ADDITIONAL REQUIREMENTS:
Must have a valid Michigan Driver’s License, current automobile insurance, and the use of a personal vehicle during work hours. Must be willing to transport persons served using personal or agency vehicles.
PREFERENCES:
LBSW or LMSW AND one (1) year of clinical experience with adults with severe mental illness or developmental disabilities.
Veterans must submit Form DD214 to determine eligibility for Veterans’ Preference

Seven Hills Behavioral Health is seeking a compassionate individual who is looking for a great opportunity to grow a rewarding and meaningful career in human services. The Rapid Response Team (RRT) - a new service model launched by Open Sky in January 2022 - is a multidisciplinary team that swiftly provides individualized, short-term interventions to people diagnosed with a mental illness, developmental disability, and/or substance use disorder, and are experiencing imminent clinical deterioration requiring crisis-level response. As a Designated Contract Organization (DCO) of the Open Sky Rapid Response Team, Seven Hills Behavioral Health provides ongoing clinical care to children and families seen through the Rapid Response Hub.
Targeted Case Managers are caring, organized, creative, and persistent. They engage with the individual in need and support them with gaining access to treatment services and community resources. They develop and assist in implementing support plans in collaboration with medical and behavioral health providers and other applicable community resources. They play a critical role in facilitating a fast turnaround for the most dire of situations.
The Targeted Case Manager hired by Seven Hills Behavioral Health will work with the Open Sky Rapid Response Team and the Seven Hills Behavioral Health Outpatient Services to support children and families seen at the Rapid Response Hub in accessing ongoing clinical supports through Seven Hills Behavioral Health.

- Bachelor’s Degree from an accredited institution in a human services field with a minimum of 1 year of experience in a behavioral health setting post-bachelor’s degree.

Certification in the following Department For Behavioral Health, Developmental and Intellectual Disabilities approved trainings to meet the requirements outlined in 908 KAR 2:260:
