Then, they may be scheduled for therapy evaluations at MKSA/KULA or bring recent therapy evaluation reports done elsewhere. To receive Early Intervention Day Treatment habilitation services, a child must have a documented developmental disability or delay, as shown on the results of an annual comprehensive developmental evaluation and qualify for at least one therapy service. Those services include (but are not limited to) fine motor delays, sensory disorders, gross motor delays, speech-language delays, hearing disorders, feeding disorders, developmental delays, cognitive delays and medical needs. Children are placed in treatment rooms based on their developmental needs and ages. Our treatment rooms are adequately staffed with teachers and teachers' assistants and are designed to celebrate the cultural differences of the children and families we serve. MKSA/KULA has many licensed occupational therapists, physical therapists, and speech-language pathologists on staff to provide needed care to the children who qualify for therapy services. And, MKSA/KULA has nurses on staff to serve children with medical needs. MKSA/KULA's services also include developmental/psychological testing by a licensed psychologist, nutritional evaluations and consultations, and audiological evaluations. Basic computer skills which includes a working knowledge of database, word processing, spreadsheet, and Internet software. Ability to handle multiple tasks in a very busy environment. Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small groups situations. Ability to add, subtract, multiply, divide, in all units of measure using whole numbers, common fractions, and decimals. Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations. Assists families with social service forms and paperwork. POSITION SUMMARY Manages referral process for potential patients and existing patients by coordinating with local physicians. Schedules evaluations for potential patients and completes intake process, if indicated. Completes PA packet on each prospective EIDT patient. Accepts all initial and re-evaluation reports. Maintains current EIDT PA on each patient by submitting updated information packets with requests for continuation for AFMC prior to the expiration date. Monitors and tracks each patients prior authorization denials and approvals and discharge date. Requests extensions and modifications to PAs when necessary. Provides quality assurance by performing chart reviews on each patient chart. Requests missing documentation and error corrections from individuals responsible for providing it. Develops a MTP for each CHMS patient. Obtains signature and send copy home to parent. Maintains Evaluation Tracking form for each EIDT patient. Completes appropriate paperwork when patients are discharged from the Academy. Sends parent surveys and discharge summaries. Interacts with patient family unit for scheduling of family psychotherapy appointments and maintains direct contact for changing family dynamics. Keeps in contact with DFCS as necessary to monitor on-going services to patients and families at risk. Investigates patient absences and follows up, as needed. Maintains adequate list of community resources and facilitates patient access to them, including food, clothing, school programs, vocational opportunities, etc. Assists families with social service forms/paperwork, such as SSI applications, Medicaid, TEFRA, AR Kids First, etc. Leads Multidisciplinary Patient Reviews and ensures they occur two weeks after admission and every six months thereafter to keep abreast of therapy goals and progress. Reports on family dynamics and changing situations in home environment in staffings. Makes home visits as needed for various problems such as patient absences, assessment of environmental risk, death in family, etc. For every service billed, the Patient Care Coordinator will provide progress notes in the appropriate format which contain all information as required by company policy, EIDT guidelines, and/or other applicable regulations. The Patient Care Coordinator will keep detailed records of all work with all patients and families he/she serves. Ensures that all progress notes relating to patient intervention reflect Treatment Plan goals and objectives and current diagnosis (es).
