Performs Pharmacist duties including but not limited to: • Screens and interprets chart orders or prescriptions for drug interactions, therapeutic duplications, allergic or adverse drug reactions, and proper dosing relative to patient age, weight and disease state. • Dispenses medications written on chart orders or prescriptions. • Maintains current, complete, and accurate patient medication records. • Oversees preparation of parenteral nutrition solutions, intravenous admixtures, chemotherapy, and piggyback solutions. • Verifies unit dose carts, IV orders, piggybacks, parenteral nutrition and chemotherapy orders prepared by the pharmacy technician, intern, or another pharmacist as required. • Verifies entry of chart orders into the computer system by the pharmacy technician or intern. • Rectifies any missing doses or missed doses. • Provides drug information to physicians, nurses, pharmacists, and other health professionals. • Reviews medication profiles and monitors patient drug therapy upon entering new orders. • Performs all other duties as assigned.
