This CDIS, Clinical Documentation Improvement Specialist on-site only opportunity is full time @ 40 hours per week, offers full benefits and a convenient flexible schedule. This individual will provide clinically based concurrent and retrospective review of inpatient medical records to evaluate the utilization and documentation of acute care services. The goal of concurrent review includes facilitation of appropriate physician documentation of care delivered to accurately reflect patient severity of illness and risk of mortality. Specific reviews are both determined internally and by requirements/requests of external payers or regulatory agencies and play a significant role in reporting quality of care outcomes and in obtaining accurate and compliant reimbursement for acute care
services.
Job Duties/Responsibilities:
Conducts initial and extended-stay concurrent reviews on all selected admissions, and documents findings within CDIS (electronic review system), denoting all key information (including working DRG and queries) utilized in the tracking process
Able to utilize inpatient admission criteria to assign only diagnoses that meet acute care criteria. In the advent that diagnosis does not meet admission criteria, communicates with Case Management Department to determine the appropriate patient status.
Collaborates with physician, physician extender, nurse, case manager/utilization review and HIM coder to identify principal diagnosis options, secondary diagnoses including complications and comorbidities, and procedures, to assign working DRGs for at least 80%-85% of the identified populations
Identifies need to clarify documentation in medical records, and utilizes strong communication skills with physician, physician extender, case manager, utilization review, nurse or other healthcare professional, utilizing appropriate tools to capture needed documentation
Utilizes monitoring tools to track the progress of the DRG Assurance program, through interpretation of the CDIS tracking reports, monitoring reports and data. Able to identify areas of focus through report analysis. Serves as a resource to physicians and Administration regarding issues related to the appropriateness of inpatient DRG assignment.

**Safety and Risk Management**

+ Ensure any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits and in compliance with fire codes.

• Assist in preparing vibrant person-centered monthly Life Enrichment calendars, which reflect residents' interests, preferences, and abilities, and offers life enrichment programs at hours convenient to the residents (morning, afternoon, some evening and some weekends.)
• Organize the activity supplies and equipment to ensure materials are available to residents.
• Prepare monthly report on progress of activity program as directed by the Life Enrichment Coordinator.
Invites and assists residents within the community to and from activities.
• Participate in trainings as requested by administration.
• Ability to recognize resident's change in condition that could require nursing intervention. Report same to Wellness Director immediately.
Maintain professional appearance, communication and confidentiality.
• Supervise volunteers as necessary.
• May assist residents on outings which may include driving the company vehicles.

QUALIFICATIONS

• Education: Minimum high school diploma or GED.
• Experience: Minimum 6 months preferred.
• Job Knowledge: Awareness of special needs and dimensions of wellness for older adults; techniques and skills to engage residents with different interests and abilities. Sensitivity to residents' feelings and well-being; ability to initiate and lead crafts, games, discussions and other activities; and to find and implement resources for LE activity programs. Computer skills adapt to new technology.
• Other: Enjoy working with senior adults, exercise judgment, display empathy, creative skills, imagination, friendliness, resourcefulness, enthusiasm, and have a caring attitude and sincere interest in others. Must have a clean driving record through the DMV.
*Team members who are scheduled to work 24 or more hours per week are eligible for benefits, families of said team members will also be eligible.
