Pathways has been changing with the times since our inception in 1977, always remaining true to the vision of our founders.
As a community-based 501 (c) 3 not-for-profit organization, Pathways has been in our community since 1985 providing compassionate care and services to those in need.
Pathways has been affiliated with El Camino Hospital and Sequoia Hospital/Dignity Health for over 30 years—since 1986
In 1991 MidPeninsula Hospice Foundation was established to raise funds for hospice programs and un- and underinsured patients.
The CHANGES Program, our Children’s Grief Education and Counseling Program was developed in 1992 in direct response to the lack of bereavement services for children in the Long Beach community.
In 1997, Pathways expanded our services to include the Volunteer Caregiving Program that provides in-home supportive services for frail and chronically ill older adults.
Working with Stanford staff, Pathways gives Stanford patients this care until they are able to transition home.In 2016 Pathways implemented a new electronic health record system (EHR) named EPIC.
In 2016 Pathways entered into a partnership with Stanford HealthCare.
