Completes quality control and DUR check on specialty prescriptions before written: (1) Drug/Drug interactions; (2) Drug/Disease contraindications; (3) Duplicate therapies; (4) Drug/Formulary requirements; and (5) Drug/Lab contraindications; Provides clinical advice to providers regarding medication treatment plans; Propose maintenance medication dosage adjustments while taking specialty meds; Propose monitoring parameters while taking specialty meds; Resolves prescription discrepancies; and Documents patient interventions with adequate detail in both clinical and non-clinical records as appropriate
2. Obtains insurance authorizations as required; Investigates and manages insurance denials as required; Coordinates between clinical staff, insurance companies, and specialty pharmacies for preparation, clarification, questions, and appeal of prior authorization requests.
3. Provides instructions for taking prescribed specialty medication via in office visit or telephone conversation; Provides instructions for any maintenance medications changes necessary during specialty drug treatment; Creates and provides treatment plan to patient to include appointment and lab requirements to include insurance requirements; Monitors patient compliance to appointments and lab work to ensure prior authorization requirements are met; Monitors adherence to specialty medication; Answers patient, pharmacy, insurance, and outside provider questions while patient is taking specialty medication; Resolves patient, pharmacy, insurance problems while patient is taking specialty medication; and documents patient interventions with adequate detail in both clinical and non-clinical records as appropriate.
4. Manages subordinate employees, including: (1) Pharmacy Technicians and (2) Pharmacy team members; Provides direct supervision of the processes involved contacting pharmacies, insurance companies, and patients;
5. Coordinate Between HOC office and telehealth site offices as needed regarding treatment plans, labs draws, and lab results.
6. Contributes to the development and review of the patient management program; Participates in and delivers patient management program services; Participates in the Quality Management.
7. Responsible for the overall safety and security of the office and its operations; Enforces all federal, state and local laws and regulations pertaining to patient privacy, as well as company policies and procedures.
8. Performs other duties as assigned.

TITLE: Quality Improvement Registered Nurse
REPORTS TO: Director of Quality Assurance and Risk Management
SUMMARY: Provide support to the Director of Quality in the planning and implementation of a systematic program for outcomes management and quality improvement efforts at the Center. In conjunction with the Quality team this position is responsible for ensuring compliance with internal as well as external regulatory reporting requirements. Review applicable employee immunization records and maintain OSHA exposure files. Works closely with clinical staff in the optimization of workflows for primary care practices as they make the transition to value-based, population health payment models. The goal is to help providers measurably improve quality scores through improved training, workflows in offices, and data capture.
DUTIES AND RESPONSIBILITIES:
1. Works in a collaborative manner with providers, administrators, department managers and others to identify and meet the continuous performance improvement requirements of the organization. Demonstrates performance improvement in the following areas as appropriate: clinical care outcomes, customer/service improvement, operational system/process, and safety.
2. Performs research, collects and/or analyzes data reports, and implements processes to achieve improved performance. Work with Director of Quality to prepare and present summary reports and trend analyses of program activities to the QA/PI committee on a regularly scheduled basis, using data obtained from sources such as internal PI/RM reports, staff and employee surveys, incident and variance reports and screens, self-inspection programs and internal & external on site audits for compliance.
3. Is responsible for Pay for Performance and Accountable Care Organization-related functions to include clinical outcome metrics review, personal health assessment, collaborating with Population Health Team and Value Based health initiatives, including hospitals for timely notification of ER visits, hospital admissions, and discharges, collaborating with sub-acute facilities for beneficiary admission, status, and discharge planning, and any other administrative activities related to the overall clinical care improvement of members.
4. Facilitates/leads organizational improvement activities utilizing the PDSA improvement methodology. Recommends tools and assists teams throughout the improvement process. Conduct concurrent reviews of the literature and websites to provide teams and service lines with up-to-date evidence and best practice recommendations.
5. Organizes and facilitates meetings and special events for the Quality Management Department; schedules and coordinates dates and times, venues, attendance, agendas, and facilities; takes minutes, and provides administrative support and follow-up on matters arising from meetings.

JOB QUALIFICATIONS:
Doctor of Pharmacy Degree in Pharmacy or PharmD or a Bachelor of Science Degree in Pharmacy required
Must have a current State of Texas Pharmacy License and be in good standing with the Texas State Board of Pharmacy (TSBP)
Minimum one (1) year experience as a registered pharmacist
Must be knowledgeable of applicable federal and state pharmacy laws
Minimum one (1) year experience working with patient management programs
Certified in Basic Life Support (BLS), American Heart Association
