Employment Support Professional supports individuals at their job to learn work tasks, find solutions to barriers, teach soft skills related to work, etc. The job coach reports to the Supported Employment Program Manager.
Essential Functions (Job Duties)
1. Coach, mentor and support individuals at their jobs. Learn the individuals job tasks and responsibilities so you can train them to complete tasks correctly, efficiently and timely.
2. Assist individual in all aspects of their work including but not limited to: Learning and adapting to new work environment, fostering natural supports, proper communication with supervisor and co-workers, learning and understanding work rules and policies, etc.
3. Model appropriate work behaviors.
4. Identify barriers and find creative solutions to help the individual succeed at the job.
5. Work with individuals at their job sites to help them overcome obstacles to maintain employment successfully.
6. Talk to individual's employer on a regular basis to ensure no issues at work. If/when issues are communicated or identified, work with the person and supervisor to develop strategies for overcoming barriers.
7. Maintain contact with person's plan writer (Employment Specialist or Job Developer) to keep them apprised of person's progress or any concerns that might affect the person's success at work.
8. Perform formal and informal assessments to learn about the person, their learning style, challenges and strengths.
9. Complete appropriate service documentation for every service provided in a timely manner.
10. Review person's employment plan and work on goals identified in plan.
11. Complete all forms required for billing purposes; and any other forms required by the agency or supervisor.
12. Complete any documentation corrections as needed in a timely manner.
13. Protect individual's confidentiality.
14. Participate in at least 24 hours of Innovations trainings within the first year of employment, and at least 6 hours annually thereafter.
15. Participate in all required and recommended agency trainings.
16. Professionally represent Citizen Advocates (Career Visions) with families, employers, other organizations and the community as a whole.
17. Adhere to all policies and regulations set forth by Citizen Advocates, Inc.

Peer Support services are peer-delivered services with a rehabilitation and recovery focus. They are designed to promote skills for coping with and managing behavioral health symptoms while facilitating the utilization of natural resources and the enhancement of recovery-oriented principles (e.g. hope and self-efficacy, and community living skills). Peer support uses trauma-informed, non-clinical assistance to achieve long-term recovery from a behavioral health disorder.
Activities included must be intended to achieve the identified goals or objectives as set forth in the individuals/families individualized service plan, which delineates specific goals that are flexibly tailored to the participant(s) and attempt to utilize community and natural supports. The intent of these activities is to assist individuals and/or their families in initiating recovery, maintaining recovery, and enhancing the quality of personal and family life in long-term recovery.
Essential Functions (Job Duties)
There are 6 categories of peer-support components. Any of the below services can be provided either individually or combined as the care/service plan indicates their need. They include:
Advocacy:
Assistance seeking and obtaining benefits and entitlements, food, shelter, permanent housing.
Assisting recipients in participating in shared decision making.
Use Peer Bridgers to assist with linkages to and systems navigation within behavioral health and allied human services systems to access appropriate care.
Benefits advisement and planning.
Development of psychiatric advance directives (PAD).
Assistance advocating for self-directed services.
Outreach and Engagement:
Companionship and modeling of recovery lifestyle, including participation in recovery activities that might be beyond the scope of treatment providers (e.g., coffee/tea at a coffee shop, attending a court date, attending an appointment, attending or participating in a recovery celebration event or wellness activity).
Raising the awareness of existing services, pathways to recovery and helping a person to remove barriers that exist for access to them.
Interim visits with individuals and/or their families after discharge from Hospital
Emergency Rooms, Detox Units or Inpatient Psychiatric Units to facilitate community tenure and increased readiness while waiting for the first post-discharge visit with a community based mental health provider, treatment provider or appropriate system of care. “Outreach” is a face-to-face service with a patient and/or their family provided by clinical staff or a peer advocate for the purpose of increasing motivation to participate in clinically indicated treatment for chemical dependence. Individuals and/or families identified for this intervention must be current patients who have failed to appear for sessions at the program and are judged to be at risk for prematurely discontinuing treatment or persons transitioning from another Office-certified program.
Self-help Tools:
Assist selecting and utilizing self-directed recovery tools such as Relapse Prevention Planning.
Assist selecting and utilizing the things that bring a sense of passion, purpose and meaning into his/her life and coaching the person as they identify barriers to engaging in these activities.
Assist individuals to help connect to natural supports that enhance the quality and security of life
Connecting individuals to “warm lines.”
Connections to self-help groups in the community.
Peer Recovery Supports and Peer Counseling:
Recovery education and counseling for individuals and their family members.
One to one peer support. Person centered goal planning that incorporates life areas such as community connectedness, physical wellness, spirituality, employment, self-help.
Assisting with skills development that guides people towards a more independent life.
Individuals in continuing care may receive counseling or peer services once per month.
Learning and practicing new skills.
Helping peers self-monitor their progress.

Transitional Supports:
Bridging from jail or prison to an individual's/family's home (note: that peer supports while in Jail are not Medicaid reimbursable).
Bridging from institutions (e.g. inpatient or residential facilities) to an individual's/family's home (note: that peer supports while in an institution are not Medicaid reimbursable).
Bridging from general hospitals to an individual's/family's home.
Bridging from an individual's/family's home to the community.
Arrangements for appropriate services (appointment dates, contact names, and numbers, etc.) are discussed and made with the individual/family, their significant others and/or family prior to the planned discharge date. Documentation of this information will be included in the individual's/family's case record. Where an individual and/or family is going from a bedded service to another service, a warm hand-off or peer service is considered where possible.
Pre-crisis and Crisis Support Services:
Providing companionship when an individual or family in an emergency room or crisis unit or preparing to be admitted to detox, residential or other service to provide non-clinical support.
Providing peer support in the individual's/family's home or in the community to support them before a crisis or relapse.

Provide direct clinical services to youth, including treatment planning, therapeutic interventions, crisis support, and the development of coping skills, emotional regulation, and social functioning.
Conduct clinical assessments, including psychosocial evaluations, risk assessments, and safety planning.
Guide, review, and approve Individualized Action Plans (IAPs) in collaboration with the youth, families, treatment team, and the Treatment Team Leader.
Participate in referral screenings, admissions, service planning meetings, discharge planning sessions, and interdisciplinary case conferences.

Direct Support Professional Team Lead

SUMMARY/OBJECTIVE: Coordinates the cleaning of contracts.
Supervises cleaning of assigned accounts using proper commercial practices meeting the requirements of the contract.
Bathrooms - all fixtures will be sanitized and free of dirt.
Offices - must be vacuumed and dusted to ensure a clean appearance.
Main areas - are to be cleaned to maintain a clean and positive environment.
Floors - carpets will be vacuumed, tile floors swept and mopped, and floors with finishes maintained.
Supervise crew of consumers for the purpose of cleaning janitorial accounts.
Complete all paperwork to include timesheets, cleaning checklists, supply disbursement and vehicle logs.
Maintain an inventory of materials and supplies
To keep record of products used in assigned contracts. Order on a timely basis product or supplies needed to perform daily work assignments.
Maintain equipment in proper working order.
To perform or have performed routine maintenance on all janitorial equipment.
Disposal of account refuse.
Refuse must be collected daily and disposed of in a safe and approved manner, in accordance with agency and commercial policy.
Communication.
Communicate to Clean Sweep Manager, either verbally or in writing, information or observations which will impede his/her in completing his/her assignment.
Maintain a safe work environment for janitorial crews
To maintain work area to be in compliance with OSHA and safe janitorial practices.

A Psychosocial Rehabilitation Specialist serves in a paraprofessional capacity as part of the crisis intervention unit.
