The purpose of this service is to i) de-escalate the behavioral health crisis and prevent harm to the individual or others; ii) assist in the prevention of an individuals acute exacerbation of symptoms; iii) development of an immediate plan to maintain safety; and iv) coordination of care and linking to appropriate treatment services to meet the needs of the individual. The service is approved for 8 hours (32 units). Duties & Responsibilities: General responsibilities include completing pre-screenings, pre, initial, and re-assessments, client ISPs, prior authorizations, and clinical supervision of staff. Complete Pre-Screening Assessment to determine clients eligibility for services. Schedule to meet with client and/or family to conduct a Biopsychosocial Assessment Collaborate with referral sources and other service providers to gather data to complete assessment. Complete Biopsychosocial assessment and addendum for agency and make appropriate service recommendations. Remain within the guideline of the referral at all times. Complete necessary legal documentation and obtain signatures of client and legal guardian(s). Complete Preliminary Individualized Service Plans. Complete Prior Authorizations. Maintain contact with Program Directors and provide updates of status of referrals. Type Biopsychosocial Assessment and Prior Authorization (Within 24 hours ) and submit to Clinical Director/Program Director for review. Responsible for development of treatment plans, therapy notes, and written reports. Provides case management services, individual, couples, group, and/or family therapy. Provides discharge planning including target discharge date and sets preliminary goals. Maintains on-going discussion with each client on their progress in reaching their goals. Monitors treatment planning and adjust treatment intensity as indicated by progress of client. Encourage client and/or guardian to schedule needed appointments with health care providers. Implement and utilize resources during sessions as deemed clinically appropriate based on clients individualized issues. Maintain a strengths-based approach while working with clients to assist in the retention of dignity and the development of intrinsic qualities. Maintains on-going documentation of all collateral contacts. Remain abreast of changes within the human services field. Responsible for the upkeep, accuracy, and completeness of case files. In consultation with the Clinical Director, refers clients to supportive services for supplemental treatment and counseling when applicable. Provides on-call support and coverage for clinical services. Complete other duties as assigned.
