The annual salary range for this position is $98,000.00 - $172,000.00 *This pay range is for a 40 hours/week schedule. Weekly scheduled hours are determined individually and the ranges for different scheduled hours are prorated based on the percentage of 40 hours. E.g., the range for a 36 hours/week schedule, which is 90% of 40 hours, is 90% of the above range. Additional compensation includes annual or quarterly performance bonuses.

Basic Qualifications
* BS in Pharmacy or Pharmacist Degree from an accredited educational institution.

* Medication therapy management and management drug therapy-related issues may be carried out by the following actions by the CPP: 1) Make direct changes to medication regimen (e.g. medication initiation, dose adjustment, and discontinuations). 2) Monitors and assesses therapeutic and adverse effects of drug therapy (including drug interactions) through selection and evaluation of physical and laboratory parameters. Orders lab tests as necessary to monitor regimen. 3) Recommendations to responsible provider for adjustments to the medication regimen for services/disease states not consulted for. 4) Referral back to provider or consult request to appropriate Practitioner when conditions necessitate and are outside the scope for the CPP. 5) Education to patients and/or caregiver regarding patients' medical conditions and on proper use and care of their medications. 6) The medication treatment plan and therapeutic endpoints will be communicated with the primary care provider.
* Serve as a consultant to other providers for intensive medication therapy management services. Provides general educational programs for staff and patient-specific evaluation and recommendations. Recommended therapeutic drug regimens take a risk versus benefit, cost versus benefit and patient-centered approach. Requests for assistance regarding medication-related issues will be provided in a timely fashion and be based on published treatment guidelines, evidence-based medicine, patient factors, and formulary issues and follow sound medical and pharmaceutical practices.

**General Role Description:**
The Specialty Pharmacy Liaison will engage directly with patients to coordinate pharmacy services through the health system's specialty and retail pharmacy. **About the Pharmacy:** Pharmacy hours are Monday through Friday from 8:30am-5:00pm
**Primary Responsibilities:**
+ Initializing and leading ongoing work in clinics and ensuring that the patient/CPS engagement is appropriate and consistent with their preferences. This includes:
+ Ensuring that every patient who can benefit from specialty pharmacy services is identified and met in clinic upon arrival
+ Explaining the benefits of our specialty pharmacy, without impeding patient choice of pharmacy, and achieving patient participation

General Role Description:
The Specialty Pharmacy Liaison will engage directly with patients to coordinate pharmacy services through the health system's specialty and retail pharmacy. Acting as a bridge between patients, providers, and the pharmacy, this role focuses on improving prescription capture, providing follow-up counseling, and ensuring smooth communication. Key duties include processing prior authorizations, assisting with financial aid, updating clinical data, and supporting administrative tasks. The liaison collaborates closely with the CPS Patient Care Services team and works under the direct supervision of a pharmacist.
Schedule: This full time, nonexempt position will work a minimum of 40 hours per week, Monday through Friday from 8:30am-5:00pm and additionally as needed.
About the Facility: Ozarks Healthcare is a Joint Commission accredited, 114-bed acute care hospital and not-for-profit medical referral center with over 1,300 employees, serving an eight-county area in south-central Missouri and north-central Arkansas. Services include family medicine, specialty clinics, rehabilitation, home care, interventional cardiac care, open-heart surgery, oncology, neurosurgery, and cancer treatment.
About the Pharmacy: Pharmacy hours are Monday through Friday from 8:30am-5:00pm
Primary Responsibilities:
* Initializing and leading ongoing work in clinics and ensuring that the patient/CPS engagement is appropriate and consistent with their preferences. This includes:
* Ensuring that every patient who can benefit from specialty pharmacy services is identified and met in clinic upon arrival
* Explaining the benefits of our specialty pharmacy, without impeding patient choice of pharmacy, and achieving patient participation
* Successfully enrolling patients into the program
* Performing all benefits investigation, prior authorization, and financial assistance work needed to ensure patients have access to the medication they need
* Overseeing services provided to enrolled patients - ensuring that everything from order entry to final verification is completed quickly, accurately, and in accordance with patients' needs
* Providing direct support to enrolled patients to ensure that they are supported throughout their therapy regimen and achieve their intended outcome. This includes:
* Conducting outbound phone calls for patient support, adherence checks, and refill reminders
* Accepting inbound calls from patients, as needed and sharing in on call coverage to ensure 24/7 patient support
* Appropriately triaging any clinical questions to an appropriate clinician (pharmacist, nurse, or provider)

The annual salary range for this position is $98,000.00 - $172,000.00 *This pay range is for a 40 hours/week schedule. Weekly scheduled hours are determined individually and the ranges for different scheduled hours are prorated based on the percentage of 40 hours. E.g., the range for a 36 hours/week schedule, which is 90% of 40 hours, is 90% of the above range. Additional compensation includes annual or quarterly performance bonuses.
