Contractors have printed county set of plans with each crew. 2. Contractors have printed county approved permit with each crew. 3. All trucks are identifiable. 4. Contractors are following the county approved set of plans. Working conditions: 1. Make sure contractors are not boring or excavating with machinery on Fridays. 2. Ensure contractors are hand digging within both the public rights-of-way, public drainage easements, and public utility and drainage easements to locate all underground utilities, drainage pipes, structures, etc. prior to horizontal directional drilling. Hand digging is needed to locate all existing underground utilities and stormwater improvements including but not limited to: * Sewer mains and structures * Sewer laterals * Gas mains/ laterals * Fiber lines * Power lines * Watermains and valves/structures * Water service connections * Storm pipe and structures 3. Check to ensure contractors do not have any open pits/ holes left unattended. (If holes are left unattended have the crew stop working and safe up the pits/ holes with the orange safety fence and caps, fill the holes or pits in or have someone staying there while pits and holes are open) 4. Ensure there will be no open pits or holes left open over night; they all need to be filled in before they leave for the day. Signs are set up correctly. 4. All vehicles, trailers and equipment are in the MOT area and coned off properly. 5. Make sure any thing in the roadway has proper MOT, if more than one MOT is needed, they need to correct it before continuing to work. 6. All flaggers are present and flagging traffic using the stop & go paddles. (Make sure flaggers are not in the trucks, sitting down and are paying attention) 7. If shoulder work is being done, ensure that there is no vehicles, trailers and equipment in the roadway and is 2 feet or more away from the edge of the road and signs are in place and cones are along the working area. 5. Check to ensure the contractors are following the county approved set of plans, this includes bore paths, depths and hand holes are to plan. 6. Check that the contractors are maintaining the proper separation of all utilities. 7. You should have contractors spot dig an area to ensure the bore is in the correct location and is at proper depth. 8. Check to ensure contractors are installing the correct size hand holes. 9. Check to ensure hand holes are not being placed in the bottom of a ditch or swale. 10. Ensure the contractors are installing hand holes correctly: This includes min 6inches of rock compacted first, then the hand hole set in place, area around hand hole is properly compacted, hand holes are flush with egesting ground and bolts are installed and tightened down to ensure safety. 11. Ensure contractors are picking up their trash, cut off pieces of pipes, and piles of dirt as they go and at the end of the day. Field Changes: AT&T as the permit holder shall be involved with all field changes. 1. All field changes needed will have to be sent to BlueStreak first, BlueStreak will go through AT&T (Mike) and if AT&T (mike) is good with it they will get ahold of Nick Dees to meet and get changes approved.
