Function/Duties of Position
Direct Patient Services and Documentation
* Provide primary care behavioral health interventions including proactive patient identification (scrubbing), brief interventions via same-day Warm Hand Offs, scheduled patient follow-ups, consultation to all care team members, assessments, and crisis intervention.
* Participate in behavioral health case management as part of multi-disciplinary team to address complex patient needs appropriately. This includes both short- and long-term involvement and a high level of coordination with internal and community-based resources.
* Maintains a professional presence for the clinic and actively engages with teams and stakeholders.
* Documents services appropriately in EHR in accordance with existing workflows to ensure continuity of care.
* Maintains technology fluency in support of in-clinic and telehealth.
* Maintains accessibility via department standard means (phone, pager, email, etc) Maintains appropriate professional boundaries as a part of the primary care team.
Teaching
* Provide effective teaching to all clinic learners including medical residents and care team members regarding behavioral health issues, evidence-based interventions, primary care integration, and patient engagement/ communication strategies.

As a member of the Care Team, the Behavioral Health Consultant (BHC) will provide support and mental health services to the NHC patient population. BHC will provide community resources and coordinate referrals as indicated. Principal Duties and Responsibilities

Compliance and Documentation

infection control, equipment operation, confidentiality, and other applicable areas (e.g., patient
identification and standard precautions).
3. Participate in proficiency testing as required by NHC or departmental policy to ensure that proper standards of
care are being maintained.

Service Position Summary
Essential Functions of Position (all basic duties that must be performed with or without reasonable accommodations)
Commitment Required
* Members will complete various hours of service over the course of 11 months based on the start and end date of October 1st, 2025 - August 31, 2026.
* Member hours commitments are dependent upon the position
* Members will be expected to work a schedule that is agreed upon between both the program director and the member. Some weekend and evening service could be required
The member will complete a minimum of hours of service during the term of service indicated in this position description:
* Reduced Full-Time Members must serve 1200 hours during a period of 11 months (110 hrs/month).
* Half Time Members must serve at least 900 hours during a period of 11 months (82 hrs/month).
* Reduced Half-Time Members must serve at least 675 hours during a period of 11 months (62 hrs/month).
* Quarter-Time Members must serve at least 450 hours during a period of 11 months (41 hours/month).
* Minimum Time Members must serve at least 300 hours during a period of 11 months (28 hrs/month).
Necessary criminal history background checks Members must agree to conduct - National Sex Offender check, State criminal check, and FBI check and notification that selection for service in the AmeriCorps program is contingent upon the organization's review of the NSCHC component results

The Behavioral Health EAP Consultant II is responsible for providing 24/7 Behavioral Health and Employee Assistance services to up to approximately 10 million lives.
How you will make an impact:
* Supports Employee Assistance Program (EAP) service department by responding to initial customer contacts and request for services; receives client request, assesses situation, determines appropriate course of action.
* Provides telephone and/or on-site support for crisis intervention, assessment, short term problem resolution and referral to appropriate provider(s).
* Manages Critical Incident Stress Debriefing requests.
* Assesses members and assists them in accessing behavioral healthcare benefits available under their health plan.
* Applies specialized knowledge in substance abuse, domestic abuse, grief counseling, workplace problems, and management consultation.
* Provides employer consultation to supervisors, managers, and Human Resources of the EAP contracted organization.
* Manages shared mailbox that is used by clients and external vendor to forward information that must be reviewed and placed in documentation system.
Minimum Requirements:
* Requires a HS diploma or equivalent and a minimum of 5 years of direct psychiatric and/or substance abuse experience and prior managed care experience; or any combination of education and experience, which would provide an equivalent background.
* Current unrestricted license such as (but not limited to) LPC (as allowed by applicable state laws), LCSW, LMSW, LMFT, LMHC, or Licensed Professional Counselor, in applicable state(s) required.
Preferred skills, capabilities, and experiences:
* MA/MS preferred.
