Patient Engagement Specialist II - Promotora
Family Health Centers of San Diego (FHCSD) is looking for a Patient Engagement Specialist II - The Promotora provides patient engagement services that promote FHCSD's Healthy Together program, which supports families who have children experiencing obesity and life stressors. In addition, provide social, emotional, and practical support families enrolled in the program.

Responsibilities:
Provide health education and coaching to families with children experiencing obesity and a high-level of exposure to adverse childhood experiences.Help families problem solve on needs that arise throughout the program or develop practice skills elsewhere Support the provision of 30-60-minute phone-based support calls to families weekly Support up to 2 home visits to families participating in the program.Establish and strengthen community partnerships and referrals for housing, childcare, food banks, employment, education counseling, legal services, and other related services.Develops, identifies, and provides referrals, within and outside of FHCSD, and effectively monitors and tracks such referrals.Is an effective, productive, and collegial team member.Written work (such as plans, flyers, reports, etc.) is accurate and timely. Effectively uses computer resources available as required to meet organization's demands.Grant funded program; continuation of employment is dependent upon availability of and/or renewed funding.Perform other duties as assigned.
