Facilities Coordinator

This position is responsible for the coordination, maintenance, proactive and reactive, security and janitorial services of multiple facilities. The Facilities Coordinator performs routine inspections, maintenance, and repairs of the building system. They provide oversight of contractors per the direction of, or in the absence of the Facilities Manager.
Under limited supervision, performs a variety of semi-skilled work in the maintenance of Mosaic properties; and performs related duties as assigned.
The Facilities Coordinator is responsible for coordination and repairs and maintenance of all Mosaic facilities acting as the contact for emergency situations that affect Mosaic's ability to open for business. Additionally, the Facilities Coordinator will assess and consider daily 1) the safety of our employees, 2) the security of our assets and 3) the service to our patients.
Facility Coordination
* Coordinates facilities tickets for completion
* Works with vendors as needed to ensure repair work completion
* Works with janitorial vendors on coordinate their daily/weekly service at all facilities
* Works with HVAC vendors to coordinate their maintenance and repair work
* Coordinates procurement of items needed for facilities repairs and projects
* Works with Mosaic's security vendor on daily issues and coordination
Facility Maintenance
* Troubleshoots minor maintenance problems involving electrical, structural, plumbing, and equipment repair or replacement. Uses hand tools and power tools in making minor maintenance repairs and maintaining grounds and walks.
* Performs preventive building maintenance duties, caulks windows and doors, repairs and replaces broken windows, hang pictures, shelving and whiteboards.
* Performs routine plumbing repairs such as unstopping toilets, sinks or urinals, fixing water leaks, leaking faucets, and water fountains.
* Performs routine electrical repairs such as changing light bulbs or fluorescent lamps and assists in repairing ballast and light fixtures as allowed by electrical code.
* Purchases supplies, equipment, and materials necessary to complete projects.
* Dismantle/install and moves furniture, hang pictures and bulletin boards, moves. Assists with departmental and employee moves.
* Repairs and replaces door knobs or locks, repairs door closures, patches holes in walls, general painting and repair work.
* Provides assistance to officials, staff, visitors, and other employees as necessary.
* Participates in building coverage by working evenings and weekend rotation as required for maintenance that cannot be performed during regular business hours.
* Transports goods or equipment as assigned.
* Performs other maintenance duties as assigned by the Facilities Manager.
* Notifies management concerning the need for major repairs or additions to lighting, heating, and ventilating equipment.
Vendor Management

Enrollment Specialist

The Enrollment Specialist is the primary point of contact for Mosaic patients with insurance applications and eligibility requirements with state and/or federal insurance programs, including Oregon Health Plan. This includes contacting patients for re-enrollment and assisting the Billing Department with insurance verifications. The Enrollment Specialists will conduct Outreach activities with community partners and other organizations to provide information and enrollment assistance. This position will develop and maintain statistical enrollment reports and manage reports provided by the organization for enrollment and re-enrollment. This position will travel throughout the region to support enrollment assistance. Additionally, this role will be handling pre-registration and registration pre verification for all patients at Mosaic, new or established. Entering, auditing, educating, and advocating for accurate registration in the patient's electronic medical record (EHR). This role identifies patients who have termed from Medicaid/Medicare. When termed, reaches out to patients to inquire as to why they termed and assists patients in navigating the Enrollment process. Provides education on Medicaid eligibility and sliding scale eligibility. Bi-lingual in Spanish/English strongly preferred.
Patient Enrollment
* Aid and process state and/or federal applications for insurance coverage for patients and community members.
* Troubleshoot application questions or concerns from patients and community members.

Direct Patient Care:

The Medical Assistant (MA) is a member of the primary care team who provides direct patient care and provider support. The goal is to provide coordinated patient centered clinical support related to visit-level care. The MA facilitates the coordinated planning of office visits via patient and team collaboration, the initial rooming of patients during office visits (including medication reconciliation, risk factor review, and health maintenance review), provider support during office visits, reviews with patient the plan of care, and assists with follow-up as needed. Medical Assistant
Mosaic Community Health job in Prineville, OR
The Medical Assistant (MA) is a member of the primary care team who provides direct patient care and provider support. The goal is to provide coordinated patient centered clinical support related to visit-level care. The MA facilitates the coordinated planning of office visits via patient and team collaboration, the initial rooming of patients during office visits (including medication reconciliation, risk factor review, and health maintenance review), provider support during office visits, reviews with patient the plan of care, and assists with follow-up as needed. In addition, the MA may also perform in-office testing and clinic services (phlebotomy, EKG, hearing and vision testing, etc.), preparation and maintenance of exam rooms, maintenance of patient records, and other tasks as requested by medical providers.
Patient Care
* Pre-visit chart scrub (EMR): Including, but not limited to referrals, reviewing medications, Immunizations, allergies, future and standing lab orders, health maintenance due, and preventive care needs.
* Visit prep: procedures/vaccine prep/foot exam.
* Medication, Immunization, and allergy reconciliation.
* Rooming the patient may happen in person or remote: Including, but not limited to vitals/medical records/reconciling care everywhere with health history (remote rooming is applicable to MA II and MA III only).
* Prepare, administer, and document medications and vaccines.
* Assist with clinical procedures.
* Review provider recommendations and after visit summary (AVS) with patient.
* Patient Communication: AVS, Q&A, results.
* Address to patient encounters via electronic health records.
* Conduct clinical services: Including, but not limited to EKG, pulse oximetry, hearing & vision testing, collect and/or process lab specimens, spirometer, venipuncture.
* Schedule patient appointments, coordination of care with appropriate staff (e.g. CHW, PharmD, RNCC, Nutrition) for appointments or tests with specialists.
* Deliver patient care within HRSA services scope and approved locations, encompassing patient homes, community-based sites, and providing home-based care when necessary and in adherence to defined services and safety protocols.
Clinic Operations
* Care of the clinic, maintain cleanliness.
* Supply ordering.
* Room Stocking.
* Sterile Instrument Processing.
* Quality Assurance routine tasks (including but not limited to running controls, checking exp. dates, etc.).
Skills & Knowledge
Critical thinking and problem solving skills. Must have excellent written, verbal, telephone and interpersonal communication skills. Familiarity/experience with client interaction on the telephone. Knowledge of available basic community services. Basic typing skills (such as 30 WPM). Basic personal computer skills and comfort with Microsoft Windows operating system. EHR experience, EPIC experience a plus.
Accredited medical assistant certification required within twelve months of hire.

Pharmacy Technician (Float)

The Pharmacy Technician assists the pharmacist in delivering high-quality pharmaceutical care as directed. This includes accurately entering all patient and prescription information into pharmacy information system; accurately filling prescriptions; troubleshooting third party rejections to get billing problems resolved; performing inventory control and management; providing excellent customer service by waiting on patients at the pharmacy window; verifying all information upon accepting Rx's and when ringing out Rx's at the point of sale; answering incoming phone calls and referring to appropriate staff member if unable to answer question; balancing the daily cash drawer and preparing deposit; filing hard copies of Rx's and other paperwork as needed; receive and shelve pharmaceuticals and OTC products to proper location and assist with inventory management. The pharmacy technician will maintain active and unrestricted licensure through the Oregon Board of Pharmacy.
Responsibilities
* Maintain a safe and clean pharmacy by complying with procedures, rules, and regulations.
* Organizes medications for pharmacist to dispense by profiling prescriptions, preparing labels, and calculating quantities.
* Ensure medication availability by maintaining accurate pharmacy inventory.
* Support patients in obtaining needed medications via various patient assistance programs as needed.
* Accurately process prescription claims and pharmacy financial transactions according to company policy and state and federal law.
* Troubleshoot pharmacy related problems, escalating issues to the pharmacist where appropriate.
* Participate in all internal and external compliance measures with company and state/federal rules and laws.
* Always maintain professional working relationships, with the pharmacy personnel, physicians, nurses, and other medical personnel.

Spanish language fluency preferred. Excellent verbal and written communication skills and reading comprehension.
