CARE COORDINATION is responsible for over site of assessment and reassessment of inpatients, including behavioral health inpatients and long term residents, for the purpose of performing utilization review, resource management and discharge planning.
RESPONSIBILITIES:
- Oversees development of the individualized plan of care that reflects assessment, planning, implementing and evaluating the outcomes of the plan
- Ensures plan of care show multidisciplinary planning, consultation and education
- Discusses patient findings and progress toward outcomes with physicians and other members of the health care team
- Ensures plan is discussed with patient and family in a timely manner
- Ensures effective development and completion of discharge plan including discharge barriers and patient/family education
- Assists all departments in the development of meaningful criteria to evaluate the quality and appropriateness of the care provided
