We are seeking skilled Physical Therapists to join our actively growing Home Health Team with territories in St. Louis City and County, St. Charles County, Warren, Lincoln, Franklin, and Jefferson Counties. Cooperative Home Care is a locally owned and operated home care company founded in 1986, with a referral-heavy Home Health division. We are looking for physical therapists to cover the clients in a set territory: company car and gas card are provided.
Responsibilities:
Review and evaluate physician's or podiatrist's referral and patient's medical record to determine physical therapy required.
Perform patient tests, measurements, and evaluations, such as range-of-motion and manual muscle tests, gait and functional analyses, and body parts measurements, and record and evaluate findings to aid in establishing or revising specifics of treatment programs.
Administer manual therapeutic exercises to improve or maintain muscle function, applying precise amounts of manual force and guiding patient's body parts through selective patterns and degrees of movement.
Observe, record, and report to the physician or podiatrist the patient's treatment, response and progress.
Instruct patient and family in total physical therapy program.
Review physician's referral and patient's medical records to help determine diagnosis and physical therapy treatment required.
Discharge patient from physical therapy when goals or projected outcomes have been attained and provide for appropriate follow-up care or referrals.
Inform patients when diagnosis reveals findings outside physical therapy and refer to appropriate practitioners.
Provide educational information about physical therapy or physical therapists, injury prevention, ergonomics, or ways to promote health.
Requirements:
Proven experience as a Physical Therapist
Valid license to practice physical therapy in the state of Missouri
Strong knowledge of anatomy, physiology, and rehabilitation techniques
Excellent communication and interpersonal skills
Ability to work effectively in a team environment

YOUR LIFE AS A CAREGIVER
This companionship and caregiving position offers flexible hours and opportunities to assist seniors on day shifts. Clients may receive as little as 4 hours of care, while others require 24-hour care.
As a Caregiver, you have the rewarding job of helping seniors live as fully and independently as possible while remaining in the places they love. You competently provide non-medical elderly care and home companionship, helping with various responsibilities such as laundry, grocery shopping, and meal preparation. With the client's dignity always at the forefront of your mind, you help them maintain proper personal hygiene. You are gentle and professional as you aid them with daily tasks such as bathing and feeding. At times, you provide companionship to clients who have dementia or cognitive impairments. Your kind and patient demeanor serves you well as you ensure they receive the highest quality of elderly care.
You carefully complete documentation for each shift, submitting timely updates. Committed to providing consistent elderly care, you communicate routinely with the office about the condition and needs of your clients.
