Essential Responsibilities:
- Hold a caseload of up to 25 people whom you get to know, support, and champion.
- Engage in communication with support teams, planning.
- Attend meetings, advocate for the person you serve, and provide them with training.
- You will become credentialed in the MN Employment Services field.

- Bachelors degree in Vocational Rehabilitation or related human services field preferred. Experience in lieu of a degree will be considered. If you hold a Social Work degree, we expect you to be licensed by the Minnesota Board of Social Work, as required.
- Knowledge of Vocational Rehabilitation Services system, documentation requirements, and funding requirements.
- Ability to complete Discovering Personal Genius training and demonstrate competence in training principles.
- Demonstrate good written and oral communication skills as well as time management skills.
- Must have a valid drivers license, clean driving record, and a vehicle.
- Must pass a Department of Human Services background study.
Work Environment
Typical work days are Monday-Friday 8am-4:30pm, some evenings or weekends for support as needed. 40 hour work week- mostly in the community with people, but also working remotely at times. A supportive and interactive work team.

RESPONSIBILITIES:
1. Provides case work services to Fairweather lodge residents, participants of Supportive Housing & Vocational Services, Pre-Employment Transition Services (Pre-ETS) and Community Employment & Employment Related Services (CEERS) S.
2. Meets with each participant of programs weekly, individually to discuss goals and objectives.
3. Writes all goals in behavioral terms.
4. Perform job development in community as needed in order to create paid employment opportunities and work-based learning opportunities.
5. Provides onsite support for residents by attending weekly house meeting at lodge and on site job supervision.
6. Develops creative ways to challenge residents/consumers to stay involved, increase work skills and habits, and socialize in an effective manner for community living.
7. Attends all State facilitated Fairweather Lodge Meetings.
8. Serves as an effective role model for good work habits, good grooming and appropriate social skills. Participates in activities on site and in the community in a manner that is professional.
9. Networks and maintains positive relationships with those involved in the participant's life; such as other mental health professionals, employers, family members, and significant others in order to provide effective services.

16. Create, conduct pre-vocational groups and camps in accordance with OVR provider agreement.
17. Will provide pre-vocational activities and job seeking skills training per client need to aid them in identifying strengths, limitations and suitable vocational goals.
18. Will provide job coaching for all persons who find employment and need on site (and off site) assistance regardless of the schedule, ie.outside of the normal work hours and on weekends.
19. Collect both indirect employer observation and direct observation data at the job site, per client need.
20. Completes employer satisfaction surveys after 30 days and 90 days.
21. Will provide on and off site support for consumers to include making all necessary referrals and linkages to insure adequate support after fading.
22. Schedules appointments and coordinates follow-up care to meet the needs of the clients.
23. Will adhere to the schedule submitted at the beginning of each week, or will notify supervisor of changes.
24. Meets with Supervisor for individual/group supervisions as scheduled and participates in team meetings.

30. Must be flexible in provision of service, for example; in person, via zoom, over the phone or in the community to suit client needs along with the health and safety due to unforeseen circumstances.
