Summary: During an in-home visit that can last up to one hour, Adobe Care and Wellness providers review member s current health status, medical history, medications, social determinates, and other risks. By completing the comprehensive visit in their own home, we are able to get more insight into a member s overall health that can be difficult to capture during routine office visits.
Ability to Make Your Own Schedule!!! $125 per visit

DUTIES AND RESPONSIBILITIES:


Travel up to 90%; regions are identified and updated based on business needs.
Conduct in-home assessments on identified patients, following established guidelines.
Formulate a list of current and past medical conditions using clinical knowledge, judgment, and current findings.
Identify urgent and emergent situations for proper intervention.
Educate members on topics related to health conditions and the continuation of care.
Identify diagnoses to be used in care management and active medical management.
Develop positive relationships with members, caregivers, providers, and other care team members as needed.
Communicate gaps in care to the member s primary care physician if needed.
Identify internal and external referral needs when available.
Interact with peers in a collegial manner, supporting efforts to improve outcomes.
Compliant with all HIPPA regulations and maintain the security of protected health information (PHI).

Looking for candidates to work Full-Time 4 pm-12 am Sunday-Thursday

Part-Time 6 am-4 pm and 4 pm-12 am (Hours can be combined for Full-Time)

DUTIES & REQUIREMENTS:

• Must be experienced at cleaning with a keen eye for details.
• Maintains a high level of cleanliness & safety throughout designated areas.
• Removal of trash & soiled linens.
• Must be able to lift up to fifty pounds (50 lbs).
• Must be able to work as a team member.
• Solid work ethic, with an initiative for quality and customer service.
• Minimum of high school diploma or equivalent
• Must be reliable & keep to schedule.

• Graduate of an accredited school of nursing, BSN preferred.
• Current, unrestricted state license as a Registered Nurse.
• CPR certification required.

Major Responsibilities:

• Abides by company policies and procedures as well as INS standards of practice, OSHA guidelines, State Nurse Practice Act and state board laws/guidelines.
• Provides infusion and other skilled nursing care to patients including, but not limited to: patient assessments, patient/caregiver teaching, development and revision of the plan of care, implementation of physician orders, monitoring patient response, response to emergency patient needs and psychosocial support.
• Provides pre-discharge patient assessments in the hospital or clinic setting, when appropriate.
• Completes all required patient care documentation and reports accurately.
• Submits all required patient care documentation and reports in a timely manner.
• Communicates to supervisor, physicians and other health care providers, as appropriate, any changes in the patient’s condition.
• Takes on call responsibility as assigned to provide 24 hour availability and provides these services in accordance with established protocols.
• Takes professional responsibility for self-protection during required work in potentially hazardous situations (e.g. handling of blood and body fluids).
• Represents self in a professional and ethical manner at all times.
• Maintains professional licensure, certification and health requirements as well as demonstrating clinical competence of orientation programs, etc.
• Participates in the orientation of other nurses as requested.
• Supports the account executives and clinical liaisons by providing educational programs or visits to referrals sources, as requested.
• Assists in effective communication between the patient and the company, to include documentation of suggestions, complaints, and requests.
• Performs other duties as required.

• Other state required health tests allowable by law
