Care Coordinator

We are seeking a Care Coordinator to join our HSVS' team. Under clinical supervision, the Care Coordinator will be responsible for coordinating all aspects of healthcare services for the Foster Boarding Home (FBH) children. The Care Coordinator will serve as the primary liaison among healthcare providers, pharmacies, managed care plans, and internal care teams. The Care Coordinator will also support foster children and families through appointment coordination, follow-up care, and ongoing health monitoring. The ideal candidate will have demonstrated experience in conducting medical records reviews, manage referrals, and maintain accurate documentation within NextGen EHR, while building trusted relationships with caregivers and collaborating closely with HSVS' 29-I Clinic, Case Planning, and Children's Health Homes teams to promote continuity of care and positive health outcomes.
Primary Responsibilities and Essential Functions of Position:
Coordinate all aspects of healthcare services for designated caseload of FBH children. Duties include, but are not limited to:
Act as liaison/point person for healthcare providers, pharmacies, and MMCP case managers
Schedule and ensure attendance at medical/dental/behavioral health appointments, per ACSs schedule of required regular evaluations; coordinate transportation to appointments as needed
Monitor and address youths' healthcare needs in between routine well-child visits, including aiding parents in securing urgent care
Under clinical oversight by Care Coordination Supervisor/LPN and/or 29-I RN Supervisor, obtain and review medical exam records to determine whether further treatment is recommended, including referrals and medications
Ensure timely follow-up on referrals, emergency care and hospitalizations
Coordinate closely with HSVSs 29-I clinic, Case Planning, and Children's Health Homes teams, as applicable, in the provision of above services and to establish treatment goals.
Develop a trusting caregiver relationship with foster care children and families which will include clear and consistent communications regarding treatment planning.
Work with Managed Care Liaison team to ensure that healthcare appointments are scheduled with providers in-network with child's Medicaid Managed Care Plan (MMCP).
Ensure family medical history, proper consents (routine, mental health treatment, psychotropic medication, etc.), HIPAA authorizations, and all other required medical record documentation are entered in NextGen EHR.
Support in the completion of adolescent assessments, sexual/reproductive health, gender identity, sexual orientation, and family planning consultation.
Complete required actions in NextGen EHR, including referrals to HSVSs Article 31 behavioral health clinic, comprehensive. treatment plans, CHH warm hand-offs, data share reconciliations, etc.
Attend trainings as directed.
Engage in ongoing review of departmental policies, procedures and practices to make recommendations for quality improvement.
Perform other tasks as assigned within the reasonable scope of duties of the position.
Qualifications
Required bachelor's degree; Preferably in health, social services, or a related field.
One or more years case management experience in a healthcare environment and/or in the child welfare system.
Preferred.
Medical assistant or similar clinical background.
Preferred.
Knowledge of:
the child welfare system and the unique complex needs of the foster care population
foster care healthcare requirements.
Good understanding of medical procedures and medical terminology.
Experience documenting in Electronic Health Records (EHR):
NextGen EHR experience a plus
Excellent written, oral and interpersonal communication skills, and be a collaborative team player.
Proficiency in Microsoft Office, including Outlook and Excel.

Qualifications

HeartShare St. Vincent's Services (HSVS) Nuevo Camino program is dedicated to providing 38 unaccompanied minors, ages 5-17, with a supportive, nurturing environment through transitional foster homes, daily education Monday - Friday, and supportive wrap-around services in the Sunset Park section of New York City. Our program is rooted in ensuring that service delivery is trauma informed, culturally appropriate, and tailored to the needs of each child while maintaining all program requirements of ORR. A Case Aide supports Caseworkers by assisting with documentation, scheduling, client transportation, and communication. They help ensure child safety, facilitate family reunification, and maintain accurate service records.
Primary Responsibilities and Essential Functions of Position:
Provide case management support and coordinate community referrals.
Assist Caseworker with essential paperwork and documentation.
Schedule and prioritize work duties in partnership with the supervisor and other team members and in adherence to ORR, state licensing, and internal deadlines.

Oversee DHSP implementation of individuals' plans, consistent with their specific services, goals, and outcomes. Participate in the planning and implementation of activities, both at the program and in the community, that address these goals and outcomes.

