POSITION OVERVIEW
Under the direction of the Executive Director, the Medical Director of La Casa de Salud, Inc (LCDS) is responsible for clinical oversight and leadership of all medical services provided by the organization. This position ensures the delivery of high-quality, patient-centered care that meets the needs of the community served by LCDS.
The Medical Director will establish, implement, and maintain the highest standards of professional medical care for patients, guiding clinical protocols and fostering a culture of excellence in healthcare delivery. In addition, this role involves identifying and implementing innovative ambulatory care services tailored to the patient population and community at LCDS.
KEY ESSENTIAL FUNCTIONS
Recruit, supervise, and evaluate the performance of medical personnel, including physicians, medical subspecialists, dentists, and mid-level practitioners.
Collaborate with the Executive Director and senior leadership to align clinical operations with the organization's strategic goals.
Provide clinical oversight of all clinical programs.
Oversee laboratory contract services and monitor contracts for medical supplies, services, and equipment.
Maintain a panel of patients, as duties will allow.
Participate in overall program planning and coordinate with other clinical areas such as MAT, Mental Health, Nutrition, Specialties, and Dental to ensure the integration of all clinical services within the FQHC, as well as services offered at locations with co-located services such as Health Homes and OTP.
Establish and coordinate services with other medical facilities, particularly local area hospitals, to ensure continuity of care for the Center's patients.
Establish and maintain medical protocols for all areas of medical practice and modify them as necessary, consistent with evidence-based standards set by various accrediting and governmental review organizations.
Coordinate with the QI/Pop Health Leader and participate in a Quality Improvement Committee and QI activities.
In collaboration with the Site Managers, monitor patient utilization and medical staff productivity to ensure timely patient access to medical services and cost-efficient operations.
Lead meetings with the staff and providers and contribute to the planning and execution of bi-monthly and annual All-Staff meetings.
Coordinate in-service training programs for clinical staff.
Attend/present at Board of Directors meetings, annual Board Conferences, staff meetings, in-service meetings, community meetings, and conferences as assigned.
Participate in the preparation of the clinical service budget and provide justification for clinical expenditures.
Prepare for regulatory agency site visits.
Develop and implement monitoring activities to ensure the appropriate maintenance of clinical records.
Coordinate with Operations and Risk Management to review all site accident/incidents related to patient/client care, provide input into the actions necessary to resolve identified problems, and implement corrective actions as necessary and appropriate.
Ensure physician participation in the Interdisciplinary Care Planning process.
Ensure compliance of all medical staff with the rules, regulations, and bylaws of the Medical Department, as well as all applicable facility policies and procedures. Initiate action in cases of non-compliance.
Provide after-hours call coverage on a rotational basis.
Undertake special projects and other duties as assigned.
REQUIREMENTS
A Minimum of three years experience as a Medical Director or in a similar leadership role in a healthcare setting, preferably FQHC
Medical Degree (MD) or Doctor of Osteopathic Medicine (DO) from an accredited school.
Successful completion of a residency program.
Current and unrestricted NYS License and registration as a physician.
Admitting privilege to at least one geographically accessible acute care hospital.
Clinical experience (minimum of 5 years preferred) practicing prima1y care, preferably in a community health center serving culturally diverse and under-served patient populations.
Board Ce1tified/Board Eligible.
DEA required
Experience in the development and management of quality, risk and utilization review processes.

POSITION OVERVIEW
Under supervision of the Director of Human Resources, the Human Resources Coordinator will support the functional and administrative duties of the Human Resources Department. The Human Resources Coordinator must be able to work effectively in a team environment as well as work independently, establish workload priorities, manage multiple tasks, take initiative and work under pressure to perform and complete a high volume of work with speed and accuracy to meet established deadlines. This position has no managerial or supervisory responsibilities.
KEY ESSENTIAL FUNCTIONS:
Assists in the on-boarding process for all new hires.
Enrolls in electronic onboarding system (People Doc) to selected candidates for completion.
Reviews and ensures to completion new hire paperwork (in People Doc) and provide list of required documents to candidates.
Coordinate's completion of all steps the pre-employment process to include background checks, reference checks, employment verifications and collection of required documents (diplomas, degrees, IDs, certifications, licensures, etc.)
Completes New Hire Checklist in its entirety for all new hires to ensure compliance and legally sound hiring practices.
Ensures new hires receives health insurance packet and enrollment forms.
Prepares final employee file for all new hires ensuring proper filing of all documents and HR forms consistent with departmental standards.
Responsible for effective and efficient management of pre-employment background check processes under OCFS/DOE
Ensure accurate completion of 6000/A Series Packet
Ensure A Series/6000 packets are submitted to the Central Clearance Unit (CCU) the day the candidate starts
Ensure packets and submission confirmation are sent to the program the day the candidate is cleared to start
Provides Pre-Employment Process Training to all newly hired Human Resources Coordinators.
Serves as Justice Center liaison in Human Resources Generalist absence. Follows all applicable steps in process.
Complete coping, scanning and archiving projects for the HR Department as needed.
Assist in monthly audits as needed.
Maintain licensure and certification renewal database, ensuring timely renewals.
Ensures terminated employee files are removed from active status and filed in terminated area in a timely manner.
Assists with tracking and compliance of various Human Resources deliverables (Annual Mandatory In services, Talent Reviews, Open Enrollment, etc.)

