JOB SUMMARY
The Diabetes Nurse Education Coordinator provides state of the art diabetes education in a variety of settings to patients and their families/care partners, helping to optimize diabetes management. The Diabetes Nurse Educator also participates in the development of educational materials and serves as a resource for quality diabetes information for patients and other health care team members. Uses an understanding of the principles of growth and development to assess each patient's age specific needs and age-specific treatment and care.

Qualifications

Education: Associates Degree in Nursing; Bachelor's degree in Nursing preferred.

Experience: Three ( 3) years of RN experience required. One to two years of experience in the care and education/counseling of patients with diabetes or another chronic disease required. Academic and/or experiential preparation in program management and in the care of people with a chronic disease required.

Licensure, Certification, Registration: Current Connecticut RN license required. Certified Diabetes Educator (CDE) or Board Certified- Advanced Diabetes Management (BC-ADM) required or must pass exam within 1 year of eligibility.

Job DescriptionProvides oversight for planning, implementation, and evaluation of a diabetes self-management education and support program. Coordinates day-to-day operations of diabetes educational services. Ensures that the National Standards for DSMES are met, person-centered, and maintained to achieve individualized behavioral and treatment goals that reduce and optimize health outcomes. May serve as a Diabetes Educator as well as program Quality Coordinator.
Essential Duties and Responsibilities
• Maintains a diabetes self-management education and support (DSMES) program, which provides quality health education for managing diabetes and preventing health complications.
• Ensures that the National Standards for DSMES align with current evidence-based practices and utilization trends.
• Coordinate patient diabetes self-management education and support through clinical assessment, education, and documentation throughout all phases of diabetes related care. • Provides patient education with an understanding and awareness of age and culturally specific needs.
• Develops quality improvement strategies, which lead to improved diabetes outcomes.
• Maintains accreditation of DSMES program, providing the ability to bill for services through Medicare, Medicaid, and private insurers.
• Maintains a well-structured referral process for building a strong and sustainable DSMES program.
• Performs ongoing evaluation of DSMES services provided, ensuring continuous quality improvement plan is updated and reviewed at least each year.
• Functions as a member of the diabetes management team along with the patient, physician, and other clinical colleagues, family, and administrative staff.
• Serves as a diabetes resource internal and external to the organization.
• Participates in community events, health fairs, etc.
• Maintains the required 15 hours of CE credits within a 12-month period.
• Must be willing to work through the pathway to becoming a Certified Diabetes Care Education Specialist (CDCES) and maintains certification.
• Works toward acquiring 200 hours of diabetes care and education prior to applying for CDCES exam, within the most recent year.

Housed within Scripps Health, Scripps Whittier Diabetes Institute (SWDI) is Southern California's leading diabetes center of excellence, committed to providing the best evidence-based diabetes screening, education and patient care. Founded in 1981, SWDI's mission is to improve the quality of life for individuals with pre-diabetes, gestational diabetes, type 1 and type 2 diabetes through diverse programs, including diabetes self-management education, behavioral health, clinical research, diabetes prevention, retinal screenings, and professional training and education. SWDI's interdisciplinary team is comprised of endocrinologists, psychologists, nurse practitioners, registered nurses and dietitians, diabetes educators, and researchers. Our current portfolio of research projects is funded by the National Institutes of Health and non-federal sponsors, and evaluates a variety of pharmacological, device, behavioral, and psychological interventions to improve the quality of life for individuals living with diabetes.

The Diabetes Digital Coordinator (also known as Research Assistant III) will play a critical and direct role in an urgent COVID initiative in our Scripps hospitals, leading the rollout of remote patient glucose monitoring through a centralized digital dashboard. The Diabetes Digital Coordinator is key in enhancing the patient care experience through innovative technology.

As Diabetes Digital Coordinator, you will collaborate with on-site nursing staff to place patients on continuous glucose monitoring (CGM) and ensure the clinical data is transmitted to the Cloud. You will act as a resource, answer questions and be the technical and integration specialist on site.

The ideal candidate works independently and is comfortable in an inpatient environment interacting with the technical team. If you have great interpersonal skills and are excited by data, this may be the position you have been waiting for.

This is a full time, 8 hour shift position which is Grant Funded through December 2023. The location is to be determined.
Preferred Education/Experience/Specialized Skills/Certification:
