Registered Nurse means an individual registered and licensed to practice nursing in Arizona RESPONSIBILITIES Coordinates hospice care by working with other team members as well as with the patients/family members to ensure needs are met. Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis. Perform evaluations for hospice care along with informational visits as needed. Utilize the nursing process to provide professional nursing care. Consult with the attending physician and Hospice Medical Director concerning changes in patients' condition requiring order changes and/or changes in the plan of care. Coordinate patient services with other members of the IDG as well as contracted services, when needed. Submit complete, accurate and relevant clinical notes in a timely manner. Submit patient visit schedule weekly. Participate in IDG and case conferences. Discuss with the supervisor if IDG determines that the patient would benefit from contracted services. Take responsibility for ensuring patients' plans of care are kept current. Performs on-call duties (including holidays) per policy and agency need. Actively participates in QAPI meetings and activities. Participate in staff development meeting. Continually strive to improve his/her nursing care skills by attending in-service education, through formal education, attendance at workshops, conferences, active participation in professional and related organizations and/or individual research and reading. Provide education to patients/family members regarding the dying process, plan of care and offers guidance with healthcare decisions etc.. Perform additional duties as they arise. JOB CONDITIONS May be stressful, however, you have a team of experts to assist you, you are not alone The ability to access patients' homes which may not be routinely wheelchair accessible is required. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care. Applicant may be asked on occasion to go outside designated area. Typical work day is 9-5 including charting done from home if needed. Job Summary We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems. Responsibilities Perform the initial home care patient visit and re-evaluate the patients needs and progress on a regular basis. Initiate the plan of care under doctors orders. Perform admission, transfer, re-certification, resumption of care, and discharge Oasis for the home care patient. Observe, assess, and document symptoms. Monitor reactions and patient progress. Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plans. Educate patients and caregiver on techniques for in home health care. Coordinate patient services. Supervise LVNs and HHAs. Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans Document any changes in patient conditions and report on changes Maintain accurate records of patient care and discuss observations with supervisor Qualifications Currently licensed Registered Nurse (RN) in state of employment Previous experience as a Registered Nurse is preferred 6 months experience preferred but not required.