REQUIREMENTS:
2+ years previous relevant experience in Human Resources Healthcare Industry experience preferred.

POSITION OVERVIEW
The Program Administrator of Substance Use Disorder Services is responsible for the administrative oversight of the OASAS certified 822 Comprehensive Integrated Outpatient Treatment Program. The program serves adults who have a primary substance use disorder diagnosis and those diagnosed with co-occurring substance use and mental health disorders. In collaboration with the Senior Administrator of Outpatient and MAT programs, the Program Administrator will ensure that her/his programs are accessible, effective, and compliant with all applicable regulations, guidelines, policies, and procedures and financially sustainable. As part of the management team, the Program Administrator will actively participate in the integration of behavioral healthcare services including the development and sustainability of a comprehensive, patient centered, trauma informed recovery model.
KEY ESSENTIAL FUNCTIONS
Direct the implementation of systematic workflows to include integration of medical, substance use, and mental health services in the 822-outpatient clinic.
Plan and develop the integration model of medical, clinical, and mental health services.
Directs the implementation of policies and procedures noting deficiencies and needed changes and amendments when necessary.
Participate in agency management and planning activities including community outreach
Provide leadership, guidance, direction, and supervision to program managers and administrative support staff.
Recruit, hire and provide training to program leaders to ensure department goals are communicated effectively, job performance is continuously evaluated
Provide guidance with the ongoing evaluation, counseling, and disciplining of program personnel.
Manages outpatient clinic operations to budgeted and planned results
Prepares reports for all funding and monitoring sources
Leads the preparation, guidance, and direction for all program audits both internal and external

REQUIREMENTS
Master's Degree in Social Work preferred, Human Services Administration, Public Health related field or 10 years of progressive management responsibility in health care related field with experience that demonstrates knowledge of operations in a health services organization industry.
NYS Licensed Clinical Social Worker (LCSW) /LMSW/LMHC preferred.
Seminar In Field Instruction (SIFI) preferred
Must have CASAC credential or obtain within 12-months of hire.
Must have minimum of 3-years clinical work experience in the chemical dependence field.
A minimum of six-years of experience in a supervisory, management and/or leadership role.
Completion of OASAS Clinical Supervision Training, I & II (or obtain within 6 months of hire)
Trained and certified in naloxone delivery to treat opioid overdose, or trained within 7 days, post hire.

Housing Specialist assess housing barriers of individuals experience homelessness to determine housing and service needs. Develop a housing procurement, financial and self-sufficiency case plan with clients: this shall include intake interview, determine client goals, needs, and eligibility.
KEY ESSENTIAL FUNCTIONS
Assesses housing needs of client upon referral from other providers.
Develops and maintains housing resources and assists clients in finding permanent, affordable housing.
Facilitates the prioritization of project-eligible individuals for participation in the project.
Procures and places participants in affordable housing and determine participant income resources that could offset subsidy amount.
Works in partnership with HUB's housing offices and under the direction of the Housing Services Director, manage the timely payment of subsidy funds for apartment rental, utility payment one-time payments for furnishings.
Maintains documentation of client progress in case records according to all contractual & regulatory standards.
Coordinates services and communicate to team about clients' housing progress along with participating in case reviews and case conferences.

POSITION OVERVIEW
The maintenance worker is responsible for the general maintenance of the facilities which include but are not limited to repairs on heating and air conditioning equipment, hardware repairs, repairing light fixtures, plumbing repairs, moving equipment/furniture, painting and plastering walls, driving company vehicles and general carpentry needs of the facilities. A basic familiarity with the facilities mechanical plant is required.
KEY ESSENTIAL FUNCTIONS
Maintain the physical plant and facility in a safe and secure manner.
Assure that the building has adequate heat or air conditioning, light, water and sewage disposal at all times; be on duty during interruptions of any of these to coordinate repairs and emergency contingency plans.
Inspect, diagnose and repair physical plant problems as assigned.

