Individualized Home Support Pay Rate $18.75 / Hourly Varied Hours The Family Advocacy and Community Education Center is seeking a caring, responsible individual to join our Individual Home Support team. The IHS staff member implements individualized programs as specified in Individual Service Plans, provides for individuals health and welfare, and ensures general cleanliness and safety of individuals homes. ESSENTIAL FUNCTIONS: * Instruct and assist individuals with personal hygiene, housekeeping, social skills, leisure time use, advocacy, and response to emergency situations. * Facilitate community integration by encouraging individual preference and providing opportunities for recreation activities. * Encourage and assist individuals to pursue relationships and develop rapport with local community resources such as banks, physicians, stores, etc. * Instruct, assist, and teach individuals meal planning, preparation and grocery shopping; and encourage individual participation to the fullest extent. * Monitor individuals medical health, assist in making doctor appointments, provide transportation as needed, and advocate for individuals when necessary. * Deal with immediate problems in the home; notify direct supervisor or the on-call supervisor. * Assist individuals with financial management to the degree specified in the Individual Service Plan; and manage petty cash, including documenting and providing receipts for all money spent. * Instruct and physically assist individuals with housekeeping activities on a daily and weekly basis; and promote a sense of pride and ownership in the home. * Ensure home is safe and free of clutter and debris; and instruct individuals in fire prevention and evacuation procedures. * Administer medication and/or monitor medication use for individuals, and document in accordance with Department of Public Health policies and procedures. * Ability to provide transportation with Agency or personal vehicle as required by supervisor. * Implement and document individual behavior programs where applicable. * Assist and/or support individuals in emergency situations. * Implement crisis management as needed and implement PBS consistently. * Work assigned schedule that may include weekends and/or holidays. * Comply with reassignments and directions from division Supervisor or Director. * Work modified schedule when individuals day programs are closed (site specific). * May be required to use employees personal vehicle for work purposes. * Perform other duties as assigned by the supervisor, division management team member, or division director including but not limited to working weekends and/or holidays. * Participate in all staff meetings and required training sessions, including in-service training, as scheduled. * Maintain all program and Agency required trainings. Full Time/ Salary/ $84,000 Our Residential program is seeking an Assistant Division Director. This position will provide leadership and guidance to a cluster of homes for the development and implementation of the Residential Program to enhance and encourage independent living for individuals functioning at various levels of self-sufficiency, and for the programs overall operation of twenty-four hours per day, seven days per week. . The Assistant Division Director will provide leadership and guidance to a cluster of homes. The Day Habilitation program is looking for a full-time, 40 hours per week, Paraprofessional to provide instruction and support to individuals with developmental disabilities. The employee works as a member of the Day Habilitation team to provide premier services while assisting program participants with daily living skills including but not limited to dining and personal care needs. ESSENTIAL FUNCTIONS: * Follow the daily schedule as written by the Program Director or Coordinator. * Complete daily data sheets, including documentation for behavior services. * Implement service plan goals as requested. * Inform the supervisor or nursing staff of any unusual behavior, sings of illness or injury. * Ensure individuals are actively engaged in assigned activities. * Ensure all protocols are followed to include but not limited to: toileting, dining, dietary, PICA (persistent ingestion of nonnutritive substances), behavior guidelines, seizures and safety. * Ensure positive behavior supports are implemented for all individuals. * Maintain all program required trainings. QUALIFICATION * Knowledge obtained through attainment of high school diploma/GED preferred. * One year of experience in a healthcare setting or working with individuals with a mental health or developmental disability preferred but not required. * Ability to escort, assist, and/or transport program participants. * Ability to work with mental health and/or developmentally disabled individuals. * Ability to assist and/or support individuals as needed and in emergencies that require full physical assistance. * Ability to physically restrain individuals exhibiting unsafe behavior. * Ability to follow policies, procedures, and directions. * Ability to work and communicate with others. * A qualifying criminal background check required, qualifying driving record, and a valid drivers license are preferred. Our Day Habilitation program is looking for an employee to serves as an interdisciplinary team member responsible for designing, implementing, and supervising services and assumes primary responsibility for each member on an assigned caseload to coordinate the implementation of the program Service Plan and maintain each members record. ESSENTIAL FUNCTIONS: * Recommend service plan goals, assist in completing annual severity profiles, and service needs assessments, as well as review new potential program individuals. (not applicable for relief employees) * Complete daily data sheets, including documentation for behavior services. * Implement service plan goals, including therapeutic goals. * Inform the supervisor or nursing staff of any unusual behavior and signs of illness or injury. * Ensure all schedules are followed and individuals are engaged in activities. * Ensure all protocols are followed, including, but not limited to, toileting, dining, dietary, Pica (persistent ingestion of nonnutritive substances), behavior guidelines, seizure, and safety. * Complete daily communication logs. * Complete monthly progress reports of service plan goals. (or as requested for relief staff) * Maintain all program required trainings. * Attend interdisciplinary team meetings as needed to participate in curriculum design. (or as requested for relief staff) * Implement curriculum in accordance with individual service plans. * Ensure positive behavior supports are implemented for all individuals. * Maintain an expertise in the day habilitation field through attendance at meetings, in-service training, continuing education, or other appropriate educational programs. * Perform other duties as assigned by the supervisor, division management team member, or division director, including, but not limited to, working weekends and/or holidays. * Participate in all staff meetings and required training sessions, including in-service training, as scheduled. * Maintain all program and Agency required trainings. QUALIFICATIONS: * High school diploma or GED is required. Experience working with adults with developmental disabilities or a health care setting is preferred. * Skill in communicating and working both independently and as a team. * Ability to escort, assist, and/or transport program participants. * Ability to work with mental health and/or developmentally disabled individuals. * Ability to effectively participate in the design, implementation, and supervision of services. * Ability to complete and disseminate all required forms and documents. * Ability to assist and/or support consumers as needed and in emergencies that require full physical assistance. * Ability to physically restrain consumers exhibiting unsafe behavior. * Ability to follow policies, procedures, and directions. * Ability to work and communicate with others. * A qualifying criminal background check, driving record, and a valid drivers license are required.