Ensure DHSPs support and teach individuals according to their services, goals, and outcomes. Once meeting is held (every 6 months) and plan is approved, send copy of the staff action plan to care manager, residence, and family within the OPWDD required timeframe (59 days).
Upload all meeting documents, including sign-in sheet, meeting minutes, and proof of plan dissemination, into Precision Care.
Train all staff on plan and document training.
Document and ensure documentation of necessary information and data in areas such as medical, behavioral, individualized services, goals and outcomes, fire safety, community inclusion, communication books, etc., ensuring their accuracy and completeness.
Complete all monthly summaries by the 10th of every month.
Ensure all required documents in individuals' program books are up to date and uploaded into Precision Care.
Provide appropriate levels of supervision on an individualized basis for staff and individuals, ensuring staff are trained on the LOS for each person.
Actively engage in all program services as needed, including toileting, behavioral interventions, community inclusion, emergencies, physical plant cleanliness, etc., as needed and required.
Report all incidents to program management immediately upon discovery as per HeartShare policy, and OPWDD & Justice Center requirements. Immediately safeguard all individuals and assist with the completion of paperwork as required.
Assist program coordinator in securing safeguards/protective measures and notifications for incidents.
Ensure all occurrence reports are reviewed and completely filled out correctly and given to the program nurse or coordinator the same day as the occurrence.
Ensure that equipment and supplies are appropriate, in good condition, and safely stored.
Oversee cleaning of program area and ensure cleaning is completed as frequently and thoroughly as necessary to meet program cleanliness standards. Report any hazards or unsafe conditions immediately to supervisor.
Undertake any additional tasks and initiatives within the scope of the role as per the specific needs of the programs and individuals and as assigned by supervisor.
Meet or exceed the DSP Core Competencies Standards by the end of the 1st year of employment.

Residence Manager

Reporting to the Coordinator, the Residence Manager is responsible for the day-to-day operations one residential program serving people with developmental disabilities. Operational Management and Safety
Ensure the residence is in compliance with all applicable regulations and organizational policies and procedures.
Develop and submit monthly schedules to the Coordinator and DOP ensuring adequate coverage based on needs and publishing bi-weekly to staff
Complete monthly inventory, purchase necessary food and supplies with approved vendors. Ensure that the residence is stoked with items that meet the dietary needs are met. Reconcile orders with deliveries, sign and send invoices to central office
Manage the facility's petty cash, reconcile biweekly and ensure proper documentation and utilization of funds.
Ensure documentation and reporting of incidents in a timely manner. Submit reporting and notifications as needed, including but not limited to quarterly submit trends, SCIP forms etc
Develop observation schedule in accordance with agency standards, publish and upload schedule and observation forms to SharePoint. Follow up and review observations as they occur
Ensure the maintenance of the facility, complete monthly agency maintenance checklist, conduct daily inspection to determine cleanliness, hazards, etc. Ensure proper documentation and submit work orders as needed for repairs
Develop fire drill schedules according to regulatory standards, review drills as they occur and ensure appropriate paperwork is processed as needed
Implement systems to ensure that programs meet and exceed all regulatory standards and that program staff follow all state and federal regulations, as well as HeartShare's internal policies and procedures. Ensure that programs satisfactorily pass both internal HeartShare surveys and OPWDD surveys/audit and that no 45 or 60 day letters are issued
Staff Supervision and Development
Recruit, hire and train residential staff according to regulations and agency guidelines. Prioritize hiring by partnering with Human Resources to provide timely feedback and updates
Ensure staff receives appropriate certification/recertification as needed. Provide recommendations for training to further develop skills and keep track of attendance and completion
Complete staff evaluations and performance reviews according to the agency policies and procedures
Manage employees time and attendance, review and approve timecards, vacation and other requests in Paycom
Conduct daily review of PrecisionCare (Pcare), ensure timely completion and coordinate with Services Support Coordinator (SSC) team to monitor compliance and prevent overuse of DCE
Program and Service Development
Develop and publish community inclusion calendar monthly. Review inclusion logs weekly to ensure they are documented appropriately
Ensure medical and clinical meetings occur according to the agency expectations and standards. Determine how appointments need to be scheduled based on the medical meetings, ensure they occur and are scheduled as needed. Record and provide meeting notes and follow up on recommendations as assigned
Participate and follow up on recommendations from Day Program and Life Plan Meetings
Incident Management
Daily check for occurrence, if occurrences exist review for accuracy, and complete needed follow-up, i.e. notifications made, medical follow up completed etc.
Monthly ensure that items needed to close out occurrences are present, i.e, investigation summary, statements, etc.
As needed submit items to SRC to answer investigator and SRC recommendations, field and address any questions that arise.
Submit SCIP forms, trends and other documents as needed.
As needed attend SRC meetings to address specific concerns.
Qualifications
High School Diploma/GED required; Bachelors or Masters degree preferred
Minimum two years supervisory experience in a residential program
Five years' experience in developmental disabilities or related field
Work varied shifts
Must work at least one weekend day per workweek
Valid NYS Driver's License required

