What is a transition coordinator and how to become one

Transition coordinators are special educators who help people manage major changes in their lives. They assist their clients from moving out of assisted housing, transitioning from student life to adulthood, and similar life-altering social situations. Working as a transition coordinator, you will be responsible for disseminating information about transition services and mediate between educational or housing institutions and individuals who need your help.

You will evaluate the situations of people in need and advise on options for transition programs. Most likely, you will work for education or health care. A master's degree is usually a requirement to fill this role, but you might be certified as a counselor or licensed as a social worker as well.
