Primary Purpose of Job:
The Case Manager for the Nassau Adult Guardianship program is responsible for all aspects, including personal needs and/or property management of home-based intervention services and case management for
incapacitated adults
in the Community Guardianship Program.
Principal Duties & Responsibilities:
Within 72 business hours of assignment, contact the relevant parties to begin the fact-finding process and begin developing a service plan, consisting of social, administrative, fiscal, and legal services for Guardianship clients.
Provide case management support including home visits, telephone reassurance, advocacy and encouragement for clients struggling with problems such as substance abuse, mental illness, housing, and finances.
Transport and escort clients in own vehicle if necessary to and from appointments when needed to obtain emergency resources or to complete applications.
Participate in case conferencing for case review and decision-making.
Maintain up-to-date progress notes on all assigned cases.
Communicate with judges, referees, lawyers, and DSS caseworkers regarding program participants as well as presenting to community referral sources.
Complete and submit Initial Report, Interim Reports, Annual Reports and court updates for clients.
Provide progress notes to finance department for billing.
Develop measurement strategies to evaluate the effectiveness of the programs.
Knowledge, Skills, & Abilities Required:
Bachelors Degree in social or behavioral sciences preferred.
Must be proficient in office and computer applications or other automated systems such as spreadsheets, Microsoft Office applications, Calendar program, and e-mail and/or database software skills necessary to perform job responsibilities.
Minimum of two years of relevant work experience preferred.
Excellent interpersonal and communication skills, both oral and written.
Ability to motivate clients.
Must have access to automobile and possess valid NY State Drivers license.

Primary Purpose of Job:
The Part Time Therapeutic Visit Specialist is responsible for providing therapeutic supervised visitation for families referred by the court. The Budgets & Contracts Manager is a critical role responsible for coordinating the financial management of over 100 programs funded by 60 different funders. This position will ensure the effective preparation, management, and compliance of budgets and contracts to support the agency's mission in delivering social services. The Budgets & Contracts Manager will support the Finance team and program leadership to maintain financial integrity and transparency.
Measurable Objectives:
Analyze final contracts to ensure complete execution and optimal spending. Communicate any relevant details to all parties (executive staff, program leaders).
Monitor expenses against budgets in real-time, generating monthly variance reports and provide {monthly reports} on budget status to program directors. Communicate any variances exceeding 10% of the budgeted amount to program directors. In conjunction with program directors, develop plans to address variances.
Process and submit claims requests within 10 business days of the end of each reporting period.
Responsibilities:
Budget Preparation and Management
Coordinate with program and finance staff to collect data, confirm accuracy, and prepare budgets for submission. Ensure that all finalized budgets are entered into internal and external funder systems and kept current for use in claims and projections.
Collect and verify key cost data (e.g., salaries, fringe, insurance, rent, OTPS) from program and finance staff to develop accurate budgets and projections that support planning, claims, and internal decision-making.
Ensure that all budgets align with organizational/program goals and funder requirements.
Regularly monitor and analyze program expenses against budgeted amounts to ensure alignment with financial targets.
Prepare and distribute monthly financial reports, providing transparency on budget utilization, remaining balances, and key budget status indicators.
Communicate significant variances of budgeted amounts to program directors and COO to collaborate to develop corrective action plans.

