* Given primary care can be a fast pace, high needs environment, a BHC needs to be able to provide population-based health approaches to behavioral health care which includes: Establishing behavioral health services within the clinic as routine patient care, assessing and triaging patient care in the teachable moment (when patients are engaging medical team members, called a warm handoff), balance scheduled visits with warm handoff opportunities so that behavioral health services are assessable to all, engage in brief assessments/screeners to assess a patient level of functioning and possible risk level, and be a generalist (work with all patients of our healthcare system no matter the age or clinical need).
* Work with primary care providers to refer cases to mental health specialists as appropriate and bridge care to these patients until patient is successful in connecting with more traditional mental health services.
* Provide brief behavioral health visits modeled after the pace and needs of primary care. The PCBH model goal is to reach more patients and have a larger impact on our patient community. A typical scheduled visit is for behavioral health is about 30 minutes in length while warm handoff visits can vary from 15 minutes to 30 minutes. Schedule follow-ups with patients based upon clinical need while keeping access to behavioral health services in mind.
* Be effective in communication with team members about patient care including providing consultation to primary care providers and other team members about assessments of patient care needs and recommended interventions.
* Teach patients evidence supported interventions to assist in self-care, preventive medicine, quality of life improvement, and improved functionality. Equally important is to be an educator of these techniques to patients' families and primary care team members so they can assist in disseminating and reinforcing strategies as well. A typical scheduled visit is for behavioral health is about 30 minutes in length while warm handoff visits can vary from 15 minutes to 30 minutes. Must have's you'll need to be successful:
* High school diploma/GED required (Associate degree preferred) possess an unencumbered WA state phlebotomy license, completion of a phlebotomy course and minimum of one (1) year of Phlebotomist experience.
* Must have a minimum of one (1) year of experience as a Phlebotomist.
* Passing the national certification exam preferred.
* At time of hire, must possess and maintain an Active Washington State Medical Assistant Phlebotomy Certification.
Proof of vaccination for COVID-19 is required, prior to start. HealthPoint does not accept the Johnson & Johnson COVID-19 vaccine as proof of vaccination. If you have received the Johnson & Johnson vaccine, we ask that you provide documentation demonstrating proof of an alternate COVID vaccine or vaccine series. All new employees are also required to show proof of immunizations and/or immunity to MMR (measles, mumps, rubella), Varicella, annual Influenza and TB QuantiFERON Gold Titer. Additionally, if you work in a HealthPoint clinic, Tdap (within last 10 years) is required. Hepatitis B. is required for clinical employees with potential exposure to blood/blood products. All immunizations are a condition of employment. Upon hire, employees must provide proof of their immunizations and/or immune titer results prior to starting or no later than their fifth (5) business day of employment.

* If you are providing immunizations from an electronic health record, please ensure that you obtain a copy of your full records rather than a screenshot. Each page of your records should include your first and last name, date of birth, and the name of the health system from which the records are pulled.
* If records do not show any data, please seek guidance from your provider for further assistance.
* If you are unable to provide proof as noted above, you can choose to have a lab titer drawn to check immunity or to be re-vaccinated. If you receive vaccination(s) or lab titers, you may obtain them through HealthPoint at no cost to you.
