Inpatient Social Worker I (PRN) - DAY SHIFT - Social Services

Overview:


As an Inpatient Social Worker I, you will be part of an interdisciplinary team responsible for maintaining continuity of patient care from admission to discharge. You will provide emotional support to patients and their support systems, utilizing appropriate criteria for inpatient care and planning timely discharges.
Responsibilities:

* Functions as part of an interdisciplinary team to maintain continuity of patient care

* Provides emotional support to patients and their support systems

* Utilizes appropriate criteria for inpatient care and plans timely discharges

* Responsible for crisis intervention, individual, family, group, bereavement, and end-of-life counseling

* Actively participates in designing and implementing institutional programs/initiatives to improve patient access to medical center resources, quality of care, cost reduction efforts, and patient satisfaction

Qualifications:

* Master's degree in Social Work from a Council of Social Work Education school of social work

* Current social work licensure in Pennsylvania

* Preferred qualifications: minimum of two (2) years of acute care case management/social work experience, knowledge of PC-based computer software (Cerner, Navihealth/Curaspan), certification in case management from a nationally recognized organization
