Coordinates care for complex patients with co-morbid mental and physical health conditions. Demonstrates knowledge of annual universal depression and substance abuse screenings and effectively provides necessary f/u services. Purpose: The Justice Services Jail Re-Entry Therapist is responsible for the provision of therapeutic interventions for persons referred from justice organizations to Clinica Family Health & Wellness. Develop and carry out a consistent plan of treatment, from intake and assessment to closure. Coordinate treatment with legal entities including probation officers. Participate in staffings with internal and external partners to coordinate care. Essential Functions: Provide a variety of case management services, including managing Social Security payeeships, assisting clients to apply for various types of public assistance and providing supporting documentation, attending multi-agency clinical case consultations, and facilitating referrals to other organizations. Participate in internal consultation and supervision with team members and other professionals in the organization. Pursue professional growth and development by seeking out learning opportunities and integrating new learning into daily work practice. Maintain working knowledge of Evidence Based Practices, current Co-Occurring Disorder treatment recommendations and trends in community mental health, including health care reform. As our Behavioral Health Tech (BHT) you will provide direct care and support to individuals with mental health and/or substance use disorders on a multidisciplinary team within our Acute Care Services (ACS) team. You will assist with identifying appropriate ACS services and enrolling clients into the best program that meets the client's needs. You will use crisis de-escalation skills to assist with unregulated clients to ensure client and staff safety. You will perform client registration into the appropriate behavioral health services, including crisis and urgent psych, within the ACS network. You will refer clients who appear to be intoxicated or in substance withdrawal to a SUD specialist to assess for appropriate level of care. You will also serve as the contact point, advocate, and informational resource for clients, treatment team, family/caregiver(s), and community resources.
