What You'll Do: * Resident-Centered Assessment: Conduct initial and ongoing assessment of residents' physical and emotional health. Document changes in residents' conditions and communicate them to the healthcare team and dietary team. * Care-Planning: Collaborate with the healthcare team to develop and update individualized care plans based on residents' needs and preferences. Participate in care plan meetings to discuss progress and adjustments. * Supervision and Delegation: LPNs are responsible for overseeing and delegating tasks to CNAs and Resident Assistants.
