Mountain Valley is currently seeking a Hospice Bereavement Coordinator to work in our Hillsville service area 20 hours per week . The Bereavement Coordinator is responsible for the provision of bereavement services to patient’s families, support and grief support to individuals, families, and groups through community referral..
Duties
- Assess the bereavement needs of Hospice patients and families, developing an individualized plan of care as needed.
- Refer families to other community resources as appropriate.
- Facilitate bereavement support groups.
- Serve as a bereavement representative at Hospice IDG/care planning meetings.
- Present training sessions on grief and loss to new Hospice volunteers as part of the overall volunteer training program.
- Assist in conducting educational workshops for the community.
Qualifications
- Completion of a Bachelor's degree in psychology, sociology, social work, or counseling, from an accredited school.
- Minimum of one year experience in grief support.
- Valid Driver's License and current automobile insurance.
- The willingness and ability to travel locally approximately 70% of the time.

Overview
We're looking for a Bereavement Coordinator to join our team.
You will report directly to the Executive Director or Administrator. You will be responsible for the identifying the bereavement needs and providing the highest quality service to meet those needs.
* You will be providing bereavement needs to families, staff and volunteers.
* You will be gathering information to develop the family bereavement service needs You will maximize family access to resources including community groups, and referrals.

About You
* High School graduate degree or equivalent required.
* Undergraduate degree in human service field preferred.
* Graduate degree preferred if providing counseling.
* Evidence of training in the areas of life-threatening and chronic illness, grief and loss.
* Clinical experience in the areas of life-threatening and chronic illness, as well as, grief and loss; One year required; three years preferred related clinical experience in varied multi-disciplinary settings.
* Counseling experience: individual, family and group therapy experience preferred.
* One year experience in a healthcare setting required; preferably hospice, home health, community care or hospital
* Knowledge of terminally ill patients and their families along with understanding of hospice and the psychosocial dynamics of illness, loss, and death
