GENERAL STATEMENT OF DUTIES:
Under the direction of the Unit Supervisor, functions as a team member providing professional services in accordance with the person-centered plans of person served, and coordinate the provision of supports and services for adults with serious and persistent mental illness and/or developmental disabilities who need assistance in accessing, coordinating and monitoring resources and services. Assists the individual and their support network to identify, select, obtain and coordinate both paid and unpaid or natural supports to enhance the person's independence, integration, and productivity consistent with the person's capabilities and preferences as outlined in their Individualized Plan of Services (IPOS); performs related work as required.

MINIMUM REQUIREMENTS:
Bachelor’s degree in Human Services (Or a related field such as Child Development/Family Relations, Occupational Therapy, Community Mental Health, Chemical Dependence, Physical Therapy, Counseling, Criminal Justice, Recreational Therapy, Rehabilitation Counseling, Social Work, Psychology, Education, or Sociology).

ADDITIONAL REQUIREMENTS:
Must have a valid Michigan Driver’s License, current automobile insurance, and the use of a personal vehicle during work hours. Must be willing to transport persons served using personal or agency vehicles.

PREFERENCES:
LBSW or LMSW AND one (1) year of clinical experience with adults with severe mental illness or developmental disabilities.

Seven Hills Behavioral Health is seeking a compassionate individual who is looking for a great opportunity to grow a rewarding and meaningful career in human services. The Rapid Response Team (RRT) - a new service model launched by Open Sky in January 2022 - is a multidisciplinary team that swiftly provides individualized, short-term interventions to people diagnosed with a mental illness, developmental disability, and/or substance use disorder, and are experiencing imminent clinical deterioration requiring crisis-level response. As a Designated Contract Organization (DCO) of the Open Sky Rapid Response Team, Seven Hills Behavioral Health provides ongoing clinical care to children and families seen through the Rapid Response Hub.

Targeted Case Managers are caring, organized, creative, and persistent. They engage with the individual in need and support them with gaining access to treatment services and community resources. They develop and assist in implementing support plans in collaboration with medical and behavioral health providers and other applicable community resources. They play a critical role in facilitating a fast turnaround for the most dire of situations.

The Targeted Case Manager hired by Seven Hills Behavioral Health will work with the Open Sky Rapid Response Team and the Seven Hills Behavioral Health Outpatient Services to support children and families seen at the Rapid Response Hub in accessing ongoing clinical supports through Seven Hills Behavioral Health.

Schedule: Tuesday 12pm - 8pm, Wednesday - Friday 8am - 5pm, Saturday 10am - 3pm (some flexibility with start times Wednesday - Friday)

**$2,500 New Hire Sign-On Bonus! (50% paid at 6 months and 50% paid at 12 months of continuous employment)

+ Engage with individuals as they enter the treatment system and support them in gaining access to treatment services and community resources.

+ Complete assessments. Develop and implement support plans. Record progress. Complete all documentation within a timely manner.

+ Act as a liaison between Open Sky & Seven Hills Behavioral Health team members, community agencies, and families to coordinate treatment, including: employment and education entities, Care Coordination entities, Emergency Service Providers, outpatient clinics, and others.

+ Exercise good judgment. Recognize and respond appropriately to risk-related situations. Communicate/ask for support from the team and program leadership when needed.
+ Provide temporary transportation assistance to clients as needed.

+ Bachelor's degree in a related field preferred. Alternative education/experience in lieu of formal education will be considered. Experience working with children and families impacted by persistent mental illness, substance use disorders, and/or underserved populations preferred.

+ Ability to stay organized and prioritize while managing a dynamic workload.

+ Creativity and resourcefulness. Knowledge of local resources and experience navigating treatment systems preferred.

+ Reliability, drive to learn, and compassion for those who are struggling.

+ Valid Driver's License.

Education & Professional Requirements:

• Bachelor’s Degree from an accredited institution in a human services field with a minimum of 1 year of experience in a behavioral health setting post-bachelor’s degree.
• Certification in the following Department For Behavioral Health, Developmental and Intellectual Disabilities approved trainings to meet the requirements outlined in 908 KAR 2:260:
  • Twelve (12) hour Targeted Case Management Core Training
  • Six (6) hour Substance Use Disorder Targeted Case Management Training
  • Six (6) hour Severe Emotional Disability Targeted Case Management Training
  • Six (6) hour Severe Mental Illness Targeted Case Management Training
  • These must be completed within six (6) months of date of contract and access to trainings will be provided by KYUMH. There will be a stipend paid when proof of completion is provided.