The Medical Administrative Support Specialist's responsibilities will include:
Identify, download and share NextGen documents as requested, such as for adoption packets, audits and investigations, etc.
Collaborate with Managed Care Compliance Coordinator to process periodic medical invoices for billing to MMCPs or payment by Heartshare Fiscal Dept., as applicable.
Assist in the tracking of authorization and reauthorization of Special/Exceptional foster care rates.
Request and obtain healthcare visit summaries and other medical records from community providers for foster care youths, including historical medical records as applicable
Maintain/manage health record; upload and file medical records and other required documents in NextGen charts.
Schedule medical/dental/behavioral health/specialist appointments to support Child Advocates; coordinate transportation to appointments as needed.
Through EHR & Excel, track and provide support to ensure compliance with OCFS/ACS mandates including consents, intake, routine medical, dental, developmental & behavioral assessments, family planning, and referrals, etc.
Assist with scheduling and follow-up with processing of reports/invoices for psychological evaluations
Liaise with HSVS Case Planning, Care Coordination, Children's Health Homes (CHH), 29-I Clinic and other staff, as well as with external NYC, NYS and community partners as assigned, to support departmental operations.
Assist departmental staff with the completion of needed actions in NextGen, including referrals to HSVS's Article 31 behavioral health clinic, CHH “Warm Hand-offs,” Comprehensive Treatment Plans, data share reconciliations, etc.
Collaborate with care coordination team to ensure foster youth being discharged have a copy of their medical record pursuant to policy.
Manage departmental mail, office supplies, etc. and perform additional administrative support tasks as assigned.
Engage in ongoing review of departmental policies, procedures, and practices to make recommendations for quality improvement.
Perform other tasks as assigned within the reasonable scope of duties of the position.

Fair Futures Coach (HS Youth Coach)

The Fair Futures Coach (HS Youth Coach) will be a full-time, trained professionals who are matched with up to 15-20 high school youth in our Prevention Program. Coaches meet young people “where they are” and build a trusting relationship with them using an authentic, trauma-informed, strength-based approach. In addition to providing weekly emotional support, Coaches help young people make progress towards their academic, career development, housing/independent living, and other life goals. When a young person is unsure of their goals/interests, Coaches help them explore and expose them to potential opportunities. They then help young people connect to schools, programs, and opportunities in line with their goals/interests, persist in those settings, and plan/prepare for the next step on their journey. Coaches celebrate young people's progress; if something doesn't work out, Coaches help young people reflect and connect to the next opportunity.
Coaches will receive training in youth engagement, coaching, and how to navigate the educational, career, and housing systems and resources in NYC. They also have access to an array of voluntary professional development supports, including 1:1 technical assistance and membership to a learning community with Coaches from other foster care agencies (see below).
We respectfully ask a minimum 1-2-year commitment to this role, given the importance of young people having consistent, caring adults in their lives, and the significant investment our team will make into supporting and developing Coaches
The High School Coach is a critical position within HSVS' American Dream Coaching Program. The Coach will provide quality, data-driven, individualized educational, employment, and housing support to HS youth. The Coach will work with the case planning team, students, families, and school staff to connect students with the services they need to raise their level of educational achievement, secure employment, prepare for housing if eligible, and ensure that key transitions receive proper attention.
The High School Coach will give particular focus to helping their youth achieve the best possible educational, and employment outcomes. The Coach will be responsible for following the Fair Futures Goals and Steps model, monitoring data, using data to help inform interventions, and inputting data into HSVS' Fair Futures Platform.
The High School Coach will be expected to create a calm, trauma-informed environment, and use a non-judgmental, strengths-based, and collaborative approach to working with the youth. The High School Coach will report directly to the Director of Youth Development.
Primary Responsibilities and Essential Functions of Position:

Participate and engage in weekly, monthly, and ongoing virtual and in-person agency-wide, Youth Development, and Education Team, ACS, Fair Futures, CUNY, FCSI, Advocates for Children, College Access and College Success, professional development, and DOE related meetings, training, events, programs, and workshops.
Maintaining relationships with school staff, and external stakeholders such as ACS, Fair Futures Coalition, and advocates for children.
Support youth with applying to scholarships, funding, employment, educational, and housing opportunities as needed.
Plan and implement engaging in-person workshops/PYA and other activities for high school youth throughout the year. Engage with young people to get them to attend.

