ESSENTIAL JOB FUNCTIONS Safely drive in a manner that adheres to the saying on the bus: SAFE, RELIABLE, COURTEOUS. Daily safety and maintenance inspection of vehicle. Relate positively to all passengers. Promote The Whole Person and encourage ridership. Perform minor maintenance (e.g., van washing). Report maintenance problems to the Supervisor. Report accidents and breakdowns to the Supervisor immediately. Comply with all vehicle operation rules and regulations. Complete required paperwork in a timely manner. Attend training meetings and, where appropriate, county support committee meetings. Regular and predictable attendance REQUIRED SKILLS Missouri Class E Driver's License is Required High School Diploma or GED 2+ years' experience driving professionally Must have a clean driving record with DMV Must be able to operate wheel-chair lift and ramp Customer Service experience General knowledge of vehicles and maintenance Must be able to work flexible hours - not to exceed 40 per week. WORK ENVIRONMENT & PHYSICAL DEMANDS Sitting on a daily basis for many hours at a time, driving, walking to and from the door of the destination and assisting passengers, bending/stooping on a daily basis while lifting packages, assisting passengers or doing vehicle maintenance, pushing and pulling daily to operate lift or ramp. Driver must also manage riders in wheelchairs. This entails negotiating single steps or curbs, positioning the wheelchair on the lift platform, maneuvering the wheelchair inside the vehicle and securing the wheelchair with the tie-down system. The weight of a wheelchair and passenger can easily exceed 200 pounds. All drivers receive training on wheelchair management and are encouraged to know their limits. The main responsibility of the specialist is to develop a strong relationship with their consumer and act as a liaison for The Whole Person. The specialist provides CDS support to consumers via monthly phone calls and annual visits to evaluate the actual living conditions of the consumer's home. Specialists also contact the Consumer via phone about timesheet corrections for their Personal Care Attendant(s) services. Specialists refer consumers to other TWP services and/or resources in the Community. Specialists also produce efficient, effective, and timely case notes in the CIL Suite Database and according to departmental policies. ESSENTIAL JOB FUNCTIONS - LEVEL I Work with CDS staff to train consumers and PCAs on Blue Summit Telephony system as part of the CDS transition from HHA Carry a typical case load of Consumers as assigned by Manager Contact each Consumer, at a minimum, once monthly via phone or face-to-face per department policy. Visit each Consumer on an annual basis to evaluate their living conditions. Act as Advocate on Duty 2-4 times a month for a four-hour period to provide information and referral services to include advocacy, peer support, transition or refer consumers to other staff to provide these services. Follow department policies regarding placing CIL Suite database entries in a timely and accurate manner. Assist in payroll corrections for Consumers and their Personal Care Attendants Assist Missouri CDS Payroll with telephony and timesheet corrections to ensure timely payment to Personal Care Attendants. Provide on-site and off-site supports to youth and adult consumers, already on your caseload, to develop goal centered independent living plans (may include evenings and weekends). Assess consumer's current status of mental health through observation and from consumer's self-report, as well as use of assessment tools (as needed), to determine appropriate referral to mental health support services and treatment. Upon request of Parents or School, participate in Individualized Education Plan (IEP) meetings as well as other community meetings with youth and adults for the purpose of increasing independence and coordinating services. Effectively manage caseload of consumers, maintaining contact with consumers per department standards to ensure goal progression, need for revisions, and closure of cases.
