Summary The Pharmacist will perform specified distributive and clinical pharmacy activities and related services as they pertain to providing safe and effective home infusion therapies. Quality Improvement: Participate in continuous quality improvement initiatives, evaluating and enhancing pharmacy services. Qualifications: Pharm.D. or residency from an accredited institution. PGY 1 Residency in Hospital. Licensed to practice in the State of Texas. A.C.P.E. accredited I.V. Certification, Completion of an accredited Residency in a Hospital Pharmacy (PGY1/PGY2).
