Outreach is seeking a Peer Driver - Mobile to work in our Outpatient Opioid and Methadone Treatment program located in Brentwood, Long Island. It is a part-time position. The schedule is Tuesday, Wednesday, Thursday and Friday with flexible hours.
What You Will Do:
Provide transportation and peer support to treatment services for clients with opioid use and substance use disorder.
Collaborate with multi-disciplinary team to provide recovery support services.
This role will require driving to various community-based stakeholders across Long Island and other boroughs.
Field-based and on-site peer support to prospective clients.
Assist client with person-centered recovery goal planning.
Collaborate with client and care management team to identify the services clients should receive based on stage of change, immediate needs, and recommended treatment objectives.
Provide services to clients to address housing, income support, education and vocational training, social supports, employment, and primary care needs.
Assist clients with linkages to and systems navigation within the behavioral health, human services and criminal justice systems as appropriate.
Act as an advocate and liaison for clients in accessing resources to support treatment plan goals.
Use motivational interviewing, recovery and trauma informed approaches when delivering peer services.
Involve family and significant others in the client's treatment with the team.
Provide individual peer support, wellness counseling, harm reduction counseling based on motivational interviewing.
Accompanying clients to appointments and events.
Conduct outreach to clients who unexpectedly disengage from clinic services.
Who You Are:
Certification as a Recovery Peer Advocate (substance use) or Peer Specialist (mental health) or eligible to obtain certification if not already acquired.
Valid NYS Driver's License and good driving record,
High School Diploma/GED and 2 years work or volunteer experience in a recovery-oriented setting.
Credential in CRPA-P or CRPA.
Lived experience using mental health and/or substance use treatment services.
Experience providing peer support to individuals with serious mental illness and criminal justice involvement, a plus.
Ability to work with diverse social, cultural, and economic groups.
Good knowledge of recovery resources such as 12step program and community support groups.
Excellent verbal and written communication skills.
Highly compassionate and attentive to detail
For this Long Island, NY position the expected base salary is $18 - $22 / hour.

Position: Maintenance / Porter Worker
The Maintenance / Porter Worker is responsible for the overall maintenance of the entire complex. This position is 2/3 maintenance 1/3 porter. The maintenance / porter person orders supplies with permission of the direct supervisor to accomplish the duties.
Job Scope:
Maintain building and surrounding area (parking lot, etc.)
Order janitorial and kitchen supplies.
Maintain heating and A.C. maintenance schedule.
Make necessary repairs in and outside of facility, i.e . electrical, plumbing, carpentry, etc.

Position: Part Time Administrative Assistant
The part time administrative assistant works with the Full-Time Administrative Assistant to ensure the overall front office activities and facilities in the facility run smoothly. They are also responsible for the accurate collection, tracking, and financial reimbursement of client fees.
Job Scope
Office Management Responsibilities:
Organizes office flow in an orderly and efficient manner.

Reimbursement Specialist Responsibilities:
Prepares Public Assistance application for HRA within 48 hours of a client's admission to Outreach House I.
Removes client from Public Assistance case within 48 hours of a clients discharge.
Returns clients PNA money to HRA in a timely manner after they have been discharged from Outreach without completion status.
Functions as a liaison between Outreach House I and HRA
Writes a monthly Personal Needs Allowance breakdown for Outreach bookkeeping in order to keep resident accounts accurate.
Monitors that HRA is paying shelter payments/PNA Allowance for all clients whose application has been accepted.
Troubleshoot any billing/payment problems that are occurring with HRA.
Monitor that parents are submitting SSI or SSD payments to Outreach House I in order to compensate for losses in shelter payments.
Collect family fees from the parents/guardians during Parent Support Group evenings.
Bills insurance and is responsible for the collection of insurance payments and denials.
Responsible for monthly revenue reports.

Using their personal lived experience this role promotes clients' recovery, offers support, advocacy, and skill development opportunities. There is 1 position available which focuses on a special initiative focused on State Opioid Response (SOR), which addresses the opioid crisis by increasing access to MAT; reducing unmet treatment need; and reducing opioid overdose-related deaths.
What You Will Do:
Represent Outreach's CRPA program in the community.
Provide peer recovery support to clients by sharing life experiences, assisting clients to develop a recovery plan, linking clients with community based programs supportive of recovery. Peer services may be provided onsite or in the community.

Initial and ongoing screening of resident and resident property in accordance with unit procedure.
Supervise clients in self-medication (under supervision).
Communicate all pertinent information, incidents and emergencies using the appropriate system including the SAC communication log, incident reports and 24 hour on-call supervisor.
Provide crisis interventions and follow up including documentation of the incident, accident and/or elopement reports.
Provide overall supervision and management of client population in the facilitation of the daily house schedule.

