The Goal of the Community Habilitation Worker is to provide assistance and support to individuals within their local communities. Services must either start or end within the individual's home. The Community Habilitation Worker must assist in the preparation and performance of support services outlined within the Community Habilitation Plan. This includes learning and understanding how to meet their mobility, dietary, medical, and behavioral needs.