Re-engage disconnected youth and help them re-enroll in an academic setting.
Assess the fit between the young person and their current academic setting and help them transfer into a better-fit high school or high school equivalency (HSE) program, if needed.
Visit the young person's school/program and build relationships with school staff, review transcripts and academic data, provide educational advocacy, and discuss progress towards graduation/HSE obtainment.
Ensure young people are attending school and have the supports and resources they need to be successful, such as tutoring, individualized educational plans (IEPs), 504 Plans, or other special accommodations.
Expose students to post-secondary pathways, such college and/or accredited vocational programs, starting in the 9th grade (or as early as possible).
Assist students with post-secondary planning, along with the guidance counselors and/or College Specialists and Career Development Specialists.
Connect the student to the College Specialist to apply to colleges, opportunity programs, and all forms of financial aid, funding, and scholarships.
Provide college persistence coaching, including ensuring students are receiving all of the financial and academic supports they need to succeed each semester.
Assist students with planning for apprenticeship and vocational education paths, as appropriate; Collaborate with all relevant partners, such as Case Planners, guidance counselors, Specialists, Tutors, Teachers, family members, and caregivers.
Conduct career exploration activities with young people.
Assist ALL young people in selecting at least one career development experience each year that is in line with their interests and academic situation/background.
Work with the young person and Career Development Specialist to identify potential longer-term career pathways based on the young person's interests, strengths, and academic/vocational plans.
Connect the young person to the Career Development Specialist, if needed, to explore workforce development programs, vocational programs, and for assistance with job applications.
Connect young people, as needed, to agency-based job readiness and/or internship programs (e.g., YA WORC or the Mentored Internship Program).
Provide persistence coaching to young people enrolled in any external program by checking in with them regularly.

Under the direction of the Vice President of Day Services the SR Director of Transition and Employment is responsible for the following:
Develop partnerships in the community for employment.
Work with both local and extended community corporations and businesses for both employment and internship opportunities
Work with Heartshare's Marketing and development department to create partnerships, internships and experiences.
Provide continuous oversight for all the employment programs
Ensure that the services provided are of high quality and meet the diverse physical, social, developmental, and appropriate needs of the individuals.
Ensure that there is adequate protection of each program participant's health, safety, comfort, well-being, and civil, human and legal rights.
Ensure that the program's physical plant is always clean and well-maintained. Work with appropriate personnel within HeartShare, as well as the building's landlord and outside vendors, to promptly handle physical plant issues.
Maintain a current table of organization that clearly delineates lines of authority and responsibility for program administration and oversight when supervisors are absent.
Ensure Asst. director maintains Policies & Procedures governing the day-to-day operation of the Employment Programs, including admission and discharge procedures.
Provide oversight of the smooth and effective functioning of the Program Planning Team process throughout the Employment Services Program.
Provide oversight to ensure all Heartshare policies for billing, attendance, monthlies, Pcare and precision care guidelines are followed by staff and oversees and ensures these are completed during the required time frame.
Ensures the QA plan that has been developed for that site if followed.
Responds and completes all follow up for Incidents and occurrences within the required time frame
Responds and completes all audit citations for both OPWDD and Heartshare within the given time frame

Primary Responsibilities and Essential Functions of the Position:
Program Leadership & Supervision
Provide weekly 1:1 supervision and group supervision to Peer Specialists and other assigned staff; ensure role clarity and purposeful self‑disclosure consistent with recovery-oriented practice.
Coach peers on documentation quality (person-centered, strengths-based, start/stop times), boundary management, field safety, and crisis escalation protocols.
Coordinate multidisciplinary case conferences; ensure peer interventions are reflected in treatment plans and progress notes.
Maintain coverage plans and back-up supervision; complete performance evaluations and individualized development plans.
Support workforce development: onboarding, Academy of Peer Services coursework, credential maintenance (NYCPS/FPA-C/YPA-C), and mentorship.
Clinical Services (Caseload & Intakes)
Carry a defined caseload within the MHOTRS clinic; provide evidence-informed psychotherapy appropriate to license and scope.
Conduct diagnostic evaluations/intakes for new patients; formulate person-centered treatment plans and coordinate warm handoffs to Peer Support.
Deliver services on-site, via telehealth, and in approved community settings; ensure timely follow-up after crises or transitions.
Document all encounters in the EHR within required timeframes; ensure medical necessity and alignment to OMH Part 599 standards.
Operations, Quality, and Compliance
Ensure compliance with OMH and Medicaid requirements for Peer Support and clinic services (e.g., documentation standards, co-sign requirements for pre-admission peer contacts, 15‑minute unit logic for H0038 where applicable).
Partner with Billing/Revenue Cycle to monitor denials and improve clean-claim rates; participate in denial huddles and corrective actions.
Support incident reporting and quality improvement; participate in audits, chart reviews, and outcomes monitoring.

