The Navigation Housing Case Manager provides case management services for Recovery Oriented Housing Program (ROHP) participants. ROHP participants are identified to be currently experiencing or at risk of chronic homelessness, high utilizers of publicly funded systems, possibly disengaged from services and in need of diverse supportive services. This role is tasked with identifying all types of barriers which may complicate a participant's ability to obtain and maintain housing, and use creative problem solving to address these issues. This Case Management position includes broad case review, scheduling and outreach, and various other supports to monitor client progress and ensure that ROHP outcome measures are met. This position works closely with Navigation's Housing Specialists, Facilities, therapists and prescribers, to identify and overcome barriers which complicate a resident's ability to remain housed, keep files compliant, and is a member of our Clinical Housing team. Essential Duties/Responsibilities: * Provides case management to voucher residents to include community-based services when needed, and will carry an average caseload of 20 clients. * Conducts outreach and engagement to residents identified as needing support to maintain housing. * Identifies potential barriers to maintaining housing and engages residents in goal setting, monitoring progress. Actively coordinates care with Jefferson Center, and other community agencies and health providers. Maintains appropriate professional standards and provides appropriate follow-up for consumers. * Identifies risk factors including lethality, and responds accordingly. * Provides needs assessment for living expenses and other needs such as food, clothing, etc., in addition to life skills education. * Responsible for the coordination and/or completion of needed paperwork including releases of information, HMIS intake and assessment, VI-SPDAT assessments, Homeless Prevention Demographics Profiles, and other needed documentation. * Maintains a positive focus and excellent customer service skills towards consumers, Jefferson Center staff, and the community. Works from a strength based, recovery based perspective and takes cultural considerations into account when appropriate. Effectively responds to client/consumer needs and problems, initiates and maintains positive interactions, timely response to phone calls, email and other requests. * Productivity/Documentation and Outcome Data: Documents all contacts according to Jefferson Center and Substance Use Outpatient Program standards while meeting productivity standards as identified by the Program Director. Other Duties: * Exhibits enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment. * Maintains effective interpersonal relationships with consumers, peers, subordinates, upper management, visitors and the general public. * Ability to multi-task, prioritize concerns, and effectively manage time independently. * Attends mandatory in-services, compliance with individualized training plan if required. * Participates in supervision by coming prepared with an agenda. Reports high risk/problem cases, and utilizes a problem solving approach as well as feedback. Attends supervision at times and intervals agreed upon with supervisor. * Submits 90% of all SRFs/Progress-to-Date forms within 3 working days (team specific). * Completes 95% of all paperwork by the due date. * Meets required number of consumer service hours or other measures of productivity as determined by Program Director. * Works cooperatively with other community agencies, as appropriate, and in agreement with supervisor. Volunteers to work on Center committees and/or task forces. * Participate in Evidenced Based Practices and other agency initiatives including Trauma Informed Care, Whole Health, Motivational Interviewing, Solution-Focused as well as other best practices. Required Education, Knowledge, Skills, & Experience: * Conferred Bachelor's degree (from an accredited University/College) in psychology, social work, or related field required. * At least two years of related substance use and co-occurring mental health experience preferred. * Certified Addition Technician (CAT) or Certified Addiction Specialist (CAS) preferred. * Experience with motivational interviewing and harm reduction models preferred. * Good communication and organizational skills. * Computer skills such as proficiency utilizing email and data entry into an electronic health records system. * Experience with Homeless Management Information System (HMIS). * Knowledge of Fair Housing. * Bilingual (English/Spanish) preferred * Motor Vehicle Insurance Coverage requires applicant must be 25 years or older. Must have an acceptable Motor Vehicle Driving record as determine by our insurance carrier. An MVR will be pulled along with a background check. The Admissions Specialist is responsible for reviewing and coordinating admissions for Jefferson Center's Children's Crisis Stabilization Unit (CSU). They implement efficient admissions procedures and processes that are customer friendly to children/youth, parents/guardians, and the referral source. They