We are hiring a Targeted Case Manager for our growing team! The Targeted Case Manager will be responsible for assisting individuals eligible under the state plan in gaining access to needed recovery supports, medical, social, educational, and other services. Duties include assessing the individualized needs of an individual, coordinating provision of services, and documenting services and outcomes. Key Responsibilities:
Maintain confidentiality and comply with company, state, federal and HIPAA rules and regulations
Charting for insurance purposes
Maintain a positive, professional attitude toward residents, staff and volunteers
Create a safe and healthy environment for clients
Have a sincere commitment to client's recovery
Transport clients to appointments when necessary
Monitor client's emotional well-being and behavior
Connect client with family and community resources
Connect client with legal entities when necessary
Motivate clients toward goal achievement
Meet with clients weekly
Help residents with long-term care plan development and educational resources
Key Experience and Education Needed:
Bachelor's degree in social work or human services field required
1 year post grad work experience or Master's Degree required
Must have a current driver's license
Basic computer skills
Ability to complete and submit reports
Be self-motivated
Have excellent documentation skills
Possess excellent written and verbal communication skills
Have a strong working knowledge of computer skills and use of search engines
Be able to handle crisis situations in a calm and supportive manner
Maintain a positive, professional attitude toward clients and staff

About the Role/Unit:
The RN Team Leader in the Operating Room at Sinai Hospital serves as a clinical and operational leader, responsible for managing the day-to-day surgical workflow within the orthopedic service line. Coordinates and supports daily surgical operations within the orthopedic OR service line by identifying opportunities to streamline case flow, reduce delays, and improve first case on-time starts.
Participates in audits, quality initiatives, and implementation of evidence-based practices specific to orthopedic surgery.
Communicates equipment, supply, or process concerns to leadership and participates in improvement planning.
MINIMUM REQUIREMENTS:
At least 2 years of RN Operating Room experience
Associate's degree in Nursing required; BSN preferred
Maryland Board of Nursing (MBON) Registered Nursing license
American Heart Association BLS Certification
ACLS and PALS preferred

SUMMARY: The Director of Nursing Education oversees the planning, designing, administration, and implementation of clinical education and academic partnerships for Northwest Hospital. Collaborates with Patient Care Services leadership to accomplish organizational objectives. RESPONSIBILITIES:

Position Summary:
A Clinical Coordinator assists with the daily coordination of personnel and resources within the scope of assignment. Acts as the liaison between the nursing departments and all of the ancillary departments to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency and compliance.

HomeCare MD is seeking a compassionate and skilled PRN Speech Therapist to join our growing home care team. This role is ideal for an experienced Speech Language Pathologist who is passionate about providing exceptional patient-centered care in the comfort of a patient's home.
Position Summary
The Speech Therapist is responsible for conducting initial and ongoing patient assessments, delivering skilled Speech Language Pathology services, and acting as case manager for speech-therapy-only patients. This role will collaborate closely with physicians, caregivers, and interdisciplinary team members to support optimal patient outcomes.
Essential Functions
Develop individualized plans of care tailored to each patient's needs.
Recommend and train patients on augmentative and alternative communication methods (e.g., automated devices, sign language).
Teach patients techniques to improve speech production, voice quality, and oral/written language skills.
Educate and train patients in safe swallowing strategies or strengthening exercises to prevent aspiration.
Maintain accurate and timely documentation, including evaluations, progress notes, and reports for insurance justification.
Conduct research-based assessments and contribute to treatment methodologies.
Participate in interdisciplinary assessments and collaborate in creating and evaluating patient care plans.
Attend case conferences and required agency meetings.
Deliver services as prescribed by the patient's physician and in accordance with the plan of care.
Provide patient, caregiver, and family counseling and education.
Communicate effectively with physicians, healthcare practitioners, and the QAPI department.
Participate in agency-sponsored in-service training and work collaboratively with clinical leadership.
Education & Experience Requirements
Current Maryland Speech Language Pathology License.
Master's degree from an accredited college or university.
Current American Heart Association BLS for Healthcare Providers.
2-3 years acute care experience preferred.
2-3 years community health experience preferred.
Valid driver's license, automobile insurance, and ability to meet fleet safety requirements (travel required).
Strong knowledge of medical terminology.
Excellent interpersonal, communication, and people management skills.
Basic computer proficiency; Microsoft Office Suite preferred.
Experience with electronic medical records; Kinnser software experience preferred.
Basic knowledge of Medicare/Medicaid/OASIS/Commercial Insurance requirements.
Leadership Skills
Ability to motivate team members and adapt education to individual learning needs.
Establishes positive, respectful relationships with colleagues.
Capable of independently managing conflict and challenging situations.

The Social Worker - Inpatient Psych acts as a coordinator of psychosocial assessment and discharge planning for the patients/clients receiving inpatient psychiatric services at the Hospital, providing leadership and psychosocial support necessary to address the individual's needs. Acting as a resource to patient/client, utilizes knowledge of community and health care resources to assist in addressing identified needs at any point of the continuum, pre-admission to post-discharge. The primary focus of this role is to support individuals diagnosed with a Serious Mental Illness (SMI) in improving their daily living skills and overall functioning. Also, will facilitate group and individual teaching sessions, provide direct support to clients, and contribute to a safe and therapeutic environment. This position requires in-person, on-site presence..
Key Responsibilities:
Assist licensed behavioral health professionals in the implementation of individualized treatment plans for clients with SMI.
Conduct group and individual teaching sessions focused on:
Routine daily living skills (e.g., hygiene, meal preparation, budgeting)
Self-monitoring of symptoms and medication side effects
Emotional regulation skills
Interpersonal skills and social interaction
Provide direct support and supervision to clients, ensuring their safety and well-being.
Observe and document client behavior, progress, and participation in activities.
Assist in creating and maintaining a therapeutic and supportive environment.
Participate in team meetings and case conferences.
Follow all clinic policies and procedures, including confidentiality and reporting requirements.
Other duties as assigned by the supervising licensed behavioral health professional.
Key Experience and Education Needed:
High school diploma or equivalent required.
Experience working with individuals with mental illness preferred but not required.
Ability to communicate effectively and work collaboratively with clients and team members.
