evidence based practice with judicious cost utilization practices.
• Actively provides improvements in managing supply costs based upon location challenges and utilization patterns

Error
Other: Graduate of WOCN program with certification in wound and ostomy.

Other: Continence certification highly preferred.

CERTIFICATION & LICENSURE

BLS-Basic Life Support Healthcare Provider

CWOCN-Certified Wound Ostomy Continence Nurse within 180 Days of hire

RN-Registered Nurse of California

OR CWS-Certified Wound Specialist

Continence certification recommended

EXPERIENCE

3 Minimum of three years professional nursing experience required.

1 Prefer at least one-year home health experience and one-year experience as a WOCN in any setting.

SKILLS AND KNOWLEDGE

Ability to form harmonious working relationships with internal and external customers.

Demonstrated leadership and negotiation skills and the ability to manage resources for patient care in a cost effective manner.

Ability to exercise independent sound judgment in planning and providing patient care.

Demonstrated ability in physical, psychosocial, and environmental assessment skills and in implementing plan of treatment.

Excellent organizational skills.

With appropriate training, demonstrates an ability to competently work with the computerized documentation tools and systems of the organization.
