Key Responsibilities:
Deliver in-home physical therapy services under the supervision of a licensed Physical Therapist
Carry out treatment plans to help patients regain mobility, function, and independence
Communicate progress and changes in patient condition to the supervising therapist
Educate patients and caregivers on exercises and safety practices
Document visits and progress notes accurately and on time
Participate in care team meetings and interdisciplinary planning
Uphold infection control and safety standards during all home visits
Maintain professional, compassionate rapport with patients and families
