Essential Functions:
1. Reviews active reports along with management to determine the need for additional support in the academic areas.
2. Prepares lesson plans and structured activities for tutoring and evening study activities.
3. Ensures that all materials needed for the evening study program are ordered and available for student use.
4. Makes recommendations for new and innovative programming that will enhance the evening study program.
5. Maintains correspondence with academic and career and technical training instructors as to the progress of students in the evening study program.
6. Issue assignments in a variety of academic disciplines to ensure proficiency in every area.
7. Prepares interactive lessons and activities to encourage group thinking and problem solving.
8. Prepares and provides homework and assignments for all students who attend tutoring or evening study programs.
9. Ensure interaction with career and technical instructors to infuse the implied academic theory.

General Description of Position: To provide direct, person-centered instruction to young adults in a community based, school-to-work transition program focused on increasing independence in the areas of employment skills, independent living skills and community involvement.
Essential Functions:
- Provide direct instruction of functional academics and independent living skills
- Develop lesson plans and differentiate activities and address goals and objectives
- Propose SMART goals and objectives while attending PPT/IP meetings
- Provide job coaching support
- Completion of daily, quarterly, and annual data collection and contribute information to reports.
- Promote increased independence in program participants
- Complete skills assessments at regular intervals
- Assist in coordination of work/internships/community outings.
- assist in planning and execution of two "family nights" per year.
- Adhere to agency policies and procedures.
- Maintain good daily attendance (days of absence, illness, tardiness.)
- Attends and participates in a mandatory training, in-services, staff meetings and conferences as required.
- Demonstrates knowledge of emergency procedures and responds appropriately to emergency situations.
Other Related Responsibilities:
- Pass medication as needed
- Transport participants to activities and worksites in agency vehicles.
- Follow Behavioral Support Plans and provide PMT interventions when necessary.

Minimum Requirements:
- High School Diploma/GED
- Valid Driver’s License and be insurable with Agency carrier.
- Must be fully vaccinated against COVID-19.
- Previous experience working in a transitional program preferred.
- Familiarity with EIP process and goal writing and lesson plans.
- Ability to differentiate and teach functional academic lessons to a wide variety of learners.
- Job coaching experience required.
- Good oral and written communication skills.
- Commitment to Person Centered Planning.
- Knowledge of basic kitchen safety and ability to follow a recipe to cook basic meals.
- Good computer skills (Microsoft Office Suite, Outlook, Teams) and experience with Therap preferred.
- Certification and training included but not limited to: PMT, CPR, First Aid, AED, DDS Medication Administration.
Other Requirements:
- Ability to obtain PSL "A" Endorsement on Driver's License and pass DOT physical to maintain Medical Examiners card.
- Ability to stand for 4+ hour shifts
