GBC is committed to providing quality person-centered, trauma-informed care to individuals experiencing serious mental illness, developmental disabilities, and co-occurring disorders. GBC programs focus on the issues that matter most in the lives of the children, families, and adults we serve.
JOB PURPOSE:
Responsible for the registration process, delivering information regarding program eligibility, assistance in completing all necessary paperwork with customers, answering and directing phone calls and scheduling customer appointments with the appropriate provider.
ESSENTIAL FUNCTIONS and DUTIES:
1. Welcomes customers, visitors, and all staff.
2. Answers and directs incoming phone calls as needed.
3. Schedules customer appointments and follow-ups.
4. Exhibits thorough understanding of Gulf Bend Center operations and menu of services and all applicable intake procedures. Explains menu of services and related paperwork (consents and customer handbooks) as needed.
5. Ensures customer information is up-to-date and accurate in the Electronic Health Record.
6. Educates customers on insurance coverage and related information, balances due, and other financial information.
7. Identifies a crisis situation and facilitates an appropriate response.
8. Communicates quickly, proactively and effectively with registration team, business team and program staff to stay on top of registration process.
9. Maintains awareness of waitlist, capacity and requirements.
10. Collect payments when applicable and maintains the cash draw and daily deposit log.
11. Provides empathetic guidance, problem solving, support, and assistance to all customers and program staff.
12. Travels to Satellite Clinic and work late clinic following a scheduled rotation.
13. Prepares Routing slips for appointments.
Trauma-informed:
A trauma-informed approach to care ?realizes the widespread impact of trauma and understands potential paths for recovery; recognizes the signs and symptoms of trauma in clients, families, staff, and others involved in the system; and responds by fully integrating knowledge about trauma into policies, procedures, and practices, and seeks to actively resist re-traumatization. EDUCATION AND EXPERIENCE REQUIRED:

8. Customer service: Demonstrates appropriate responsiveness to internal/external customer needs.

10. Fleet: Completes fleet checklist as required. Reports all fleet problems directly to the Program Director within 24 hours of identifying problem. Uses GeoTab key fob in fleet vehicles and maintains road safety compliance.

1. Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
2. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business. Safe driving record as defined by Gulf Bend Center.
3. Must have computer experience to include Microsoft Word, Excel (beginner to intermediate level), Outlook, Teams, and Zoom. Ability to type 40 wpm. Ability to operate office equipment (phones, copy machine, fax).
4. Must complete all training requirements prior to the due date. Attend required training and team meetings. Relias training and any other training required for the role is 100% compliant.
5. Must maintain regular and reliable attendance, including compliance with the telework agreement. This is a full-time position. The Department of Radiology in the Joe R. and Teresa Lozano Long School of Medicine at UT Health San Antonio is seeking a Nuclear Medicine Radiologist to join our growing department. This is a full-time faculty position at the Assistant Professor to Professor rank. The faculty member will provide clinical services at the UT Health San Antonio Multispecialty and Research Hospital, UT Medicine, and affiliated facilities to include Nuclear Medicine night and weekend, and ER weekend coverage as needed. Additional responsibilities include teaching medical students, supervising residents and fellows. An established record of accomplishment in education and research are desired. Candidates should be authorized users and experienced with full range of Nuclear Medicine diagnostic studies and therapies.
Our hospital and clinics are regional referral centers for Oncology and has partnership with MD Anderson Cancer Center, for trauma, organ transplantation, other surgical and medicine subspecialties. The Nuclear Medicine section is also site for the combined ACGME-accredited Nuclear Medicine residency program in collaboration with San Antonio Uniformed Services Health Education Consortium (SAUSHEC). JOB PURPOSE:
Ensures that any individual seeking admission into a Medicaid-certified nursing facility or currently residing in such a facility is screened and evaluated for mental illness, intellectual disability, and developmental disabilities; identifies alternate placement options when necessary or requested; identifies specialized services that may benefit the individual; and meets routinely with individual at the nursing facility to monitor the specialized services.
ESSENTIAL FUNCTIONS and DUTIES:
* Complete a PASRR Evaluation (PE) within required timeframes to determine if the person has an MI, ID or DD diagnosis and, if so, to: assess the person's need for care in an NF; assess the person's need for specialized services; and identify alternate placement options.
* Complete initial and annual assessments that assist in determining specialized services that the individual is eligible to receive.
* Facilitate initial, quarterly, annual, and as needed Service Planning Team (SPT) meetings with person, Legally Authorized Representative (LAR) or actively involved person, Nursing Facility staff, and other collateral to aid in monitoring services and developing a person-centered plan of care.
* Provide habilitation coordination by identifying person-directed outcomes, and arranging needed services and supports, by advocating, and collaborating with the Service Planning Team (SPT) to develop a person-directed Habilitative Service Plan (HSP) which supports the customer in achieving a quality of life.
* Ensure the provision of habilitation coordination according to the Texas Administrative Code (TAC) and the PASRR Handbook.
* Provide crisis prevention and management by supporting the customer/LAR or actively involved person to secure services and supports that will enable them to prevent or manage a crisis.
* Meet face to face with a person monthly, or more frequently if needed, to provide habilitation coordination activities.
* Monitor all services on the HSP at least quarterly to ensure that the customer receives needed services (delivery), evaluate the effectiveness and adequacy of services (satisfaction), and ensure identified outcomes are meeting the customer's needs and desires (progress/lack of progress) as indicated by the customer/LAR or actively involved person determined by supervisory review of the HSP and progress notes.
* Provide education about community living options for the individual and families initially and every six months thereafter. Assist as needed to arrange community services outside the nursing facilities.
* Maintain accurate spreadsheets tracking monthly contacts and deadlines related to monitoring requirements, SPT Meetings, CLO, Habilitative Assessments, and other job-related criteria as directed by supervisor.
* Provide customers/LARs with an oral and written explanation of rights at least annually, when necessary, requested, or due to changes in legal status or revisions of HHSC documents.
* Complete thorough and accurate contact documentation in the Center's Electronic Health Record (EHR) within two business days at least 95% of the time.
* Ensure necessary forms/documents are filed in customer charts.
Trauma-informed:
A trauma-informed approach to care "realizes the widespread impact of trauma and understands potential paths for recovery; recognizes the signs and symptoms of trauma in clients, families, staff, and others involved in the system; and responds by fully integrating knowledge about trauma into policies, procedures, and practices, and seeks to actively resist re-traumatization." The six key principles of a trauma-informed approach include: safety; trustworthiness and transparency; peer support; collaboration and mutuality; empowerment, voice and choice; and cultural, historical and gender issues (Substance Abuse and Mental Health Services Administration [2014]).