The DM is a member of the Development Team and is a liaison between the agency and its donors. The DM is responsible for cultivating the agency's current list of active donors and creating new opportunities with individuals, corporations, foundations, and community stakeholders. The ideal candidate will bring expertise and experience in fundraising, donor solicitation, event planning, grant & foundation writing, relationship building, project management, and implementation of data-driven fundraising strategies.
Principal Duties & Responsibilities
The Development Manager plays a key role in advancing EAC Network's mission by supporting strategic fundraising initiatives, strengthening donor engagement, and ensuring efficient development operations. This position requires strong relationship-building skills, excellent communication, and proficiency with modern nonprofit fundraising systems and technologies.
1. Fundraising Strategy & Execution
Support the planning, implementation, and evaluation of annual fundraising goals, campaigns, events, and appeals.
Coordinate and execute multi-channel fundraising campaigns, including direct mail, digital fundraising, Giving Tuesday, and donor stewardship initiatives.
Research and cultivate new individual, corporate, and foundation prospects to expand the organization's donor base.
In partnership with senior leadership, identify opportunities to diversify revenue streams and support strategic growth.
2. Donor Cultivation, Stewardship & Relationship Management
Manage a portfolio of individual and corporate donors, ensuring timely cultivation, solicitation, and stewardship strategies.
Prepare donor communications, acknowledgment letters, impact updates, and personalized engagement materials.
Support the development of donor pipelines, including major donors, lapsed donors, and new donor prospects.
Collaborate with program staff to gather stories, outcomes, and impact metrics to strengthen donor engagement.
3. Development Operations & Fundraising Technology
Maintain accurate, timely, and complete donor records using DonorPerfect or similar nonprofit CRM systems.
Utilize Constant Contact, email platforms, and social media tools to execute donor communications and fundraising outreach.
Use Excel, data analytics tools, and reporting platforms to track KPIs, analyze donor trends, and prepare development dashboards.
Employ Instrumentl or other grant research platforms to identify funding opportunities, track deadlines, and support grant management.
Ensure proper gift processing, tracking, and acknowledgment following industry standards and internal procedures.
Manage fundraising infrastructure and back-office systems to support high-quality development operations, including CRM hygiene, data integrity, and reporting.
4. Events Management & Community Engagement
Assist in planning and executing EAC Network's annual fundraising events, including sponsorship outreach, guest management, vendor coordination, and donor follow-up.
Engage community partners, corporate volunteers, and civic groups to increase organizational visibility and support.
Represent EAC Network at community events, meetings, and public engagements to strengthen relationships with stakeholders.
5. Grant Support & Research
Research, write and apply for foundation and grant funding.
Support the grant development process by tracking deadlines, collecting program data, and coordinating required documentation.
Conduct prospect research on foundations, corporations, and government opportunities using fundraising research tools and platforms.
Maintain grant calendars, submission logs, and reporting schedules using technology platforms such as Instrumentl, Excel, or CRM integrations.
6. Communications & Marketing Support
Collaborate with the communications team to develop consistent messaging for appeals, newsletters, social media, and donor impact stories.
Support the production and distribution of the Annual Report, program updates, and donor-facing collateral.
Leverage digital channels to strengthen donor engagement and amplify EAC Network's mission and services.
7. Administrative & Cross-Departmental Collaboration
Work closely with Finance to ensure accurate gift reconciliation, reporting, and audit compliance.
Assist senior leadership with Board development, preparing materials, presentations, and donor reports.
Maintain high standards of confidentiality, professionalism, and mission-driven integrity in all development activities.
Perform other duties as assigned to support the mission and strategic goals of EAC Network.
Requirements for this Role:
3 to 5 years of development experience in grant writing, event planning, and fundraising. Ability to organize major fundraising events and apply for major grants and sponsorships.
Must be able to attend agency events- this person acts as a salesperson for EAC Network. Some evening/weekend hours required for events and meetings.
Demonstrated analytical and persuasive writing skills, in addition to excellent organizational, multitasking, and communication skills.
Ability to communicate clearly and engage/influence others via grant proposals, phone, email, and in person. Eagerness to make cold calls, research, and meet with prospective supporters to tell the EAC story and secure community partners, donors, and grant opportunities.
Strong administrative skills and self-motivated, with the ability to set priorities and manage multiple tasks under minimal supervision in an effective and efficient manner. Able to work independently and as a team. Must maintain strict confidentiality of client, agency, and donor information.
Proficiency in computer applications and online systems such as Microsoft Office, DonorPerfect, Instrumentl, Constant Contact, Authorize.net, spreadsheets, calendar, email, and database software necessary to perform work assignments.
Possession of valid New York State driver's license and access to an automobile for local travel and some work outside of business hours.

The Forensic Interviewer conducts trauma-informed, developmentally appropriate, and culturally responsive forensic interviews of children and adolescents referred by the Department of Social Services (DSS), law enforcement, and the District Attorney's Office. This position reports directly to the Nassau CAC Director and works in full collaboration with the Multidisciplinary Team (MDT).
What You'll Be Doing
Conduct forensic interviews in accordance with Nassau County MDT guidelines, National Children's Alliance (NCA) accreditation standards, New York State Office of Children and Family Services (OCFS) protocols, and recognized best practices.
Coordinate interview scheduling with DSS, Special Victims Unit, and the District Attorney's Office to ensure timely interviews.
Actively participate in MDT pre-briefings and debriefings to support coordinated case planning.
Participate in peer review sessions to ensure consistency, quality assurance, and continuous improvement of forensic interviewing practices.
Accurately document interviews and case information in the assigned database; maintain records in compliance with funder, agency, and MDT requirements.
Provide testimony in criminal, family, and civil court proceedings when subpoenaed; defend the integrity of forensic interviews conducted.
Provide information and support to child victims and non-offending caregivers regarding the interview process and criminal justice system procedures.
Assist the CAC Director with program data collection, statistical reporting, and scheduling functions as assigned.
Attend mandatory and supplemental professional development trainings to maintain current knowledge of research, techniques, and standards in forensic interviewing and child abuse investigations.

Knowledge, Skills And Abilities Required:
Bachelor's degree in Criminal Justice, Social Work, Psychology, or a related field required; Master's degree preferred.

Successful completion of, or ability to complete, NCA, OCFS, or Nassau County MDT-approved forensic interviewer training program upon hire.

