The Hospice LMSW is responsible for the provision, direction, supervision and evaluation of medical social services to clients in the home environment in accordance with agency policies and procedures, applicable health standards, governmental laws and regulations. The Hospice MSW will strive to proactively meet the needs of the client in the achievement of individualized and measurable outcomes. Responsibilities: Participates in development and re-evaluation of POC for clients needing MSW. Instructs and counsels the client and family in treating and coping with the social and emotional responses to illness. Participates in discharge planning. The primary purpose of the Clinical Liaison is to serve as a clinical leader and centralized point of contact responsible for assessing patient eligibility, supporting goals-of-care discussions, and coordinating the timely and appropriate transition of patients from the Palliative Care Division to Hospice Care. This role ensures continuity of care, regulatory compliance, and alignment with patient and family goals as clinical needs evolve. The Nurse Navigator communicates, coordinates, and directs patient care transitions in collaboration with palliative care providers, hospice teams, attending physicians, facility partners, hospitals, and internal referral and admission teams. The role supports high-level accounts and referral relationships by facilitating referral management, community education, and outreach initiatives that promote awareness of hospice eligibility and end-of-life care options. In this role, the Nurse Navigator assists with and leads goals-of-care conversations; provides education to patients, families, and healthcare partners regarding disease progression, hospice eligibility criteria, and end-of-life services; and fosters effective communication between the palliative care team, hospice interdisciplinary team, and facility staff. The NN ensures appropriate follow-up with facility staff, physicians, hospitals, patients, and families to support informed decision-making and seamless care transitions. The NN conducts visits to patient homes, facilities, hospitals, and physician offices as needed; collaborates closely with referral and admission coordinators; and coordinates the provision of end-of-life resources and services on behalf of patients and families, ensuring compassionate, patient-centered, and clinically appropriate care throughout the transition process. Make sure patient is as comfortable as possible at all times Changing bed linens when necessary and keep linens tight without wrinkles under the patient. Duties include assisting patients with daily mouth care, bath functions, dressing/undressing, hair care functions, non-diabetic nail care, shaving, toileting, feeding, giving back rubs, assisting with exercises and ambulation, applying elastic stockings and/or heel/elbow protectors, checking intake/output records, checking bowel movements and character of stools, preparing and giving enemas, assist with lifting, turning, moving, positioning, and transferring into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., assisting with the care of the dying patient, providing post mortem care as instructed, observing and reporting the presence of pressure areas and skin breakdown, report injuries of unknown source, including skin tears, assisting with special treatments as instructed (i.e. indwelling catheter care, perineal care, nonsterile vaginal irrigation/douches, application of bandages, etc.). Document tasks performed and other comments related to visit. Communicate with family or facility staff to ensure nursing services can be adequately maintained to meet the needs of the patient.