Assist and advocate for families and children in the attainment and maintenance of public benefits such as financial, educational, social, and community services.
Assess responsible transition of client service into and out of Health Home care, between child and adult health homes, and between inpatient and community care as appropriate.
Participate in mandatory and optional training, including but not limited to: CANS-NY (achieving a score of 70 or higher), Documentation, Mandated Reporter, Motivational interviewing.
Participate bi- monthly Case Specific Supervision with Children's Health Home Supervisor/Director of Care Management to discuss Case load, barriers and successes.
Maintain quality assurance systems to ensure that services are in place for clients and documentation is complete and up to date. Participate in quality assurance program.
Independently manage day-to-day working relationship with external partners including consistent field visits, effective email correspondence and monthly feedback loop that tracks referrals and troubleshoots customer service issues with a sense of urgency.
Develop strategies to meet enrollment targets and with the ability to execute client and community engagement activities that are compelling and successful with effective follow-through.
Complete outreach to clients to enroll into services if there is a decrease in current caseload.
Adhere to all government and funder regulations.

Counsels' individuals and/or families related to psychological or emotional stressors such as substance abuse, self-regulation, or family settings
Establishes permanency and treatment goals for children and youth, utilizing historical and current available information in collaboration with the multi-disciplinary team.
Assist children and youth to develop skills as defined by permanency and treatment goals.
Provides emotional and behavioral support and therapy focused on healing and permanence.
Provide individual therapy, substance abuse counseling and treatment, trauma-competent therapy, family and group therapy, transitional support with a focus on healing and permanence.
Promoting integration with community resources and skill building.
Providing psychoeducation, wellness education and all other assigned training presentations.
Taking lead on mental health and trauma learning and training for the agency.
Communicating with family, case planning staff, all relevant stakeholders and medical practitioners.
Maintains contact with collateral resources where indicated, including but not limited to referral sources, schools, foster care providers, and hospitals.
Explains the function of behaviors to group and foster home staff.
Uses interventions drawn from evidence-informed behavioral health strategies.
Structures interventions to understand maladaptive behavior and increase developmentally appropriate responses that lead to healing opportunities.
Interviews clients to obtain information concerning mental health history or other pertinent information.
Observes clients to build understandings and insights of children and youth's behaviors.
Provides individualistic and holistic interventions
Discusses progress toward goals and permanence with client, such as self-regulation, self-awareness, stress, or substance abuse.
Consults with medical doctor or other specialists concerning treatment plan and amends plan as appropriate
Responsible for the engagement, retention and outreach to clients.
Completes all required documentation, including but not limited to session notes, treatment plans, psychosocial summaries, billing documentation, and client correspondence.
Refers client to supportive services to supplement therapy and permanency plans.
Conducts monthly family outreach searches using search platforms to help reconnect youth with family.
Performs other related duties as assigned, within the scope of the role.
Job Behavioral Expectations:
Maintain regular consistent and professional attendance, punctuality, personal appearance, and adherence to relevant health & safety procedures.
Safe and successful performance, including meeting productivity standards.
Maintain an understanding of the principles, methods and processes needed to perform the job. Attends staff meetings, seminars and in-service training as assigned.
Demonstrates the ability to complete work in an acceptable manner by the time and/or date established with accuracy, neatness and thoroughness and minimal amount of errors.
Maintain established departmental policies and procedures including maintain strict confidentiality and adherence to all HIPAA guidelines at all times.
Be able to complete Mandated Reporting self-directed online training.
Qualifications
Master's degree with a minimum of 2 years' experience in children's mental health, addiction, and/or foster care/child welfare/juvenile justice.
Required LMHC, MHC-LP or LMSW.
Have a valid driver's license and dependable transportation.
Have experience working with children (preference given to those with experience working with children with special needs).