will interact with both outside agencies and with team members with diplomacy, respect and professionalism. We are looking for day and night shift coverage. Day shift: 7am to 7pm. Night shift 7pm to 7am. Jefferson Center's Children's Crisis Stabilization Unit (CSU)'s mission is to use an interdisciplinary team of clinicians, residential counselors, medical staff and admissions staff to provide brief, 3-5 day, crisis stabilization treatment for children and youth with behavioral health issues, including depression, suicidal thoughts, other trauma reactions, and family issues. Our team coordinates treatment and provides tools and safety planning for children/youth and families so that they can safely return to their homes and the community. We are committed to serving Colorado's children/youth while maintaining a non-judgmental and inclusive approach. Essential Duties: Monitor and coordinate timely admission-decisions and admissions of children/youth for the Crisis Stabilization Unit to promote safe and appropriate admissions while maximizing capacity. Admissions decisions should be made and communicated to the referral source in 30 minutes or less, except for cases with unusual complexity. Greet clients and visitors upon arrival, sign them in, obtain insurance information and any other information needed, process payments for copays & facilitate signing of consents. Answer multi-line phones and direct caller to the appropriate party or handle the caller's needs directly, demonstrating courteous and professional communication and prioritizing confidentiality. Serve as admissions liaison between all stakeholders to facilitate efficient, professional communication and problem-solving activities. Acquire pre-authorization and collect insurance information, including verifying Medicaid Eligibility. Enter incoming child/youth's information into the electronic medical records (EMR.) Also provide accurate and detailed billing information in EMR, to include documenting primary and secondary insurance information. Schedule transport for new clients, if needed. Assemble client's chart and distribute intake information to key individuals. Accurately and concisely complete the Census Report for the end of each shift. Essential Duties: In addition to the primary role in treating and caring for consumers, responsibilities also include educating consumers and the public about a variety of medical conditions related to substance use such as HIV, TB and Hepatitis C as well as providing emotional support and advice to the families of their consumers. Provide injections for clients under direction from prescribing authority and medical team. Complete UA's as needed. Perform routine medical procedures (vitals, including blood pressure, oxygen levels, temperature, blood alcohol levels, drug testing, pregnancy testing. Collaborate with local public health departments in the effort to prevent harm from substance use, including needle distribution/recovery. Individual will distribute sterile needles and other harm reduction supplies, recover used needles and other supplies, and provide information and containers for their safe disposal. Assist in the coordination of HIV, TB, Hepatitis and other infectious disease screening and referral for testing to the local health departments. Collaborate with the telehealth psychiatrist and other Jefferson Center programs. Engage child and adult consumers and their families in the treatment process and goal-setting. Identify risk factors including lethality for suicide, homicidal and/or grave disability. Work constructively with consumers and the MAT team to reach agreed upon outcomes, and coordinate care with internal and external providers. As appropriate, complete involuntary mental health holds (27-65) according to Center protocol. Demonstrates knowledge and skills to develop therapeutic alliance with consumers and to work effectively and with cultural competence with consumers from diverse backgrounds. Coordinate prescription and PAP refills; respond to all refill requests within 3 business days. Review and order labs, notify prescriber of any abnormal labs requiring immediate attention. Assist prescribers in tracking and meeting timeframes for annual labs and AIMs Ensure compliance with OBH, Signal and OSHA regulations. Coordinate medication processing and applications with PAP office. Order/stock and reconcile the medication cabinets and order medical supplies for the mobile unit as needed. Provide consumers with (MAT) and other medication education. Monitor consumers for side effects of meds, including AIMS testing or any medical condition that may affect their mental illness. Monitor and review consumers' charts for medication/medical issues with appropriate follow-up. Coordination of care with consumers' prescribers, including documentation of medical medications & dosing, physical diagnoses, monitoring of potential medication interactions with MAT and/or psychiatric medications, and referrals as indicated. Ensure Medicaid eligibility and submit prior authorizations to consumers' insurance in a timely fashion; address medication changes with prescribers as needed.