Ensure that all garbage and trash, including hazardous materials, are disposed of properly.
Perform in a wide range of trades including but not limited to maintenance, test, repair, installation and replacement of parts and equipment involving general skills in carpentry, plumbing, masonry, electricity, HVAC, refrigeration, sanitation, grounds, painting, wall covering, cleaning transportation and the preventative maintenance program.
Maintain a clean, neat appearance wearing prescribed uniform at all times.
Cleans all dust, dirt and debris accumulated as a result of maintenance performed, equipment malfunction and resulting repair, construction or renovation.

POSITION OVERVIEW
Under the supervision of the Senior Compliance administrator, the Quality Improvement Coordinator is responsible for performing comprehensive analyses of clinical documentation, review of key administrative and compliance requirements, as well as informs the leadership team of audit findings, potential deficiencies and assists in the development of corrective action plans to evaluate procedures and services performed as required by OASAS , OMH and any other regulatory agency for behavioral health programs. In addition, this role is responsible for ensuring quality of documentation, performance trends with a high degree of accuracy and quality, maintaining current knowledge of healthcare regulatory guidelines that impact reimbursement and quality outcomes and communicates with up-line management of identified trends and areas of risk to provide recommendations for improvement in the patient experience and clinical outcomes.
KEY ESSENTIAL FUNCTIONS
Concurrent reviews of OASAS and OMH medical records to validate completeness, compliance and consistency with clinical documentation in accordance with federal and state regulations.
Champion Evidence Based Practices by leading behavioral health electronic record review, PDSA's and performance improvement projects/
Provides constructive recommendations to management as determined by audit findings.
Ensure audits are complete, accurate and documented timely.
Facilitate monthly continue Quality Improvement (CQI) meetings with programs. Monitor and analyze behavioral health documentation (monthly program specific chart audits). Document quality assurance audit reports/summaries.

POSITION OVERVIEW
The Utilization Manager is responsible for the day-to-day functions of collaborative communication with external case managers at referring provider facilities and/or managed care organizations (MCO) for data collection, interpretation, and certification/recertification from third party payers at pre-admission through discharge, ongoing medical necessity reviews and interdisciplinary team support with respect to MCO requisites. In addition, this position is responsible for having a thorough understanding of patient treatment plans through participative discussions with the care plan (interdisciplinary) team, identifying and referring requests for services to the Medical Director when guidelines are not met and reviewing residential services requiring MCO approval.
KEY ESSENTIAL FUNCTIONS
Obtaining the required pre-authorization/certification of specified services from the MCO and communicating end of benefit status and/or reimbursement changes to Finance and others as deemed necessary.
Conducting ongoing medical necessity reviews, which may be pre-service, concurrent and/or retrospective in nature, by evaluating clinical data and submitting appropriate documentation to update the patient/resident's status to the MCO as scheduled.
Acting as a liaison between the residential program and the MCO to better facilitate the reimbursement process.
Referring MCO denials and non-covered medical services to the directors for appeal determination and next steps
Conduct medical record reviews of all managed care patients within the first 24 to 72 hours (if admitted on a Friday after 5:00pm) then weekly as required to promote desired clinical and financial outcomes by, but not limited to:
Meeting with all managed care residents/patients to better assist in the selection of appropriate provider resources and identification of quality and cost-effective services throughout the continuum of care.
Acting as a resource to the interdisciplinary team regarding MCO requirements for admission, continued stay and discharge planning.
Providing early identification of covered care and ensuring that claims are approved when the requirements are met.
Collaborate with clinicians to identify discharge needs and coordinates with Social Services in the creation and implementation of a discharge/transfer plan.
Coordinates post-discharge follow-up care with selected patients and community providers.
Maintains strong leadership skills to perform the multiple functions and a wide variety of tasks requiring independent judgment, ingenuity, and initiative.
Interacts with executives, directors, key members of the residential team, physicians, and their staff, third party payers, state and federal agencies, auditors, and vendors.
Maintains a high degree of computer proficiency with MS Word and MS Excel which is critical for the analysis conducted using this technology.
Maintains all information in a manner such as to assure strict confidentiality and compliance with HIPPAA.
Maintains full knowledge and understanding of state and federal regulations as they pertain to resident assessment requirements.