Outreach is seeking a qualified candidate for the position of Part Time Intake Counselor to work in our Centralized Calling/Medication Assisted Treatment (MAT) Hub located in Brentwood, Long Island. The Intake Counselor is responsible for managing initial admissions assessments and intakes, level of care determinations, transfers and referrals to the appropriate treatment modality. Monitors the assessment and intake process to ensure that appropriate level of care determination is being made, that applicants are being properly engaged, and that admissions and referrals are being implemented in a manner consistent with regulatory and clinical standards of care.
Facilitates communication, in conjunction with the Administrative Coordinator, between program locations to ensure client access to a prescriber within 24 hours for those in need of MAT.
Utilizes Motivational Interviewing (MI) and supervises other intake workers at the Hub in MI to engage prospective clients in the treatment process.
Identifies and assists in problem solving with management any issues impacting open access, connection to MAT services and/or client engagement in care.
Works with the Administrative Coordinator to ensure the smooth onsite operation of the MAT Hub, managing the flow of clients related to open access.
Maintains compliance with Agency Standards of access to care and provides expedited intake services, where indicated.
Maintains appropriate contact with referral and regulatory agencies to report on client progress, if appropriate, and matters of compliance and to ensure a continuing flow of referrals.
Works with staff to appropriately use SearchTreatment to connect individuals to services that Outreach does not provide.
Serves as a liaison to AHN/RHS for referrals from other providers through SearchTreatment.
Screens prospective clients for social determinants of health and other needs, such as medical care, psychiatric or vocational referrals, and other client identified barriers to treatment, through the use of in house case management or peers.
Maintains relationship with appropriate outside agencies to stay informed of advances in client treatment, changes in the field and to represent Outreach to these agencies.
Provides direct clinical services (assessment, individual, group, family counseling, crisis intervention, toxicology testing), as needed.
Exercises due diligence in the delivery of quality care in line with agency Medicaid Compliance Plan and systems of accountability.
Insures communication through the compliance officer and/or the supervisory structure of any violation of non-compliance with the agency's Medicaid compliance plan and personnel policy and procedure.

Position: Community Support Specialist
The community support specialist is responsible for coordinating care and participating in discharge planning. Duties include addressing case management needs of adolescent clients and their families including access to employment, education, entitlements, medical and psychiatric services, housing, socialization, and other supports.
General Job Scope
Provides assessment and referral to needed services which will support treatment goals and prevent untimely discharge from treatment.
Provides clinical intervention, addressing barriers to obtaining needed services.
Advocates on behalf of clients to obtain needed services.
Coordinates case management of clients with requirements of juvenile justice, foster care, probation, or other mandating agencies.
Provides home visits to assist in assessment and engagement in treatment, and to identify barriers to family self-sufficiency.
Conducts outreach, develops working linkages and coordinates with other service providers to assist with client needs.
Attends and participates in staff, service provider and community meetings, where appropriate.
Completes all needed documentation of services and contacts in a timely manner.
Maintains communication and teamwork with clinical staff to integrate and enhance clinical services.
Conducts toxicology tests, as needed.
Liaison to district school providing a seamless transition and support after residential treatment.
Exercise due diligence in the delivery of quality care in line with agency Medicaid Compliance Plan and systems of accountability.
Insure communication through the compliance officer and/or the supervisory structure of any violation of non-compliance with the agency's Medicaid compliance plan and personnel policy and procedure.
Community management of therapeutic milieu.
Group, family and individual counseling
Discharge planning
Treatment planning, progress notes, probation reports and other related documentation.
Qualifications
CASAC-T, CASAC, LMSW or LMHC preferred.
Experience working with adolescents and their families.
Experience working in residential treatment preferred.
Experience in marketing a plus.
Bilingual Spanish preferred.
Commitment to providing quality services and outcome driven performance measures.
Ability to work effectively in collaboration with diverse groups of people.
Experience with AVATAR electronic health records a plus.
Position Status
This is a full-time non-exempt position.

This job operates in a professional office environment and outside in the field. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Must be able to travel among assigned sites and locations as needed.

Position:

Outreach is seeking a qualified candidate for the position of Part-Time Recreation Counselor for our Men and Women Residential program located in Brentwood, Long Island. The Recreation Counselor is responsible for all aspects of client recreational activities including planning, documenting, and overseeing each function. The schedule for this position is 6 to 12 hours per week to be discussed. The hours are flexible and must be able to work holidays.
In this residential setting, you will: Provide supervised highly structured indoor and outdoor recreational activities to residents. Supervise highly structured indoor recreation in the weight room with residents. You will plan and oversee special events with residents and their families. Assist with providing scheduled activities for residents in orientation, early treatment, middle treatment, and late treatment phases of the program to the supervisor. Work with supervisor to provide daily group notes indicating activities and resident participation. You will actively participate in supervision and supervise residents in self- medication. Complete accurate reporting of incident reports, resident elopements, and crisis intervention and communicate all pertinent information/incidents/emergencies via SAC book/House Manager.
