Second, you will develop care plans for our members, in collaboration with our onboarding RNs, to address clinical and non-clinical needs and barriers. You will prioritize members' goals and develop concrete plans for our Oncology Nurse Navigators and Community Health Workers to reach these goals. To do this, you will review available medical records, work with external providers, give clinical insights to our care team, and adhere to best documentation practices to drive high-quality care for our members. Third, you will be responsible for the oversight and direction of interdisciplinary case conferences for our members, in which individualized care plans are created and monitored by our team of Community Health Workers and Oncology Nurse Navigators. You will provide clinical oversight to the case management team and direct clinical care, as needed.
