CHECK-IN
1. Check-in new and return patients, obtaining new patient paperwork on new patients and updating any demographic or insurance information on return patients. Make sure HIPAA forms are up-to-date, as well as consents and Welcome forms.
2. Collect and post any co-pays, balances and bad debts that are required at time of service.
3. Call no-show and cancellation patients to reschedule.
4. Balance cash drawer and front office journal at the end of the day and lock money in safe.
CHECK-OUT
5. Check-out patients and schedule follow-up appointments and classes according to encounter form from provider.
6. Maintain bariatric product supply room. Sell and bag products for the patients.
7. Prepare encounter forms for next day's patients.
8. Maintain cash drawer and balance at the end of the day. Lock money in the safe.
MEDICAL RECORDS
9. Maintain faxes throughout the day. Call patients that have re-scheduled through confirmation calls.
10. Process medical reports and schedule patients as needed.
11. Process medical records requests. Send out no-show and cancellation letters.
MISCELLANEOUS
12. Answer phone and route calls appropriate, mainly through tasking in Intergy.
13. Scan any necessary paperwork and records into EHR.
14. Schedule patients from phone calls and faxed requests.
15. Process incoming referrals, gather medical records for provider approval and call patients to schedule appts.

Essential Duties & Responsibilities
The RN Patient Care Coordinator is a full-time position for an experienced RN who will serve as the contact point, advocate, and informational resource for patients, family/caregiver(s), PCP, payers, and community resources. The Patient Care Coordinator is responsible for engaging, cultivating, and supporting primary care and specialty providers co-management with timely communication, inquiry, follow-up, and integration of information regarding transitions of care and referrals. The key focus of this outreach is to improve the health of patients and reduce health care costs for the managed population of patients.

Qualifications
· Licensed Registered Nurse
· 3-5 years' experience in clinical setting
· Evidence of essential leadership, communication, education, and counseling skills
