• You have completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C)
• You hold an unrestricted NP or PA state license, including board certification, or the ability to become board certified within 2 years.
• You have strong comprehension of clinical standards of care and are focused on quality of life for your patients
• You may also have experience or be certified in one or more of the following areas:

o Nephrology (CNN or CNN-NP)

o Wound Care

o Diabetes Educator

o Hypertension

o Palliative Care

What you need to know:

As an Advanced Practice Provider with our company, you influence and ensure the best clinical care for your patients. You leverage an integrated technology platform and are complimented by an entire interdisciplinary team including registered nurses, nurse care managers, health coaches, dieticians, pharmacists, psychiatrists, social workers, and even cardiologists and podiatrists – all within the the company.

Primary functions for this role will include:

• Comprehensive management of patient conditions, including assessment, diagnosis, treatment, and consultation, on a longitudinal basis
  
• Transitions of care support via collaborative pathways with post-acute care facilities
  
• Working under agreed-upon clinical workflows and protocols
  
• Partner with the company and our client partners' care teams to identify and eliminate social determinants of healthcare for your patients, including functional capacity, home environment, health literacy, housing instability, food insecurity, and caregiver support.

Environmental Aide (Housekeeping/Custodian)- Eddy Memorial Geriatric Center Troy NY - Full Time 2nd Shift
Must be available to work between the hours of 3pm-11:30pm which includes every 3rd holiday

What you will do:

The goal of Environmental Services is to provide a clean, orderly, and safe environment for patients, visitors, and staff in keeping with the mission and philosophy of St. Peter's Health Care Services. We are looking for the right candidate to join our Environmental Servicesteam. Responsibilities:

* Candidate must be able to lift, pull, and push up to 50 pounds, possesses a basic knowledge of how to dust, vacuum, mop, and sanitize.

* Thorough room cleaning: follow all processes as designated by training policies to insure all area, fixtures, and surfaces and surfaces are cleaned daily or as assigned, and in a timely manner.

* Comply with any infection control policies and procedures.

* Maintains good working relationships and communicates with other departments, residents, families and fellow staff. Responds pleasantly to all requests.

* Communicates to Housekeeping Supervisor only problems encountered while performing daily routine.

* Performs any other duties assigned by Housekeeping Supervisor or Director.

What you will need:

* Performs functions, duties and carry out responsibilities of custodial duty.

* Demonstrates SPHCS Standards of Behavior.

* Read, write and speak English at a minimum 6'th grade level.

* Follow written and verbal instructions and complete high school level computational skills.
