MAIN FUNCTION:
The Pioneer Valley Memory Care Initiative (PVMCI) is a social model of supportive care composed of a coalition of providers from Cooley Dickinson Health Care (CDHC), Cooley Dickinson Medical Group, Valley Medical Group, CDHC VNA & Hospice, Neighborhood Organizations, Alzheimer's Association and other health care & community providers created to provide social support, education and training to those individuals living with memory loss, dementia, and Alzheimer's disease and their family caregivers (dyad).
The Memory Care Specialist (MCS) is a core member of the PVMCI team composed of a Program manager, Geriatrician and Geriatric nurse. You will work collaboratively as part of the PVMCI interdisciplinary care team to deliver innovative, community oriented supportive care. You are the primary contact person to those enrolled in PVMCI, responsible for addressing the needs of individuals living with memory loss, dementia and Alzheimer's disease and their family caregiver-the dyad, enabling them to age successfully in the community setting.
This is a grant funded position allowing the candidate to participate in program growth and expansion.
This position reports to the Program Manager and operates within established organizational and departmental policies and procedures.
MINIMUM REQUIREMENTS:
* Bachelor's Degree required; license or certification in specialty field (RN, PT, OT) with demonstrated experience and skills to successfully perform the assigned duties and responsibilities strongly preferred
* Minimum of two years of relevant experience in working with older adults, preferably those with dementia and Alzheimer's disease, people with disabilities, and other complex medical needs in a health care, long-term care, social service or community setting required
* Proficient and effective verbal and written communication and presentation skills, organizational skills, and problem-solving abilities.
* Ability to work effectively in an organized fashion in a fast-paced, team-based environment and complete tasks and documentation within specified time frame.
* Computer proficiency and use of Outlook, Microsoft Word, PowerPoint, and Windows and other web-based programs.
* Knowledge of community resources and services and ability to negotiate securing such services to address needs of dyads required
* Highly self-motivated, creative and passionate about working with older adults and people living with memory loss, dementia and their family caregivers.