The EAC Nassau County Child Advocacy Center located in Bethpage, NY provides for children a safe and respectful response to situations involving allegations of sex crimes. When assigned, the Victim Advocate will offer support and advocacy to children and non offending families impacted by abuse or trauma. This involves coordinating services and assisting the Child Advocacy Center Director in ensuring that appropriate support is provided to victims and their non offending family members.
What You'll Be Doing
Act as a liaison between victims and social services agencies.
Assist victims in preparing and filing paperwork necessary for OVS Victims Compensation.
Maintain case records and statistics as required by funders.
Conduct crisis intervention / needs assessments with clients and provide referrals as needed.
Coordinate with other victims' services agencies to avoid duplication of services.
Provide back-up and support to CAC Division Director for office coverage.
Attend multidisciplinary team case review meetings to support informed decision making.
Support the Community Educator by networking, attending outreach events and identifying new resources for CAC clients.
Assist in administrative responsibilities of the Child Advocacy Center.
After reviewing for accuracy, complete Medical Staff Vouchers and submit PO for services provided.
Assist in coordinating forensic interviews and foreskin examinations when necessary
Transport and escort clients to / from appointments if needed in own vehicle when necessary including the ability to lift, carry, and secure children into appropriate child restraint car seat.
Perform all other relevant duties as determined by Supervision and EAC NETWORK Administration.
Knowledge, Skills And Abilities Required:
Associate's Degree required, Bachelors Degree, preferred.
Minimum of two years of relevant experience in advocacy, victim services or case support.
Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
Excellent communication, organizational and interpersonal skills.
Must have access to a vehicle and possess valid NYS driver's license.

What drives the core purpose of this job:
The Program Director helps to ensure the quality of EAC Network's Children's Health Home program by sharing a solid understanding of Health Home policies and best practices, reviewing and providing feedback on EAC Network's Health Home work, and supporting the Care Coordinators in providing excellent care to the clients.
What are the key Responsibilities:
Supervise and provide support to the NYC Care Coordination staff to ensure quality work.
Provide care coordination services to a caseload, including assessments, development of care plans, and the coordination of medical, behavioral and community services, independently with minimal supervision, ensuring at least the 1-2 required in person contacts are completed monthly.
Oversee all Care Coordinators with their caseload, including assessments, care plans, and progress notes. Cover caseloads and perform 1-2 required in person contacts as needed.
Lead and participate in meetings with Health Home team including Care Coordinators, clinicians, treatment providers and/or the Lead Health Home. Attend external meetings on behalf of EAC Network's Health Home program.
Share best practices among Health Home staff in various offices. Work with staff and Senior Director to identify and resolve issues that may hinder the quality or growth of the program.
Randomly and regularly review the progress notes and other documentation from Long Island Care Coordinators to ensure consistently high standards. Assist in quarterly chart audits.
Remain informed about the evolving NY DOH Health Home policies and procedures, and share information about changes, trainings, etc. to staff.
Track Health Home activities across the program. Collect and distribute monthly activity reports.
Complete CANS training and pass CANS certification with an 80%. Monitor employee's CANS certifications.
Customarily and regularly exercises discretionary powers.
Transport and escort or arrange transportation for clients to/from appointments when needed.
Complete all annual required trainings as governed by the health home.
Supervises: NYC Children's Health Home Care Coordinators
Perform all other relevant duties as determined by Supervision and EAC NETWORK Administration.
What key knowledge, skills, and abilities are required to excel in this role:
NYS-licensed RN and/or Master's Degree in a human services discipline preferred.
Experience with New York's Health Home Care Management program preferred.
3+ years of experience providing case management to children preferred.
2+ years of managerial experience preferred.
Excellent organizational and time management skills, interpersonal skills, verbal and written communication skills required.
Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
Must be able to travel either with automobile or public transportation throughout NYC.

Temporary Position: This role will be active through July 2026.
Primary Purpose of Job:
The Veteran Peer Mentor is responsible for providing on-going culturally responsive and appropriate support for clients referred to the program by fostering positive relationships with BMVTC program participants and facilitating groups. As well as assist the Coordinator in creating a volunteer peer mentor program for the Court.
Principal Duties & Responsibilities:
Participate in planning for services for clients, through face-to-face meetings and facilitation of group discussions.
Assist program participants in completing required assignments.
Meet with clients on an on-going, as needed basis on and off-site and be available via email and cell phone for support, advice, and guidance
Participate in team meetings.
Participate in regularly scheduled on-going training's and supervision
Assist in connecting clients to community-based support resources, clubhouses, and other ancillary services.
Maintain client charts and notes, update after each interaction with the client and have available for inspection by supervision at any time.
Assist the Coordinator in engaging and training peer volunteers to provide support for the clients.
Knowledge, Skills, & Abilities Required:
Minimum High School Diploma or educational equivalent.
Must have good communication skills, both oral and written.
Ability to work cooperatively with multiple agencies.
Knowledge of target population and area resources.
Experience with group facilitation preferable.
Experience using cognitive behavioral therapies, motivational interviewing and/or positive youth development preferable.
Must be proficient in computer applications and email and/or software necessary to perform work assignments.
