Transition Coach
to join their team in New Bern. Position can be
FT or PT (Mon- Fri 9:30a - 5:30p)

WHAT YOU WILL BE DOING:


  • Coach individuals during Job Club activities, volunteerism, and job shadowing.
  • Accompany individuals being supported to their community-based site to provide on-the-job training.
  • Analyze job duties and tasks and completes a thorough job task analysis.
  • Assist individuals in mastering skills and meeting production standards, fading support as appropriate.
  • Assist in performing an individual's job duties to maintain necessary production as well as model work behavior.
  • Facilitate social interaction with coworkers and provides reinforcement as learning is demonstrated.
  • Support the Transition Coordinator in the development of activities for the program.
  • Maintain documentation and evaluations as required by the Transition Coordinator and funding body's standards and regulations.
  • Coordinate communication with Empower leadership, parents, and other agencies serving the individuals being supported. This means that documentation must be entered into the electronic medical record system within 24 hours of service delivery.
  • Maintain good work habits regarding attendance, punctuality, appearance, and effective working relationships with co-workers, MCO staff, employers, and individuals being supported.
  • Provide supplemental evaluation, work development, and intensive training activities with individuals being supported according to their service plan and work schedule.
  • Assist with the personal care needs of the individuals being supported such as toileting, dressing, eating, etc. as needed.
  • Respond to the safety needs of individuals being supported (i.e. choking, positioning, etc.)
  • Keep the Transition Coordinator informed on all issues of importance.
  • Attend appropriate work-related workshops, in-service training, and conferences.
  • Complete required training within prescribed timeframes.

POSITION SUMMARY:

The Employment Coach provides tailored and individualized job coaching for people with disabilities to ensure access, success, and retention within competitive integrated employment position(s).

TYPICAL DUTIES:
