Associate Medical Director - Inpatient Palliative Care
Schedule: Monday-Friday, 8:30 AM - 5:00 PM Reports To: Chief Medical Officer, Providence Care
Providence Care is seeking a dedicated and compassionate Associate Medical Director for Inpatient Palliative Care to lead and provide high-quality palliative care services across two hospitals. This physician will serve as a key member of the interdisciplinary palliative care team, focusing on symptom management, goals-of-care discussions, and care coordination for patients with serious illness.
Responsibilities
Provide inpatient palliative care consultations and management for hospitalized patients.
Supervise and collaborate with one dedicated palliative care nurse practitioner, as well as provide consultation and support for other nurse practitioners on the palliative care and hospice teams.
Participate in interdisciplinary rounds and coordinate closely with hospitalists, specialists, nursing staff, and case management.
Maintain availability for minimal procedural weekend call, primarily consultative in nature.
Attend bi-monthly (2x/month) house calls/palliative care team meetings.
Engage in leadership and teaching opportunities, including mentoring nurse practitioners and educating nursing staff on palliative and hospice principles.
Option to assume a Hospice Medical Director role if desired, offering opportunities for expanded leadership and cross-continuum care delivery.

The primary purpose of the Clinical Liaison is to serve as a clinical leader and centralized point of contact responsible for assessing patient eligibility, supporting goals-of-care discussions, and coordinating the timely and appropriate transition of patients from the Palliative Care Division to Hospice Care. This role ensures continuity of care, regulatory compliance, and alignment with patient and family goals as clinical needs evolve.
The Nurse Navigator communicates, coordinates, and directs patient care transitions in collaboration with palliative care providers, hospice teams, attending physicians, facility partners, hospitals, and internal referral and admission teams. The role supports high-level accounts and referral relationships by facilitating referral management, community education, and outreach initiatives that promote awareness of hospice eligibility and end-of-life care options.
In this role, the Nurse Navigator assists with and leads goals-of-care conversations; provides education to patients, families, and healthcare partners regarding disease progression, hospice eligibility criteria, and end-of-life services; and fosters effective communication between the palliative care team, hospice interdisciplinary team, and facility staff. The NN ensures appropriate follow-up with facility staff, physicians, hospitals, patients, and families to support informed decision-making and seamless care transitions.
The NN conducts visits to patient homes, facilities, hospitals, and physician offices as needed; collaborates closely with referral and admission coordinators; and coordinates the provision of end-of-life resources and services on behalf of patients and families, ensuring compassionate, patient-centered, and clinically appropriate care throughout the transition process.
